Position Summary
Desnol Healthcare Services is seeking an experienced Registered Nurse / Designated Manager to provide clinical, operational, and regulatory leadership for Community Residential Services supporting individuals with mental health needs. This role is designed for a licensed RN who can lead residential services with strong clinical judgment, calm decision-making, staff accountability, documentation oversight, and Minnesota 245D compliance awareness.
The successful candidate will oversee day-to-day program operations, support person-centered service delivery, supervise and coach direct support staff, review clinical and behavioral documentation, coordinate with interdisciplinary partners, and ensure that services are delivered in a safe, ethical, compliant, and resident-centered manner. This position requires a leader who can protect client rights, maintain licensing readiness, identify risk early, and reduce unnecessary executive escalation through competent independent judgment.
Why This Role MattersThis position is not a standard bedside RN role. It is a leadership role at the intersection of nursing practice, mental health residential services, staff supervision, compliance, documentation quality, client safety, and program accountability. The Registered Nurse / Designated Manager must be able to translate clinical knowledge into practical residential support, ensure staff follow approved support plans and policies, and maintain strong communication with providers, case managers, guardians, families, and regulatory stakeholders as appropriate.
Primary ResponsibilitiesClinical and Mental Health Oversight· Provide RN-level clinical oversight within the scope of Minnesota nursing practice and applicable program requirements.
· Review resident health, mental health, behavioral, medication-related, and safety concerns and determine appropriate follow-up.
· Coordinate care with providers, pharmacies, case managers, guardians, therapists, and other members of the support team as appropriate.
· Monitor changes in condition, behavioral escalation patterns, medication refusal, crisis indicators, hospital discharge needs, and follow-up requirements.
· Support development, review, and implementation of individualized service plans, support plans, crisis plans, and health-related protocols as applicable.
· Provide direct care that cannot be delegated.
245D/CRS Program Leadership and Compliance· Provide program oversight consistent with Minnesota 245D expectations, CRS licensing requirements, internal policies, and person-centered service standards.
· Maintain licensing readiness through accurate records, timely follow-up, staff training verification, incident review, and corrective action tracking.
· Ensure services are delivered in a manner that protects client rights, dignity, privacy, choice, health, safety, and community inclusion.
· Participate in internal audits, quality assurance activities, policy implementation, and preparation for external reviews or licensing visits.
· Identify compliance gaps early and implement corrective actions before issues become repeated deficiencies.
Staff Supervision, Training, and Accountability· Supervise, coach, and support direct care staff in daily service delivery, documentation, resident interaction, and crisis response expectations.
· Ensure staff understand and follow support plans, medication-related procedures, incident reporting expectations, emergency procedures, and resident-specific protocols.
· Assist with onboarding, competency verification, ongoing training, performance feedback, and corrective coaching.
· Promote staff retention through clear expectations, consistent communication, timely support, and accountability-based leadership.
· Address performance concerns promptly and escalate serious employment, safety, or compliance issues through the proper leadership channels.
Documentation, Incident Review, and Quality Assurance· Review daily notes, behavior logs, medication records, incident reports, appointment documentation, and follow-up records for accuracy and completeness.
· Ensure documentation supports services delivered and reflects resident needs, interventions, outcomes, and required follow-up.
· Lead or participate in incident review, root-cause review, trend identification, corrective action planning, and staff retraining when needed.
· Track documentation quality indicators and ensure records are audit-ready and timely.
· Use data, documentation trends, and staff reports to identify patterns that may require clinical review, service-plan updates, or team communication.
Operations, Occupancy, and Communication· Support stable program operations, including staffing coordination, resident safety, service continuity, appointment follow-through, and communication flow.
· Assist with occupancy-related readiness, admission coordination, service-start preparation, and transition planning for new residents as assigned.
· Communicate professionally with external stakeholders, including case managers, guardians, providers, families, and county or state representatives as appropriate.
· Reduce unnecessary executive escalation by resolving routine operational, clinical, and compliance matters within assigned authority.
· Provide timely updates to executive leadership regarding risk, staffing concerns, compliance matters, resident needs, and operational priorities.
Required Qualifications· Active Registered Nurse (RN) license in good standing in the State of Minnesota.
· Experience working with adults with mental illness, behavioral health needs, intellectual/developmental disabilities, or complex psychosocial needs.
· Demonstrated leadership experience in a healthcare, residential, assisted living, behavioral health, disability services, or similar regulated setting.
· Working knowledge of documentation standards, medication-related oversight, incident response, resident rights, and person-centered service delivery.
· Ability to supervise staff, coach performance, review documentation, identify risk, and follow through on corrective action.
· Strong written and verbal communication skills with the ability to interact professionally with residents, staff, providers, case managers, guardians, families, and regulators.
· Ability to pass required background studies and meet all applicable employment and licensing-related requirements.
Preferred Qualifications· Prior experience in Minnesota 245D services, Community Residential Services (CRS), intensive residential support, group home operations, assisted living, or residential mental health programs.
· Experience as a Designated Manager, Program Director, RN Supervisor, Clinical Manager, Director of Nursing, Assisted Living Director, or comparable leadership role.
· Knowledge of DHS licensing expectations, service planning, positive supports, crisis planning, incident reporting, and staff competency documentation.
· Experience improving documentation quality, staff retention, occupancy readiness, audit readiness, or corrective action systems.
· Comfort using electronic records, spreadsheets, scheduling systems, and AI-assisted tools for drafting, summarizing, trend review, and workflow organization while maintaining human review and accountability.
Compensation and Bonus StructureCompensation Element
Description
Base Salary
$105,000-$110,000 annually, depending on experience and demonstrated fit for the role.
Probationary Period
Bonus eligibility begins after successful completion of the probationary period and satisfactory demonstration of leadership, compliance, and documentation competencies.
Annual Performance Bonus
Up to $5,000-$7,500 annually based on compliance performance, occupancy support, documentation quality, staff retention, and reduced executive escalation.
Mileage/Work Expenses
Eligible work-related mileage or approved expenses may be reimbursed according to company policy.

Work Environment and Expectations· This role requires presence in a residential services environment and regular interaction with residents, staff, and external service partners.
· The position may require flexibility to respond to urgent resident, staffing, incident, or compliance concerns.

Mental Health Technician/ Mental Health Support Specialist
Work Shift/Work Hours: Generally, shifts are 7:00 a.m. to 3:00 p.m. and 3:00 p.m. to 11 p.m.
Days of work: Varies; may include weekends and holidays
Travel Required: Yes (Transport and accompany individuals to medical appointments, community outings, recreational activities, and spiritual events)

Job Summary:
Desnol Healthcare Services is seeking a dedicated professional to support individuals in our residential home for individuals with intellectual disabilities, mental health diagnoses, substance abuse disorders, and brain injuries. Desnol seeks passionate, caring, hardworking individuals with integrity to join our team. You will work alongside fellow caregivers to provide high-quality, patient-centered services.
Responsibilities:
· Provide courteous and respectful care to all residents.
· Spend most of each shift interacting with people served, developing relationships, building rapport with residents, and performing tasks that are directly related to the care of the residents.
· Provide input in staff meetings and treatment planning.
· Monitor the residents regularly as per each resident’s care plans.
· Responsible for knowledge and implementation of the resident’s support plan.
· Remain current on resident’s plans and observation notes completed by other staff.
· Encourage and monitor support plan compliance.
· Utilizes crisis assessment tools for suicidal and aggressive behavior.
· Plan and coordinate recreational/social activities.
· Identify individuals prone to having behavior emergencies and recommend additional programming appropriate for their needs.
· Help defuse and de-escalate any potential behavior emergencies.
· Assist residents with cleaning their rooms, maintaining personal hygiene, and doing laundry.
· Assist residents in independent living skills; hygiene checks, housekeeping, budgeting, etc.
· Administer medications according to Desnol Healthcare Services policy and procedures under the supervision of the facility’s nurse.
· Participate in routine cleaning duties.
· Inform other staff and staff lead when the quantity of residents' medications is low.
· Order resident medication as needed.
· Inform the lead and nurse when medication changes occur.
· Produce and distribute incident reports within (8) hours of the relevant event, as needed.
· Develop weekly menus and shopping lists and do grocery shopping for the facility, encouraging participation by residents.
· Schedule resident appointments as needed and escort residents to their appointments.
· Plan holiday activities and special events.
· Prepare meals, serve, and clean up according to food safety standards.
· Assist in training new staff.
· Provided staff coverage and ensured the safety of the residents and the facility.
· Responsible for securing coverage of any missed shifts, planned or unplanned.
· Perform light house maintenance or request maintenance assistance as needed.
· Must be able to transport the residents in the community.
Qualifications/Requirements:
· Must pass the DHS background study
· High School diploma
Must be eligible to work in the U.S· Dependable
· Ability to complete the Desnol training program and successfully pass an associated competency test and pass a competency test to administer medication
· Must be able to perform the essential functions of the job safely
· Must have a valid driver’s license and be able to drive residents.
· Sensitivity to resident needs and the ability to build positive relationships and rapport with residents
· Current CPR / First Aid certification is required; if not currently certified, we will give 30 days to complete at your cost
· Ability to work well in teams
Preferred:
Experience working with individuals with mental illness.
Experience working in Adult Foster Care Homes (Family or Corporate).
