The LPN/LVN provides professional nursing care to patients within an assigned unit, clinic or region.
    You will work on-site at the Conviva De Leon Care Center.
    You will report directly to the Area Operations Director.

    Job Functions

    The LPN/LVN assesses patient health problems and needs.You will take vital signs, administer medication, develop, and implement nursing care plans, and maintain medical records.You will advise patients on health maintenance and disease prevention or provide case management.You will need to understand own work area professional concepts/standards, regulations, strategies, and operating standards.

    Required Qualifications

    You will have a current licensure as an LPN or LVNThis role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

You will report to the Associate Medical Director for Corpus Christi Market and float amongst clinics in the region. As a Nurse Practitioner with Conviva You will provide health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. Prescribe medication, examine patients, diagnoses illnesses, and provide treatment. Advise executives to develop functional strategies (often segment specific) on matters of significance.
