    POSITION SUMMARY:

    The Registered Nurse Case Manager in the Living with HIV department is responsible for coordination of medical interventions through specialty clinics and community/residential site visits, and promotion of optimal health through monitoring and interventions. The RN Case Manager applies a holistic approach to the coordination of client services. The RN Case Manager reports directly to the Medical Unit Manager.

    ESSENTIAL FUNCTIONS:

    Accountable for demonstrating the qualities outlined in the “Customer Service Standards of Excellence” program when interacting with patients, their families and friends, and fellow employees.Understands and complies with Health Resources Services Administration (HRSA) requirements under the Ryan White Grant.Describes program and clinic services to clients and completes care plans per standards of care.Coordinates patients to receive care within the specialty clinic and other community agencies.Performs nursing assessments.Provides nursing care using appropriate clinical judgment and critical thinking, while following organization protocols.Collaborates with the provider to determine the best medical/nursing interventions.Provides health education instructions to patients based on the nursing assessment and provider orders.Develops a multidisciplinary plan of care.Develops an understanding of the quality assurance and quality improvement components of patient care.Dispenses over the counter medications following organizational policies.Dispenses prescription medications following provider orders.Provides an atmosphere of professionalism, privacy and confidentiality in clinical area.Assesses clients need for other services and assists patients by making appropriate referrals.Demonstrates knowledge of and interventions for complex psychosocial issues.Maintains current information on community resources and linkages.Keeps accurate records.Participates in team staffing and meetings.Acts as a medical/nursing resource for other team members.Participates in educational trainings relevant to HIV care and the Ryan White Grant as deemed necessary by the Medical Unit Manager.Other duties as assigned by Medical Unit Manager or designee.

    EDUCATION/EXPERIENCE/TRAINING:

    Education:

    Experience:

    Ambulatory care, hospital, or public health requiredUtilization of an electronic medical record requiredOne year of registered nurse experience in direct patient care preferred

    POSITION SUMMARY:

    This is a direct service position with Crusader Community Health (CCH) that provides services in the clinic and in the community and within the context of the Integrated Primary Care model. The Behavioral Health Case Manager works with patients (child, adolescent, adult) with behavioral health needs, including mental health, substance use, and psychosocial issues. The Behavioral Health Case Manager works in conjunction with the provider(s), patient, family, specialty services, community entities and managed care organizations. The Case Manager will be responsible for the provision of services including comprehensive case management, behavioral health counseling and education, patient and family skills training and support, health promotion, transitional care, as well as advocacy, linkage, coordination, collaboration and referral to/with community services and agencies. This position reports to the Chair of Behavioral Health.

    EDUCATION/EXPERIENCE REQUIREMENTS:

We take pride in delivering exceptional Women’s Health Services and are seeking an experienced and caring Registered Nurse to join our Women's Health Services team. If you are passionate about maternal health, patient education, and providing meaningful support during pregnancy, we would love to meet you.

    Position Summary

    As a Women’s Health RN, you will provide direct patient care to routine and high-risk obstetric patients. You will perform and interpret fetal monitoring, support care coordination, educate patients on their care plans, and collaborate with a multidisciplinary healthcare team. This role is essential in promoting optimal health outcomes for our pregnant patients and ensuring high-quality, compassionate care.

    Key Responsibilities

    Perform NSTs, fetal heart rate monitoring, and evaluate fetal status.

    Recognize and respond appropriately to non-reassuring fetal heart rate patterns.

    Assess, develop, implement, and evaluate individualized patient care plans.

    Identify high-risk factors and promptly notify appropriate providers.

    Provide patient education and offer psychosocial support during clinical visits.

    Maintain accurate documentation of assessments, interventions, and outcomes.

    Assist providers with procedures and coordinate patient flow within the unit.

    Adhere to nursing standards, state regulations, and organizational policies.

    Promote a positive, professional, and patient-focused environment.

    Perform routine clinical support activities, including equipment operation, room readiness, and sterilization processes.

    Qualifications

    Required:

    Graduate of an accredited school of Nursing

    Current Illinois Registered Professional Nurse (RN) license

    Current CPR certification

    Strong computer proficiency, including Microsoft Office

    Ability to work collaboratively in a team-based environment

    Excellent communication, critical-thinking, and organizational skills

    Preferred:

    Completion of a Basic Fetal Monitoring course (basic, intermediate, or certification); ongoing education every 1–2 years

    Experience in obstetrics, ambulatory care, hospital, or public health settings

    Experience with Electronic Health Records (EHR)

    Bilingual fluency (written and verbal)
