Salary Range: $104,500.00 - $120,000.00 / year The Family Nurse Practitioner will provide advanced primary and specialty nursing services. The NP performs patient health assessment, creating strategies to improve or manage a patient's health, and introducing habits for health promotion. Our expectation is that this position will see at least two patients per hour of patient facing time. This person will support CHPs mission, vision, and values and will adhere to compliance protocols as well as CHPs policies and procedures. POSITION REQUIREMENTS Family Nurse Practitioner Essential Duties and Responsibilities: Perform physical exams and patient observations. Record patients' medical histories and symptoms. Create patient care plans and contribute to existing ones. Order, administer, and analyze diagnostic tests. Monitor and operate medical equipment. Diagnose health issues. Administer medicine and other treatments. Detect changes in patients' health and modify treatment plans, as needed. Consult with healthcare professionals and families. Train patients and their families to manage and prevent illness and injury. This person collects co-payment and notifies the clinic that patients have arrived for their appointments. The Receptionist is responsible for the providers schedules and ensures that patients and providers are notified accordingly. This person will support CHPs mission, vision, and values and will adhere to compliance protocols as well as CHPs policies and procedures POSITION REQUIREMENTS Medical Receptionist Essential Duties and Responsibilities: Welcome patients to the Health Center; ensure that all pre-exam information is up to date and accurate before appointments; collect co-payments; make sure that referrals are received. Provide schedule support to providers. Perform clerical duties as requested. Provide information to patients/outside callers. Record and deliver messages. Answer, screen and route outside calls to the appropriate area or individual. Conduct scheduling/reminder/re-scheduling phone calls to patients.
