Responsibilities: Perform comprehensive Home Health Assessments for current patients. Assess, plan, implement, and evaluate individualized care plans. Coordinate with physicians, case managers, and other healthcare professionals. Ensure all care meets regulatory and agency standards. Qualifications: Active Texas RN license (unencumbered). 2+ years of clinical experience (1+ year home health experience REQUIRED). Excellent communication and organizational skills.
