Job Description:
The LPN/ RN provides skilled nursing services to patients in accordance with the current nursing plan of care within their scope of practice. Employees are also responsible for the completion and documentation of vital signs and observations, coordination of care activities, and delivering nursing services for each assigned patient. The LPN/RN also promotes safety and education, working in a professional and organized manner.
Essential Job Functions and Responsibilities:
Provide skilled nursing care to assigned patients, as identified in current nursing plan of care and within nursing scope of practiceComplete and document observations, vital signs, body systems, pain, etc.Complete skilled nursing procedures, such as specimen collection and related patient servicesPromote health and prevention to decrease incidents of hospitalizations and/or re-hospitalizations by providing patient and caregiver educationAct as a professional nursing liaison between patients, caregivers, physicians, and other health care providersCoordinates care with CBNS staff, other agencies, and health professionals, as neededComplete medication related tasks, documentation, physician order coordination, medication change-over, medication check-in, and other related tasksContribute to a safe and secure environment for patients, caregivers, and other nurses by taking prompt action to remediate; if unable, then escalate to supervisor for instruction and/or assistanceAssist in keeping nursing supplies and equipment clean and up to date at the work site and/or patients homeReview and implement nursing plans of care during each patient skilled nursing service, within scope of practiceParticipate in CBNS Supervisory Visit and review with the Clinical Manager, at intervals required by regulations and CBNS policy and proceduresComplete incident and infection report promptly, including notification to internal/external contacts, as requestedElectronically verifies location of start and end of each skilled nursing visit by patient in electronic verification record, promptly reporting errors and/or omissions for correctionComplete shift report via secure email system, including copy to Clinical Manager, and others as requestedUphold and promote the mission of CBNS and the value of home health nursing servicesDemonstrate constructive problem solving and team-oriented resolution to issues and/or concernsRemain in compliance with CBNS policy/procedures, Ohio Board of Nursing scope of practice, and CBNS Code of Conduct, as well as ethical business practicesQualifications:
