    Use evidence-based modalities to establish therapeutic rapport and facilitate active engagement in treatment. Conduct diagnostic assessments Develop Individual Service Plans that identify areas of clinical intervention and/or need which include a specific time frame for the completion of each identified goal. Provide therapeutic intervention to address identified treatment and recovery goals Participate in case consultation as indicated. Continuously assess client status and determine ongoing medical necessity for treatment. Routinely assess for level of risk and respond to emergent situations. Coordinate care with relevant internal and external providers and informal supports Complete all documentation and charting as required by law or agency contract. To maintain an acceptable number of client, contact hours per month. Possess knowledge of client rights and ensure an atmosphere that allows for consumer privacy, dignity, and well-being in a safe secure environment.Perform all job responsibilities in accordance with prescribed safety and infection control procedures. Conduct group counseling as required.

    Education, Licensing and Experience:

    Summary:

    To provide clinical and clerical support to patients, medical staff, and other providers.

    Essential Job Responsibilities:

    Provide direct patient care including.Obtaining vital signs.Obtaining and recording patient history.Preparing and maintaining examination and treatment areas.Maintaining medication and immunization records.Screening and following up on test results as directed by a health care practitioner.Monitoring and reporting patient symptoms and side effects.Administering intradermal, subcutaneous and intramuscular injections.Collecting specimens for laboratory testing. i.e. blood, urine and saliva.Check and return voice messages for medical department and triage to appropriate staff when needed.     Summary:

    This member of the Mobile Health team is responsible for assessing physical health needs; coordinating with the Mobile medicine prescriber, making appropriate referrals to community physicians; providing a range of limited wound care/cleaning and other standing orders; blood draws; providing medication education and training. This position entails working on a mobile unit in the community as part of a multi-disciplinary team providing care to houseless and unstably housed individuals.

    Essential Job Responsibilities:

    Provide service coordination for community membersTriage acute and chronic medical conditions.     Summary:

    Peers provide support services as part of an integrated team to clients who are involved in treatment services. Provides direct peer support to participants to assist them on their identified goals. The Mental Health Peer Support Counselor is responsible for a wide range of scheduled activities to assist clients in exercising control over their own lives and recovery process (e.g., promoting socialization, self-advocacy, developing natural supports and maintenance of community living skills). The Peer Support will model competency in recovery and ongoing coping skills for other clients. Peer support services may include but are not limited to self-help support groups, telephone support lines, drop-in centers, and sharing of the peer counselor’s own life experiences.     Starting Pay Range –

    $115,000-$145,000 annually depending on experience.

    Summary:

    Work in close collaboration with the Trueblood Element Programs, FPATH (Forensic Projects for Assistance in transition from homelessness), FHARPS (Forensic Housing and Recovery through Peer Services), OCRP (Outpatient Competency Restoration Program), and the DSHS (Department of Social and Health Services) FNs (Forensic Navigators), as well as the internal program MA (Medical Assistant), to provide effective medical and Prescriber services to Program Participants where they are in the community and in county/city jails as appropriate, regardless of medical insurance coverage or Medicaid status. Program Participants are typically houseless, at risk of homelessness or have been recently and/or temporarily housed.

    Essential Job Responsibilities:

    Coordinate with the Trueblood Element Programs (above), local jails, hospitals, crisis facilities, and other community touchpoints to ensure accessible prescribing services, formulate treatment plans, and reduce gaps in medication maintenance

    Provide basic medical care services for the Program Participants, including but not limited to the following: wound care and foot care, sexually transmitted infections, Human Immunodeficiency Virus (HIV), and Hepatitis C testing, Birth control, Naloxone and overdose prevention training, vaccines, and care for acute and chronic conditions.

    Provide services without stringent requirements or policies for prescribing and accessing medications to the Program Participant. Provide appropriate trauma-informed approaches, harm and risk-reduction information, methods, and tools when providing services to Program Participants.

    Non-essential Job Functions:

    Support agency mission, goals, and objectives.Attend staff and planning meetings as scheduled. Maintain communication and cooperative working relationships both within the agency and community at large.Active driver’s license and valid insurance required.Other duties as assigned by Supervisor.

    Education, Licensing and Experience:

    An active, unrestricted Physician or Advanced Registered Nurse Practitioner CPR certifiedOne year or more of experience in behavioral health prescribing preferredExperience working with the aged, the uninsured, the underinsured, individuals with severe mental health disorders, and/or substance use disorders preferred.Ability to credential with Medicare, Medicaid and commercial insurers

    Summary:

    Provide initial diagnostic interview for adult consumers to determine eligibility for CRMHS services.

    This is a Union role, represented by OPEIU 11.

    Essential Job Responsibilities:

    We are seeking a Mental Health Therapist to join our Adult Outpatient team! The successful candidate will have a master’s degree specializing in counseling or social work.     Job duties will include:
