    JOB SUMMARY

    Responsible for ensuring that all program participants are provided with the services necessary to locate, acquire, maintain, and retain permanent housing.    Provide landlord outreach and advocacy, housing leads and applications.  Is knowledgeable about and accurately completes applications for housing subsidy programs.

    Pay: $24.04 -$25.25

    FLSA: Non-exempt. Eligible for overtime. 

    Location: Bronx, Brooklyn NYC

    JOB-RELATED DUTIES – ESSENTIAL FUNCTIONS

               These duties are essential and specific to the successful implementation of this position.

        Adheres to and enforces program regulations, policies, and procedures. Ensures that all clients are receiving quality services that are in compliance with the program.Assists clients in locating housing within their specified budgets.Provides information, referrals, direct service, and advocacy services for local housing pool.Develops and maintains a resource network for referrals as related to the housing; realtors, landlords, housing complexes and local Section 8 offices.Is familiar with NYC and NYS housing subsidy programs, supportive housing options, SROs and affordable housing.Collaborates with DHS, property owners, and other service providers, and shelter Case Manager to assist families in fulfilling ILP and program requirements.Mediates between clients and property owners or representatives.     Responsible for collecting and reviewing Housing Logs and informing case managers of any concerns related to documents. Completes Housing Packets and Shelter Supplement Program paperwork.Submits all internal and external reports in a timely and correct manner as required by funding agencies and/or CHI (housing logs, unit inspection sheets, progress notes, as required by CHI and DHS.Completes progress notes, referral, and updates client’s CARES case.Maintains charts, files, and housing paperwork in an orderly fashion. Always maintains clinical appropriateness.What do I need to apply?

    Education and Experience: Bachelor’s Degree in human services or related field preferred. 3+ years’ experience in a related field assisting people with obtaining permanent housing can replace degree requirement. Must have at least two years’ experience working in a related field such as homelessness or housing.

    Must have working knowledge and insight into NYC housing resources including experience helping people find affordable housing and working with different NYC and NY State agencies and programs the facilitate permanent housing goals. Communication:  Excellent verbal and written communication skills.  Proficiency in English required,bi-lingual (Spanish) is a plus.Computer skills:  Basic competency in MS Word, Excel, Outlook and the internetMath Skills:  Ability to formulate simple financial budgetsPhysical Performance:  Ability to make home visits (walk distances, climb several flights of stairs, etc.)Reasoning Ability:  Ability to prioritize, make appropriate decisions and judgment callsOther Skills:  Ability to relate to the underserved, homeless and disabled populations.     WHAT WILL I BE DOING?

    CHI is looking for a full-time Case Manager for our Medford location. You will be responsible for ensuring that all program participants are provided with the services necessary to promote personal and economic independence, to have skills required to obtain, maintain, and retain permanent housing, and are living in well-maintained units.

    •    Adhere to and enforce program regulations, policies, and procedures. Ensure that all clients are receiving quality services that are following the program requirements.
    •    Assist clients in developing an Independent Living Plan (ILP) by identifying needs and goals. Evaluate client progress on short- and long-term goals.
    •    Provide information, referrals, counseling, crisis intervention, direct service, and advocacy services for all clients.
    •    Develop and maintain a resource network for referrals as related to the needs of the individuals being served in supervised programs. Collaborate with DSS and other service providers to assist clients in fulfilling ILP and program requirements.
    •    Assist clients with direct services such as current and projected client budget, current resume as appropriate, assistance with ADL skills, etc.
    •    Responsible for face-to-face meetings with clients.
    •    Inspect living space regularly as required by the program to assess clients ADL needs and program compliance to assist clients with maintaining housing.
    •    Limit and document improper use of agency resources by clients such as unit damages, furniture, or appliances.
    •    Evaluate, document, and inform the direct supervisor of physical plant needs of the housing unit and/or common areas.
    •    Complete discharge plans with clients.
    •    Submit all internal and external reports in a timely and correct manner as required by funding agencies and/or CHI (housing logs, unit inspection sheets, progress notes, ILPs, authorization forms required by DSS, special projects, etc.). Maintain charts and files in an orderly fashion.

    WHAT WILL I BE DOING?

    CHI is looking for a full-time Program Manager based out of our Commack location.  As Program Manager, you will be responsible for all aspects of social work services within the respective program in Suffolk County, ensuring that quality housing services are provided effectively and in accordance with guidelines established by CHI, HUD, Section 8, Suffolk County Department of Social Services, and other funding sources as appropriate.


    Ensure assigned supervised program is providing quality services and is compliant with program requirements. Promote sound social work practices through supervision including case consultations, case file reviews, and employee performance appraisals. Ensure the shelter meets New York State OTDA license requirements and standard operating regulations.Maintain accurate records related to annual state inspections.Provide regular supervision for staff on site; Case Managers, Food Handler, Resident Support Staff (RSS), Housing Specialist, and other managers.Conduct 1 to 1 meetings with members of your team and update the performance goals, accomplishments, notes, and feedback in Paycom on a regular basis (weekly/bi-weekly).Ensure the timely and correct submission of all internal and external reports in the manner required by funding agencies and/or CHI (housing logs, client contact sheets, unit inspection sheets, progress notes, authorization forms, census reports, etc.).Inform Emergency Housing Director of any program or agency wide deficiencies.Develop and maintain a resource network for referrals as related to the needs of the individuals being served at the shelter.Provide crisis intervention as appropriate.Prevent loss of income to CHI as a result of vacancy loss, lost shelter payments, and/or DSS housing sanctions.  Adheres to proper use of company resources (cell phones, office supplies, internet, etc.).Coordinate admission and discharge processes.  Act as a liaison with agencies responsible for referring eligible clients.Maintain professional appropriateness at all times.Participate in the hiring process, initial and annual employee evaluations, and disciplinary measures for direct reports. Responsible to oversee appropriate coverage and payroll procedures within department. Provide on-call coverage as needed.Ensure that all staff maintain mandated and recommended trainings.Act as an agency representative to external partners.


    WHAT DO I NEED?

    Education:  Master’s Degree in Social Work or similar field required. Five years' experience with two years supervisory in client services and program administration. 

    In lieu of a Master's degree, a candidate must have a Bachelor's degree in human services.

    Experience:  Seven years of experience in social services with three years supervisory in client services and program administration.

    Certificates or Licenses: Valid driver’s license required. New York State Social Work License preferred.

    Communication:  Excellent verbal and written communication skills.  Proficiency in English required. Bi-lingual (Spanish) is a plus.

    Computer Skills:  Basic competency in MS Windows, MS Office, and internet usage. Knowledge of Foothold Awards system a plus.

    Math Skills: Ability to formulate simple budgets.

    CHI is looking for a full-time Resident Support Specialist Supervisor for our Riverhead location.  You will be responsible for all aspects of management for Resident Support Specialist staff within emergency housing programs. Ensuring that quality services are provided effectively and in accordance with guidelines established by CHI, OTDA, Suffolk County Department of Social Services, and other funding sources as appropriate. 

    •    Provide consistent supervision and support for all Resident Support Specialist staff.
    •    Conduct 1 to 1 meetings with members of your team and update the performance goals, accomplishments, notes, and feedback in Paycom on a regular basis (weekly/bi-weekly).
    •    Ensure RSS staff are up to date and in compliance with all required trainings. Provides on-site training and feedback as needed.
    •    Responsible for scheduling and coverage of RSS shifts at location.
    •    Act as a liaison for CHI by implementing timely and effective communication with outside entities. 
    •    De-escalate potential crisis situations and document all situations and interventions.
    •    Audit logs, incident reports, and all other required documentation to ensure compliance with applicable rules and regulations. Provides guidance on appropriate documentation to RSS staff.
    •    Conduct fire drills and evacuate the building when necessary.
    •    Ensure that the program is providing quality services and is compliant with program requirements.  
    •    Educate clients about the program policies and procedures.
    •    Prevent loss of income to CHI as a result of vacancy loss and lost shelter payments, property, etc.  
    •    Assist the management team in communication as well as being an on-call supervisor.

    JOB SUMMARY

    The Case Manager is part of a human service team that develops and executes service and care plans for residents to provide a supportive and empowering environment that fosters stabilization, independence and self-reliance, and skills development.     JOB SUMMARY

     The Director of Social Services (DSS) leads the social service unit that develops and executes service and care plans for residents to provide a supportive and empowering environment that fosters stabilization, independence and self-reliance, and skills development, and enhance the health and general well-being of residents; and ensures services are appropriate and culturally sensitive. The DSS provides operation and administrative support to ensure program and contractual compliance and ensures the accurate and punctual submission of required documentation and reporting to CHI, city, state, and federal agencies. The Director builds and sustains a strong service network with external providers and community-based organizations to support staff and assist residents towards stability and independence.

    Pay: $78,000- $85,000 annual salary.

    FLSA: Exempt. Not eligible for overtime.

    Schedule: Sunday -Monday and Wednesday 9am-5pm Tuesday and Thursday 11am-7pm. (Friday and Saturday off). 24/7 on call availability Location-Midtown Manhattan

    JOB-RELATED DUTIES – ESSENTIAL FUNCTIONS

               These duties are essential and specific to the successful implementation of this position.

    SERVICE AND CARE COORDINATION

    Ensure residents receive quality and appropriate services per organizational principles and program requirements.Develop and maintain a resource network, inclusive of formal and informal partnerships and service agreements, of external providers and organizations that offer support, advocacy and direct services for immigration and citizenship issues, health and mental health issues, employment, education, childcare and youth service connections, trauma recovery and counseling, cultural adaptation, and English language acquisition, life skills and independent living skills. Ensure program team leverages network partnerships in service planning and execution.Ensure Social Worker and Case Management teams develop service and care plans that are aligned with biopsychosocial assessments. Ensure assessment and plans provide a culturally sensitive and comprehensive analysis and identify appropriate and adequate services and resources.Ensure recreation workshops are engaging, promote education, employment, self-care and healthy habits, cultural adaptation, English language proficiency, communication skills, and independent living skills.Identify and codify best practices; coach program team to implement best practices.Review discharge plans to ensure continued enrollment and participation in public benefits programs and service linkages to community-based organizations are in place.

    PROGRAM MANAGEMENT

    Ensure program deliverables are met. Develop enhancement strategies for underperforming areas. Ensure CHI program KPIs are aligned with program metrics and deliverables.Ensure proper and timely follow-up to Priority 1 and 2 incidents with clients, and reporting to DHS and CHI.Ensure program team adheres to CHI code of conduct and abide by contractual, city, state, and federal rules and regulations.

    SUPERVISION

    Directly supervisor Case Management Supervisor, Social Work Supervisor, and Recreation team.Troubleshoot difficult resident cases; intervene and directly assist in special needs cases.Conduct 1 to 1 meetings with members of your team and update the performance goals, accomplishments, notes, and feedback in Paycom on a regular basis (weekly/bi-weekly).What do you need to apply?

    Education and Experience: Masters degree in Social Work or related field required. LMSW or LCSW preferred. Five years of experience plus two years of supervisory and managerial experience in social work, case management, counseling, or direct advocacy for clients in social service settings.     A bachelor’s degree in social services and ten years of experience in social services with 5+ in a supervisor setting can be used in lieu of the education requirement.
