Location: NW Portland, OR
Job Type: Full-Time | Direct Hire
We are seeking an experienced RN Case Manager to oversee one floor of a 54-room Assisted Living and Memory Care community. This position is responsible for approximately 17–19 residents and serves as the clinical lead and floor manager during the swing shift.
Schedule
2:00 PM – 10:30 PMDays Off: Sunday, Monday, Tuesday
Key Responsibilities
Coordinate and oversee resident care plans and nursing servicesComplete resident assessments, admissions, and service plansMonitor medical, behavioral, and daily living care needsPerform wound care, medication oversight, and clinical assessmentsCollaborate with families, physicians, hospice, home health, and other providersSupervise documentation and ensure regulatory complianceSupport and educate caregiving staffServe as Manager on Duty during evening hours and address operational or security concerns
Requirements
Active Oregon RN LicenseMinimum 2+ years of Assisted Living and/or Memory Care experienceStrong leadership, case management, and clinical assessment skillsExperience working with behavioral health, dementia, challenging behaviors, or addiction support preferredCPR/First Aid CertificationAbility to pass Oregon DHS background check
