Job DescriptionJob DescriptionWe are seeking a compassionate and dedicated Case Manager/QMHP to join our team. The ideal candidate will have a strong background in case management and a passion for helping individuals in the behavioral health field.
Qualified candidates MUST have a minimum of a Bachelor's a degree in: Psychology, Social Work, Sociology, Psychology, or Counseling.
Duties/ Responsibilities
• Conduct thorough assessments of clients’ needs and circumstances.
• Develop and implement individualized case management plans.
• Provide behavioral support to clients.
• Collect data to monitor client progress and outcomes.
• Coordinate care with other professionals, including healthcare providers and educators.
• Advocate for clients’ needs within the community and healthcare systems.
• Maintain accurate documentation of client interactions and case progress.
Minimum Skills /Requirements Bachelors Degree or higher in psychology, social work, sociology, educational psychology, or counselingGood listening, verbal, and written communication skillsGood work ethic to complete tasks independentlyFunctional knowledge of computer programs (Word, Excel, Web based programs)Must meet state requirements for QMHP status (CANS and ANSA Cert)Experience working with youth or families in a behavioral health settingGood driving record with at least 3 years of driving historyReliable vehicle with insuranceBilingual in English and Spanish preferred but not requiredJob Type: Full-time/ Contractor
