    WHAT WILL I BE DOING?

    CHI is looking for a full-time Shelter Manager for our Middle Island location. You will be responsible for ensuring that all program participants are provided with the services necessary to promote personal and economic independence, to have skills required to obtain, maintain, and retain permanent housing, and are living in well-maintained units. You will be responsible for overseeing maintenance of operations and ensuring a safe, secure environment for shelter residents and staff. This is accomplished by performing the following functions:

    Adhere to and enforce program regulations, policies, and procedures. Ensure all clients are receiving quality services that are in compliance with the program.Assist clients in developing an Independent Living Plan (ILP) by identifying needs and goals and evaluate client’s progress on short- and long-term goals.Provide information, referrals, counseling, crisis intervention, direct service, and advocacy services.Develop and maintain a resource network for referrals as related to the needs of the individuals being served in supervised programs. Collaborate with DSS, property owners, and other service providers to assist clients in fulfilling ILP and program requirements.Assist individuals with direct services such as current and projected budget, current resume as appropriate, assistance with ADL skills, etc.           Responsible for face-to-face meetings with clients twice a week. Inspect living space regularly as required by the program to assess individual’s ADL needs and program compliance to assist client with maintaining housing.Limit and document improper use of agency resources by clients such as unit damages, furniture, or appliances.  Evaluate, document, and inform Program Manager of physical plant needs of the housing unit and/or common areas.Provide regular supervision to Resident Support Specialist (RSS). Directly supervise RSS, be part of any hiring and disciplinary actions, track time off, RSS scheduling, Paycom payroll system, and schedule trainings.Conduct 1 to 1 meetings with members of your team and update the performance goals, accomplishments, notes, and feedback in Paycom on a regular basis (weekly/bi-weekly).Responsible for ensuring the site is safely covered 24/7 which includes scheduling staff, intervening as needed when there is an incident both by phone and/or as needed in person. Covering of staff in emergency.Ensure the timely and correct submission of all internal and external reports in the manner required by funding agencies and/or CHI, dashboards, incident reports (IRC and OTDA), corrective action plan (CAP), housing logs, client contact sheets, unit inspection sheets, progress notes, authorization forms, admission and discharge sheets, census reports, etc.

    WHAT DO I NEED?

    Education:  Bachelor’s Degree in human services or related field, Master’s Degree in Social Work preferred.  Extensive experience may be substituted for educational requirement.Experience:  Two years’ experience working in a related field such as homelessness or housing. Management experience preferred.Certificates or Licenses:  Valid driver’s license.Insurance:  Valid automobile insurance.Communication:  Excellent verbal and written communication skills.  Proficiency in English required.     JOB SUMMARY

    The Case Manager is part of a human service team that develops and executes service and care plans for residents to provide a supportive and empowering environment that fosters stabilization, independence and self-reliance, and skills development. The Case Manager plays a key role in executing and updating service and care plans, coordinating direct services and service referrals to external providers, and monitoring resident behaviors and practices.

    Schedule: Tuesday-Saturday. 11am-7pm (T, W) 8am-4pm (Th, F, Sat). Off Sunday & Monday.

    Schedule: Tuesday - Saturday. Tuesday, Thursday-Saturday 9am-5pm Wednesday 11am-7pm(Off Sunday & Monday).

    Schedule: Sunday - Monday 8am-4pm Tuesday- Thursday 11am-7pm (Friday & Saturday off).

    Schedule: Tuesday - Wednesday 9am-5pm Thursday- Saturday 11am-7pm (Sunday & Monday).

    Salary: $24.04 an hour.

    FLSA: Non-exempt. Eligible for overtime.

    JOB-RELATED DUTIES – ESSENTIAL FUNCTIONS

    These duties are essential and specific to the successful implementation of this position.

    SERVICE AND CARE COORDINATION

    · Ensure residents receive appropriate quality services per organizational principles and program requirements.

    · In coordination with Social Worker (if available), design service and care plans that provide customized information, counseling, crisis intervention, direct service, and advocacy services to residents; and coordinate linkages to external and community-based services.

    · Collaborate with other service providers and external organization to help residents resolve immigration and citizenship issues, secure public benefits and charitable gifts and donations, pursue employment and entrepreneurial opportunities, independence, and long-term stability.

    · For family sites: coordinate with Recreation Team (if available on site) to ensure school-age children receive transportation services, school transfers, and ESL/ELL and IEP curriculum as needed.

    · Develop and maintain a resource network for referrals as related to service needs of residents.

    · Meet with residents in assigned caseload at least once a week to assess progress with service and care plans. Modify plans as needed.

    · Conduct weekly room inspections to ensure healthy hygiene and housekeeping. Coordinate with Social Workers and/or Treatment Team to mitigate behaviors that compromise the health and safety of residents.

    · Design discharge plans to ensure continued enrollment and participation in public benefits and service linkages to community-based organizations are in place.

    BENEFITS / ENTITLEMENT COORDINATION

    · Ensure residents in assigned caseload are enrolled in and receiving eligible public benefits and city programs in accordance with DHS requirements.

    · Ensure residents follow through with appointments, documentation submissions, and other required activities to maintain enrollment and participation in benefits and DHS programs.

    · Coordinate with Mayor’s Office of Immigration Affairs and other immigration services and advocacy organizations and enroll residents in immigration services and programs if applicable.

    GENERAL DUTIES

    · Document interactions and communications with and on behalf of residents in CARES and AWARDS.

    · Perform other duties as assigned by supervisor and Program Director.

    ENTRY LEVEL QUALIFICATIONS AND REQUIREMENTS

     

    Education and Experience: High School Diploma or GED required. Bachelor’s degree in social work or psychology preferred. Three years of experience working in a shelter or similar field can be substituted for the education requirement.

    · Experience: Two years’ experience in case management, counseling, or direct advocacy for clients in social service settings. Experience with immigrants and immigration issues is a plus.

    · Communication: Excellent verbal and written communication skills. Proficiency in English required, multi-lingual and Spanish proficiency a plus.

    · Computer skills: Basic competency in MS Word, Excel, Outlook and the internet

    · Math Skills: Ability to formulate simple financial budgets.

    · Physical Performance: Ability to make unit visits (walk distances, climb stairs, etc.)

    · Reasoning Ability: Ability to prioritize, make appropriate decisions and judgment calls.

    Develop clinical partnerships with external providers and organizations that offer support, advocacy, and direct services for mental health issues, substance use, and trauma recovery.

    Provide linkages to community-based mental health care, substance use, and psycho-social supports and communicate with providers at referring agencies, as necessary.

    Lead monthly or quarterly group discussions and case conferences with other social service staff across the organization to discuss service modalities and behavioral management strategies and to troubleshoot difficult client cases.

    Develop a library of content that focuses on topics related to mental health, substance use, working with difficult populations, self-care, and other wellness skills development -communication, conflict resolution, and anger management.

    Continuously assess the learning needs of clinical staff and develop and implement learning objectives.

    Helps identify staff’s training needs and, in response, plans, develops, and/or conducts training toward maintaining and improving clinical staff competency and compliance with agency policy and procedure.

    GENERAL DUTIES:

     

    Maintain detailed documentation of collateral, progress and group notes, and other reports.

    Collect and utilize data to provide informed decisions on training/education topics and services.

    Attend supervision to review and discuss issues affecting the broader unit.

    In coordination with the HR Training Unit, provide program position-specific content that can be incorporated into the onboarding and employee development process.

    Lead a committee to support frontline worker well-being and reduce burnout and high levels of stress.

    Supervise Social Work interns as a field instructor or task supervisor.

    Perform other duties as assigned by the supervisor.

    Screening and collecting referrals and information prior to assessment of client.

    Provide de-escalation, assessment, and linkage to individuals in crisis

    Conduct crisis assessments as part of CHI’s crisis response for children/adolescents with full range of DSM diagnosis including mental health and substance use.

     

    WHAT DO I NEED?

     

    • Education: MSW from an accredited university

    • Licenses or Certificates: New York State Licensed Clinical Social Worker, SIFI

    • Experience: 2-5 years providing clinical and educational services in an approved setting

    • Communication: Excellent verbal and written communication skills. Proficiency in English is

    required.

    • Computer Skills: Excellent competency in Office365, HMIS, and other web-based sources

    • Math Skills: Must be excellent.

    • Physical Performance: Ability to tour properties, walk distances, climb stairs, etc.

    JOB SUMMARY

    Provide counseling and pre-purchase education to homeownership clients on a one-on-one and/or group basis and administer grants for the homeownership program. Ensure that all grant funds are dispersed in a timely manner to qualified applicants. Work closely with team members to organize outreach and present education seminars in order to maintain and grow the First Time Homebuyer Education program. Ensure all client files are audit ready for grant and funding compliance.

    Pay: $27.88 an hour, $57,990.40 annually 

    FLSA: Non Exempt. 

    Schedule: 9am to 5pm Monday through Friday.

    First Time Homebuyer Counseling:

    Manage intake of homeownership applications, including organizing files and maintaining waitlist.Review all grant applications to determine the eligibility of an applicant for a grant, to assist in the purchase of their first home. Conduct an eligibility and affordability analysis for every completed application that is submitted adhering to the PB 11B-IRS Form 1040 Income and Asset Guidebook for Homeowner Program Guidelines. Including but not limited to, the reviewing of credit report, income documentation, monthly household budgets, verification of employment and other relevant documentation required as part of the application review process.Provide face to face, phone or internet counseling to first time homebuyers. Prepare individualized action plans for clients as needed.Review and update the grant wait list on a weekly basis.Provide counseling to all First Home Club and Smart Move participants and oversee their application and progress for the duration of the program.Evaluate and provide counseling to all eHome America student. Approve and/or decline certificates.Submit lender invoices when applicable to accounting.Maintain all client files to ensure files are in compliance with HUD.     Grant Administration:

    Review grant applications deemed complete and eligible to finalize eligibility for the AHC First Time Homebuyers grant.Schedule and conduct grant certificate meetings in Westchester and Long Island and via video conferencing; topics include review grant guidelines, program requirements, rules, and procedures with the client. Document in Counselor MaxReview all proposed purchases for eligibility under the Grant Program Requirements before a home purchase contract is signed; affordability may be reviewed by DirectorCollect and review all required pre-closing documents: mortgage loan commitment, home inspection report, HUD Planner’s scope of work, 1003, purchase contract, title report, etc.Works with client, lender, loan processer, attorneys and contractors as needed during the closing processPrepares fund request for AHC, with Final Review provided by Director.Prepares CHI’s closing documents and wires funds to attorney for closing, and files final closing packagePrepares a Contractor Agreement to document the rehab plan, budget and timing and ensure all parties sign before work beginsOversee the rehab process.     What do I need to apply?

    Education:

    Three years of housing counseling, banking and/or financial services experience will be considered in lieu of a degree. Bachelor’s degree with one year banking experience preferred.

    Special Knowledge and Skills:

    WHAT WILL I BE DOING?

    CHI is looking for a Part-time Rental Supplement Program Counselor for our Nassau location. This position will include travel and field related work. Great opportunity at a growing non-profit agency for the right candidate. Help us grow our Housing Retention Services in Nassau County. This is accomplished by performing the following functions:

     

    Respond to referrals via HMIS and collaborate with referring agencies to collect and upload client eligibility information.Prepare initial, annual, and interim income certifications as outlined by Suffolk County’s HUD CoC funded programs.Work with new clients, including initial discussions, completion of all intake forms and review of financial documents.  Execute basic math skills.Contact all participating households at least once every three months regarding moves, changes in household composition or monthly income, budgeting issue and supportive services.Responsible for all required reporting and compliance – cases must be managed in accordance with New York State requirements; including database and file management.Experience working with clients experiencing homelessness/residing in shelter.Knowledge of the local housing market and available resources.Conduct annual unit inspections to ensure housing quality standards are met for potential participants.Contact landlords to mediate Housing Quality Standard issue, lease renewal and potential client repayment plans.Connect clients with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of the referral.Maintain up-to -date records in AWARDS and HMIS. Document all interactions in a clear and concise manner.Establish good working relationships with referring agencies.Represent the company with community outreach and events.Remain up to date with industry standards and increase professional and technical skills through continuing education annually

    WHAT DO I NEED?

    Education:  Bachelors' Degree in a related field preferred. Extensive experience may be substituted for educational requirement.Experience:  Basic knowledge and understating of Federal, State and local homeless and housing programs and rental subsidies.Communication:  Excellent verbal and written communication skills.  Proficiency in English required. Bi-lingual (Spanish} is a plus.Computer Skills:  Basic competency in MS Windows, MS Office, and on the Internet. Math Skills: Must be excellent.Physical Performance:  Walking upstairs, driving, and set up conference rooms.Reasoning Ability:  Ability to prioritize, make appropriate decisions and judgment calls.Other Skills:  Ability to work independently or as part of a team, be highly organized, assume responsibility, work well under pressure, meet deadlines, multitask, be flexible, and maintain a high level of attention to details. Must be even­ tempered, have good negotiation skills, and the ability to meet unexpected deadlines.Valid driver's license and automobile insurance.
