LEAD THE FUTURE OF HOME DIALYSIS CARE
Cooper Home Health is seeking an experienced healthcare executive and Registered Nurse to serve as our Director of Patient Care. This is a senior clinical operations leadership position responsible for patient care quality, home dialysis operations, clinical workforce leadership, regulatory compliance, physician collaboration, and program growth across multiple Florida markets.
This is not a clinic-based position. Our clinicians provide home dialysis education, treatment support, and interdisciplinary care directly within patients’ homes. The Director of Patient Care leads the systems, teams, and quality infrastructure that enable those services to be delivered safely, consistently, and effectively.
ABOUT THE ROLE
The Director of Patient Care provides executive oversight of Home Hemodialysis (HHD) and Peritoneal Dialysis (PD) programs, patient care personnel, quality initiatives, and interdisciplinary care services throughout the organization.
This leader works closely with executive leadership, nephrologists, administrators, nurses, registered dietitians, social workers, and other healthcare partners to promote exceptional patient outcomes, regulatory readiness, workforce stability, and sustainable organizational growth.
The successful candidate will be a hands-on executive who can operate at the strategic, operational, clinical, and leadership levels while developing scalable systems and strengthening accountability across a remote, geographically distributed workforce.
KEY RESPONSIBILITIES
Develop and execute patient care and clinical operations strategies aligned with organizational growth objectives.Provide leadership and oversight of Regional Administrators, Market Administrators, Registered Nurses, Registered Dietitians, Social Workers, and other patient care personnel.Establish and maintain standards of patient care consistent with evidence-based practices, regulatory requirements, and organizational policies.Monitor patient outcomes, clinical quality indicators, hospitalizations, infection prevention measures, treatment performance, retention, and patient satisfaction.Lead quality improvement initiatives, QAPI activities, clinical audits, corrective action plans, and survey-readiness efforts.Ensure compliance with applicable federal, state, CMS, ESRD, accreditation, and healthcare regulatory requirements.Oversee patient care documentation standards, quality assurance activities, and clinical risk-mitigation initiatives.Develop workforce plans, staffing models, performance expectations, succession strategies, and leadership-development programs.Coach and mentor administrators, clinical leaders, and interdisciplinary team members.Lead the development, implementation, expansion, and optimization of HHD, PD, and related patient care programs.Collaborate with nephrologists, medical directors, referral sources, and interdisciplinary care teams to strengthen care coordination and clinical outcomes.Establish operational and clinical performance metrics, dashboards, and executive reporting processes.Participate in budgeting, workforce forecasting, productivity management, and resource planning.Support market expansion, implementation of new technologies and workflows, and organizational change initiatives.Travel periodically to support patient care operations, leadership meetings, quality reviews, audits, training, and organizational growth activities.REQUIRED QUALIFICATIONS
Bachelor of Science in Nursing from an accredited institution.Active, unrestricted Registered Nurse license in Florida.Minimum of 10 years of progressive healthcare experience.Minimum of 5 years of senior clinical or healthcare operations leadership experience.Demonstrated leadership experience within dialysis, renal care, home-based healthcare, outpatient healthcare programs, hospital-based services, or another complex regulated healthcare environment.Experience overseeing clinical leaders, interdisciplinary professionals, or patient care teams across multiple service areas or markets.Strong knowledge of healthcare quality standards, regulatory compliance, clinical documentation, and patient care operations.Demonstrated experience with quality improvement, performance management, workforce planning, and leadership development.Ability to analyze clinical and operational data and translate findings into measurable improvement plans.Ability to work effectively within a remote, geographically distributed organization.Ability and willingness to travel periodically throughout Florida based on organizational needs.Must reside in Florida.PREFERRED QUALIFICATIONS
Leadership experience with Home Hemodialysis and Peritoneal Dialysis programs.Strong knowledge of CMS ESRD Conditions for Coverage.Experience with QAPI, regulatory surveys, corrective action plans, and clinical audit readiness.Experience leading multi-market or multi-state healthcare operations.Experience supporting a startup, high-growth, or rapidly expanding healthcare organization.Experience partnering with nephrologists, medical directors, executive leadership, and other healthcare stakeholders.MBA, MHA, healthcare administration training, or related executive leadership education.COMPREHENSIVE BENEFITS PACKAGE
