    OVERVIEW
    The Licensed Practical Nurse (LPN) will be part of our clinical care navigator team. They will be responsible for day-to-day front and back-office activities. Such activities include regular correspondence between our staffed physicians, our patient population, and our care navigation team. Our LPNs provide remote patient care by monitoring vitals and keeping regular contact with their patients on a weekly basis. They assist the physician in conducting telehealth appointments and documenting outcomes to ensure patient success. Additionally, they collaborate with our Care Navigators to perform new patient intakes, home assessments, and serve as a clinical resource for the Care Navigator Team.
