Key Responsibilities:
Individual Support
Assist with daily living activities (hygiene, feeding, medication, mobility)Support individuals in achieving personal goals and community engagementPromote rights, dignity, and choice in accordance with HCBS standardsEnsure safety and well-being while providing individualized careProgram & Documentation
Accurately document services, incidents, and daily activitiesAdminister and document medications as requiredReport incidents, concerns, or rights violations promptlyMaintain clear communication with team members and familiesProfessional Conduct
Maintain respectful, professional relationships with individuals and stakeholdersDemonstrate initiative, flexibility, and teamworkModel positive behavior and uphold confidentiality standardsOperations & Safety

Complete required trainings and maintain certificationsParticipate in ongoing staff development and HCBS trainingQualifications:
High school diploma or equivalent (required)Minimum 1 year of related experience preferredKnowledge of IDD and person-centered practicesValid drivers license with safe driving recordPhysical Requirements:
Ability to lift at least 25 lbsAbility to stand, walk, bend, and assist individuals as neededWork Environment:
Must follow Universal Precautions and safety protocols at all times
