GENERAL DESCRIPTION OF DUTIES
The Community Response Specialist is an integrated peer specialist and case manager who is responsible for responding to crisis calls. In nature, these calls will have an inherent high risk associated with them as individuals may be suffering from mental health and/or substance use. He/she is to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. This position is responsible for being a trusted member of the community, where they can be engaged to provide psychosocial support to participants who were at increased need for an array of community-based harm reduction services. The general duties are to develop authentic peer-to-peer relationships to promote recovery, referrals to appropriate treatment resources and supports, and, to promote safety for persons in crisis. All activities are designed to be consistent with CRT’s Salesforce ISP service model, based on educating and empowering individuals for self- advocacy and to make self-identified changes in their lives with reducing need for emergency services.

Respond to crisis calls and/or referrals.Perform outreach activities and prevention education to foster good community relations, educate about program services and assess needs in Hartford neighborhoods where there is high crime and utilization of 911 for crisis intervention. Engage high-risk, hard to reach people who struggle with mental health, drug use, or engage in sex work.Identify potential recruitment locations. Engage in dialogue with participants and other community members to continually improve our services and ensure that CRT is operating its sites and engaging in outreach activities in the best locations at the best times.Culturally appropriate harm reduction, health promotion, and health education such as safer sex and safer drug use education. This also includes distribution of harm reduction supplies such as syringes (and collection of used syringes) and other health-related resources and tools in accordance with public health regulations.In a manner consistent with Harm Reduction principles, provide substance use and sexual risk assessments and discuss individualized risk reduction and harm reduction strategies (including safer injection practices; safer sex practices; overdose prevention and reversal; drug use management techniques; HIV, HCV and STI prevention; and other topics relevant to people who use drugs, or participate in other high-risk behaviors).Manage a caseload of program participants and provide low-threshold referrals and connect participants with medical care, substance use and mental health treatment, and other life-stabilizing services. Solicit and track feedback from participants to ensure that the agencies to which we refer are effective and treat participants with respect and in a manner appropriate to harm reduction programs. This includes, helping connect to schedule appointments to identified service/resource. Emphasizes person-centered, recovery-oriented approaches and interventionsPrepares participants to recognize and respond to various mental health and addiction crises, including trauma; develop effective communication and connection skills.Establish and maintain enhanced communication between community members and providersModel proper participant engagement and outreach Provide guidance and mentorship as necessary.Keep accurate, consistent, complete records and data on all program activities and/ or participants; provide program director with reports in a timely manner.Prepare, set up and dismantle program sites and other program materials. Ensure the integrity of the sites, and maintain inventory of program supplies and bi-lingual educational materialsComplete vehicle daily maintenance logs which include oil, tires, gas, and structural checks. Document and report any incidents or accidents within 24 hours of occurrence. Ensure that vehicles are kept clean, properly stocked, and ready for serviceMust be available to work a varied schedule, late evenings, weekends, and unconventional hours.Perform all other duties as assigned.
MINIMUM TRAINING, EXPERIENCE, AND HEALTH CERTIFICATIONS
Education: Associates with an approved combination of experience, certification, and/ or education.
Minimum Years of Experience: A minimum of three years of experience in client case management.
Active Certification in good Standing: Certified Recovery Specialist and Community Health Worker Certification is required. Must have and/or acquire both certifications within 6 months of employment.
Demonstrate skills: Ability to manage diverse operations, effectively multi-task and a style that encourages creativity, growth, collaboration, problem solving, open communication and accountability. Organized, self-motivated team-player, with strong interpersonal and communication skills. Proficient in MS Office Suite and in using technology for data collection as a management reporting tool. Ability to compile written reports and maintain case notes. Ability to work with diverse, multicultural populations. Ability and willingness to be hands-on and solve problems as they arise. Willingness to work outdoors (sometimes in inclement weather).
Knowledge: Knowledge of local community resources. Must have lived experience, and a working knowledge of mental health, substance use, and co-occurring treatment systems.

· Set and clear tables and arrange furniture as needed.
· Serves and prepares some menu items (e.g. desserts, salads, coffee, juice soda and water) to members in a supportive and accommodating manner and checks and properly fills food service-related dispensers to meet the daily needs of the users.
· Assists members as needed to ensure their safety during the meal (e.g. helping them to maneuver their chairs and removing any obstacles which could create tripping hazards).
· Maintaining daily kitchen clean up duties.
· Wash dishes and breakdown the dishwasher to ensure proper sanitization after each use
· Sweep, dry and wet mop floor by completely covering surface with correct solution and removing all spillage, stains, dirt, debris and/or miscellaneous matter from fibers according to required frequency.
· Cleans surfaces in eating area as assigned or as an identified as needing attention (e. g. walls, windows, light fixtures, ceilings, furniture, equipment and miscellaneous surfaces removing dust, dirt, debris, and/or miscellaneous matter from surface leaving a streak less, clean appearances).
· Checks and properly fills food service-related dispensers to meet the daily needs of the users.
· Dry and wet mop floors completely covering floor and removing all spillage, stains, dirt, debris and/or miscellaneous matter from fibers according to required frequency.
· Vacuums carpets removing all dust, dirt, debris and/or miscellaneous matter from fibers according to required frequency.
· Ensures trash, cans and bottles are packaged, transported, disposed or recycled according to established regulatory guidelines.
· Empties receptacles and/or picks them up according to required frequency.
· Break down boxes and take to the trash.
· Set tables for next person to come and dine.
· Follows all organizational and departmental policies.
· Completes other appropriate duties as required.
ADDITIONAL JOB FUNCTIONS· Maintains confidentiality of all members’ information while assists co-workers, members, visitors and volunteers in a respectful and dignified manner.
· Ensuring a pleasing environment in the serving area for members’ enjoyment during the meal (e.g. addressing members in a calm and respectful manner, keeping the dining room organized and quiet).
· Leaves equipment in good operating condition for the next co-worker.
· Uses chemicals according to OSHA Hazard Communication Standard (Right-to-Know) and chemical safety.
· Recognizing changes in behavior or appearance of members’ which could signal possible changes in their conditions. Reporting observed changes to Service Manager or Food Service Manager or on-call nurse as soon as the changes are identified.
· Understands the working and cleaning schedules, reports (e.g. quality assurance), appearance standards, frequencies and time standards.
· Reacts appropriately to fire and emergency preparedness drills and events according to policies.

Understands infection control and salty guidelines as required by various regulatory agencies. Utilizes personal protective equipment to protect self and co-workers from injury. Utilizes good body mechanics and lifting techniques.

MINIMUM TRAINING AND EXPERIENCE
Education: High School diploma or GED desirable.
Minimum Years of Experience: One (1) year experience in a healthcare food service position or two (2) years’ experience in commercial, hospitality or other food service setting desirable.
Active Certification in good Standing: Will be required to attend to obtain Food Handler’s Certificate (e.g. Serve Safe).
Demonstrate skills: Must be able to communicate extremely well with individuals who are affected by psychiatric conditions. Must have excellent customer service skills.
Knowledge: Must have knowledge in serving food and beverages in a manner which accommodates physical and physiological limitations of members. Have knowledge to recognizing changes in behavior or appearance of members.

Demonstrate skills: Experience billing Medicare, Medicaid and private insurance preferred. Computer proficiency (MS Word, MS Excel and MS Access) required. Excellent oral and written and customer service skills. Experience in behavioral health, domestic violence, or immigration services preferred.
Knowledge: Must have knowledge of managing schedules, community outreach, and working with a diverse population.

Responsible for ensuring continued eligibility for applicants in the CRT Federal, State, and Utility based Weatherization Programs in accordance to program and company regulations. Ensures continued compliance at all times with funding sources and collaborates with clients through each phase of the program to completion and ensures all information and supportive documentation is continually updated and verified. Additional weatherization eligibility requirements are reviewed as applicable, and referrals are made to other programs within or outside the agency as necessary.

MINIMUM TRAINING AND EXPERIENCE
Education: High school diploma or GED required, Associate or Bachelor degree strongly preferred.
Minimum Years of Experience: 3+ years in office administration required. Weatherization assistance, energy conservation/efficiency, and or construction or maintenance experience strongly preferred.
Active Certification in Good Standing: Building Performance Institute (BPI) Building Analyst and Envelope Certifications, U.S. EPA Retrofit, Repair and Painting (RRP) Certification, OSHA-10 Certification strongly preferred, or ability and willingness to obtain as directed.
Demonstrated Skills: Excellent customer service and communication skills with the ability to work and educate clients from various backgrounds. MS Office and database proficiency. Experience in financing and production operations preferred. Organizational skills including the ability to handle multiple projects, write concise reports, plans and correspondence. Demonstrated ability to maintain accurate and timely records.
