This opportunity involves providing in-home and community-based behavioral health services to youth with IDD Dioagnosis. Services are primarily delivered in Princeton, Middlesex, Warren, Burlington County, Somerset and other counties.
Key Responsibilities Include:
* Provides in-home and community-based outreach services to assigned youth.
* Assists in the creation and implementation of individualized behavioral interventions.
* Maintains a caseload and delivers consistent case management support.
* Completes timely, accurate documentation, including progress notes and care plans.
* Participates in the development and monitoring of Individualized Service Plans (ISPs).
* Facilitates skills-focused sessions and therapeutic activities for youth and families.
* Conducts outreach to re-engage families and youth who miss scheduled appointments.
* Collaborates with multidisciplinary team members and external service providers.
* Builds and maintains community relationships to enhance service linkage and referrals.
