    KEY RESPONSIBILITIES

    Assist residents with activities of daily living, such as bathing, dressing, grooming, toileting, and transferring, as needed, while adhering to community protocols, licensing regulations, and guidelines for resident and employee safety.Prioritize independence and emotional support for the residents by providing compassionate care, engagement, and companionship.Encourage and assist with participation in life enrichment activity programs.Assist with light housekeeping duties, such as making beds, tidying rooms, etc.Promote open communication between healthcare professionals, families, residents, and staff.



    Requirements:

    CANDIDATE QUALIFICATIONS

    Education:

    High School Diploma or equivalent.Current caregiver training/certification per state requirements, such as CNA/GNA/PCA/DCA, or the willingness/ability to obtain.Current First Aid and CPR license or ability to obtain.

    Experience, Competencies, and Skills:

    POSITION SUMMARY

    Our Wellness Nurse (LPN) plays a vital role in ensuring the health and well-being of our residents. In collaboration with the Health and Wellness Director, this role provides high-quality care through health assessments, medication management, and coordination with residents, families, and the care team. Responsibilities include monitoring resident health, addressing changes in condition, staff training and supervision, and promoting a safe and supportive environment that aligns with our mission to provide exceptional care.


    KEY RESPONSIBILITIES

    Perform resident assessments and reassessments according to policies, monitoring any changes in condition.Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services.When necessary, participate in hands-on care, medication administration, and specific nursing tasks.Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.Supervise, train, and support care team members, promoting teamwork and adherence to care standards.Assist in the transcription of physicians' orders and the review of new physicians' orders.Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.Coordinate care planning with home health agencies on-site, working within community and company policies.Address residents’ special needs, requests, and complaints.Coordinate admissions with the sales team, including supervising move-ins.Understand the community's care regulations along with state-specific regulations.Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.

    Education and Certifications:

    A valid, incumbent nursing license (LPN/LVN) is required.High School Diploma or equivalent.An associate degree or higher in nursing is preferred, or a combination of education and experience.     POSITION SUMMARY

    The Health and Wellness Director (LPN/LVN or RN) is an integral member of the management team and works closely with the Executive Director to ensure the health and wellness of our residents, performing initial and ongoing assessments and observation of our residents' physical and psycho-social needs, while coordinating with other departments to assure the highest quality of care. They interact directly with employees, residents, resident family members, vendors, and professional contacts within the community. They provide training, supervision, and coaching of the clinical team while helping build a positive and energetic atmosphere within the community. Responsible for the day-to-day clinical operations of the community while ensuring compliance with all applicable federal, state, and local laws, rules, and regulations, as well as company policies.

    KEY RESPONSIBILITIES

    Foster a resident-centered approach to planning and delivering services, supporting residents' rights to participate in decision-making and treating residents with dignity and respect.Assess residents' health, functional, and psychosocial status, initiate individualized service plans, proactively manage care and services for each resident, and maximize their opportunity to remain in the assisted living or memory care environment.Establish and maintain the medication program.Oversee all hiring, promotions, disciplinary actions, and termination of employment of licensed nursing staff (LPN/LVN), Med Techs/CMA/CNAs, and all Care staff.Hands-on training, supervision, and staff development. Communicate regularly with residents' families regarding health care needs.Maintain current departmental policies, procedures, and licenses, following company, Federal, State, and local requirements.Ensure resident records and charting are current and up to date.Participate in the management team's marketing efforts by representing the community in a positive manner to outside agencies.

    POSITION SUMMARY

    Our Care Partners play a critical role in their community. You will provide compassionate and personalized direct care to our residents, assisting them with activities of daily living and ensuring their comfort, safety, and well-being while promoting independence, maintaining their privacy, and respecting their dignity. Our Care Partners work in a team setting to ensure a supportive and nurturing environment for our residents.


    KEY RESPONSIBILITIES

    Assist residents with activities of daily living, such as bathing, dressing, grooming, toileting, and transferring, as needed, while adhering to community protocols, licensing regulations, and guidelines for resident and employee safety.Prioritize independence and emotional support for the residents by providing compassionate care, engagement, and companionship.Encourage and assist with participation in life enrichment activity programs.Assist with light housekeeping duties, such as making beds, tidying rooms, etc.Promote open communication between healthcare professionals, families, residents, and staff.


    Requirements:

    CANDIDATE QUALIFICATIONS

    Education:

    High School Diploma or equivalent.Current caregiver training is a plus.Current First Aid and CPR license or ability to obtain.

    Experience, Competencies, and Skills:

    At least 12 months of experience in a professional caregiving setting.Experience with memory care is preferred.Strong communication skills and a teamwork mindset.Positive attitude, empathy, patience, and commitment to treating our residents with dignity and respect.Attention to detail, physical stamina, and high integrity.Willingness to participate in weekend rotation.
