    Job DescriptionJob Description

    CHCA is seeking an experienced Bilingual Client Services Coordinator, with exceptional customer service skills, energetic, compassionate, and able to work in high pressure environments.

     

    As a Client Services Coordinator, your position will include but is not limited to:

     

    Customer Service

    Answers and responds to all incoming calls from clients, client’s family members, HCWs and or health plan in a professional mannerCommunicates with health plans in a timely mannerResponds to messages received through the agency’s ticketing system in a timely mannerProvides ongoing support to the Home Care Workers using a Coaching approach to communication

     

    Client Services Coordination

    Proactively manages caseload and provides support to team members with case coverageResponsible for scheduling Home Care Workers (HCWs) to clients to provide the highest quality service to clientsWorks diligently to match HCW with client to establish a strong connection between worker and clientAct as a liaison between clients and scheduled HCW to communicate changes and needs on a timely basisCommunicates with clients and their families regarding appointment scheduling, appointment changes, aide no shows and/or cancellationsArranges for case coverage for permanent schedules, emergency replacements, weekends, holidays, leave of absences, and vacation periodsProvides HCWs with an orientation to the client’s needs during case assignment Responds to new patient intake inquiries from the Business Development Department in a timely mannerParticipates in the agency’s weekend staffing and on-call rotating scheduleCommunicates any observations of changes in patient’s clinical status received to the appropriate departmentCommunicates any reports of Emergency Department visits/hospital admissions to the appropriate department

     

    Workforce Management

    Responds to communication received from the Compliance Department regarding HCWs who have not fulfilled their compliance requirementsInforms the Human Resources Department of any HCW reporting a work-related injuryInforms the Human Resources Department when HCWs have been out of work sick for more than 7 daysReviews the Overtime Report and proactively attempts to reduce HCW OvertimeApproves HCW vacation requestsOrients HCWs assigned to work on “Sleep-in Cases” to the agency’s documentation requirements for these casesResponds to all reports received by Client Services Leadership Team that are used to manage the workforce.

     Knowledge, Skills, and Abilities

    Strong interpersonal and communication skillsSeek and accept supervision and guidance; comfortable asking questionsProactively reports outcomes and issues to managerAccepting of feedbackSelf-starter, motivated, creative problem solver and takes initiativeStrong team playerOrganize, prioritize work, and meet deadlinesAble to deal with difficult or sensitive situations in person or on the phoneExcellent customer service having empathy and patience in working for older adults and people with disabilitiesAble to multitask and work in an extremely fast paced environment

      

    Minimum Qualifications

    Two years of experience in the Health Care field and/or Social ServicesBilingual (English/Spanish) preferredStrong command of Microsoft Office SuiteFlexibility in work hours/schedule

    RN Nurse Instructor / Home Care Educator

    Queens & Staten Island

     

    Status: Hybrid/Full-Time                             

    Salary: $100,000

     

    Cooperative Home Care Associates is seeking an experienced and motivated Registered Nurse (RN) to join our team as a Nurse Instructor/Home Care Educator supporting workforce development and training initiatives throughout Queens and Staten Island. 

     

    This is a hybrid position combining remote instruction with scheduled in-person training and competency sessions. 

     

    This role is ideal for an RN with home care experience who enjoys teaching, mentoring, and supporting the development of Home Health Aides and Personal Care Aides in a community-based setting.

     

    Hybrid Training Structure

    This position follows a hybrid training model that includes a combination of remote teaching and scheduled in-person instruction. Instructors teach virtual classes remotely on designated online training days and conduct onsite hands-on training, skills demonstrations, and competency evaluations as required by each program. Training schedules vary depending on the program and borough assignment.

     

    Key Responsibilities

    Conduct Home Health Aide (HHA) and Personal Care Aide (PCA) trainingsConduct skills demonstrations and competency evaluationsProvide educational support and instruction to trainees and staffMaintain training records and required documentationTravel to scheduled training and competency locations in Queens and Staten IslandSupport workforce development and grant-funded training initiativesCollaborate with clinical and training leadership teams

     

    Minimum Qualifications

    Current New York State Registered Nurse (RN) license requiredMinimum of two (2) years’ experience as a Registered Professional NurseAt least one (1) year of experience in the provision of Home Health Care Services requiredStrong communication and organizational skillsPrior teaching, training, or staff education experience preferredAbility to work independently and manage scheduled travel between training sitesBilingual in Spanish Preferred
