    PROGRAM MANAGER:

    Conifer Park is looking for a Program Manager to join our Inpatient clinic in Glenville, NY. In this role, you will focus on maintaining a therapeutic program for patients while developing and implementing, as well as evaluating and reporting, on program goals and objectives. You will plan, schedule and coordinate the delivery of services for the assigned program in cooperation with all other relevant departments for all shifts as well as managing all program changes as needed.


    DUTIES AND RESPONSIBILITIES:


    Ensures that patients receive individualized care and attention. Manages patient census effectively. Provides timely resolution of patient concerns & complaints (patient-patient; patient-staff). Maintains a well-coordinated provision of interdisciplinary care for patients before, during and after discharge. Coordinates the timely delivery of specialized services for patients from other relevant departments and disciplines (medical, nursing, psychiatry, activities, dietary). Participates with Intake Department and Admissions Department to ensure the efficient and timely admission of patients. Provides individual and group supervision to assigned staff, covering both clinical skills and administrative issues. Ensures adequate numbers and presence of properly credentialed staff at all times. Develop, deliver and ensure staff participates in and all mandatory trainings. Provides timely feedback, praise and discipline to staff (working cooperatively with HR & the Union). Continually strives to improve the quality of service to provide excellence in patient care. Continually monitors and, along with staff, refines the clinical program to meet the needs of the patients (using chart audits, patient & referent satisfaction surveys, staff competency reviews, etc). Maintains current knowledge of OASAS regulations and JCAHO standards. Ensures staff and program maintain compliance with OASAS/JCAHO (particularly documentation). Ensures that all relevant staff are aware of individual patient care issues, inter-departmental developments and organizational policy. Holds regularly scheduled staff meetings and shares information from other departments. Maintains current policies/procedures for staff on the unit(s) and ensures staff awareness of policies. Maintains a confidential environment in which both patients and staff feel respected and safe.

    Requirements:

    REQUIREMENTS:

    Bachelor's Degree || QHP Such as CASAC, LMHC, LMSW...

    Experience on related field.

    Basic PC skills desirable (spreadsheet and word processing)

In this role, the LPN collaborates with the RN in the implementation of a total health care regime, provides data to assist in the evaluation of patients' responses to plan of care, and administers medication safely and accurately while working closely with Medical and Clinical staff.


    Schedule Options:
    SCHEDULES:Days: 2 Weeks Rotation 7:00am - 3:30pmEvenings: 2 Weeks rotation 3:00pm - 11:30pmOvernights: 2 Weeks Rotation 7:00pm - 7:30am


    SIGN-ON BONUS: $6,000 (Based on 40 hours, will be prorated based on hours): $1,500 after completion of the orientation period, $1,500 after six (6) months of continuous employment, and $3,000 after one (1) year of continuous employment.

    Conifer Park Inpatient Clinic in Glenville, NY is currently seeking a Nurse Practitioner. As Nurse Practitioner with Conifer Park Inpatient, you will be responsible for Performing comprehensive medical history and examination and records initial diagnosis in patient medical record; Administers or orders diagnostic tests, such as x-ray, electrocardiogram, phlebotomy and other laboratory tests; reviews and interprets test results; Assesses for the appropriateness of medication assisted treatments for substance use disorders, including but not limited to, buprenorphine, methadone, naltrexone, acamprosate and disulfiram. Provides induction, monitoring, titrating, maintenance, and tapering as clinically appropriate, Evaluation and treatment of medical emergencies. Emergency evaluation referral of unstable patients to appropriate medical facilities; Maintains training in the use of naloxone and emergency first aid response techniques such as CPR; Instructs and counsels patients regarding compliance with prescribed therapeutic regimens and treatments; Performs pre-admission clearance for patients with medical diagnoses and symptoms. Performs pre-admission interviews when indicated; Develops and implements patient management plans, medication and therapy recommendations, and assists with continuity of care planning; Performs and documents follow-up visits with updates in diagnosis and medications, as needed. Assesses and manages effects and/or side effects of medications; Participates in individual case assignments, case conferences, treatment team meetings, educational presentations for staff and patients, medical staff meetings, and additional duties as assigned by the Medical Director; Provides medical on-call coverage as assigned; Participates in peer review activities and performance improvement initiatives; Preparation and education of patients for discharge medications and instructions.

    Participates in individual case assignments, case conferences, multidisciplinary treatment team meetings, educational presentations for staff and patients, medical staff meetings, patient care monitoring conferences, and additional duties as assigned by the Medical Director.


    Sign On Bonus: $7,000 SIGN-ON BONUS (Based on 40 hours, will be prorated based on hours): $3,500 after completion of the orientation period, $3,500 after two (2) years of continuous employment.

    Requirements:

    A qualified Nurse Practitioner for Conifer Park will require: A Post Graduate Degree – Masters in Nursing; Nursing License to practice in State of New York; National Certification as Nurse Practitioner (e.g., through American Nurses Credentialing Center, National Certification Center, or American Academy of Nurse Practitioners Certification Program); Greater than 1 year experience as a nurse practitioner, preferred; Experience in addiction medicine or treatment of substance use disorder preferred.
