    Job Summary

    College Medical Center Phoenix is seeking a Nutrition Assistant to support daily meal service for patients and hospital guests. The ideal candidate will be responsible for serving food and assisting with minor food preparation while following department leadership direction, dietitian expectations, and established service procedures. This individual will be expected to comply with all food service regulations established by the Maricopa County Department of Health while upholding the ethics and moral standards of College Medical Center. This is an opportunity to be part of a team that reflects the mission, vision, and values of CMC while supporting a diverse workforce.


    Qualifications:

    At least 1 year of food service experience (preferably healthcare experience) in a high volume food service environment

    Proficient in use of cerner, reading census information, and using software in department to manage dietary guidelines and procedures to oversee the proper trays are served to the proper patient.

    Consult with RD to assist in care plan for patients.

    High School Diploma or GED Equivalent

    Ability to obtain (or already have) a Food Handlers Permit for AZ. Ability to obtain a ServeSafe Certification. Must have FH or ServeSafe within 30 days of hire date.

    Must be able to work weekdays, weekends, holidays. Day or Night shift as required

    Must be able to stand for long periods of time (12 hour shifts), lift heavy objects (up to 50 lbs) and work in high volume environments


    Job-Specific Duties:

    Prepare and serve all menu items according to standardized recipes and department service standards pertaining to menu prep for service.

    Support hospital in areas of food service, nutrition, stocking nourishments, and dishes.

    Follow guidelines for patient diet restrictions and management.

    Maintain a safe working environment including following health department standards, regulatory procedures, all kitchen logs, and ensuring all kitchen equipment is used and maintained to proper department standards.

    Maintain all kitchen and storage areas following local regulatory standards, department and company leadership standards, and required sanitation and safety standards.

    Complete all assigned tasks while on duty adhering to safety standards and policies of CMC.

    Complete Initial educational training modules and any subsequent modules assigned by CMC on time, and complete additional training when assigned within the required timeframe.

    Additional Responsibilities:

    Attend all staff meetings, or additional educational modules assigned by leadership.

    Role Description

    The Community Liaison Director – PHP is a full-time, on-site position based in Phoenix, AZ. In this highly visible role, you’ll build and strengthen relationships throughout the community, lead strategic outreach efforts, and support home care initiatives that advance the organization’s mission. You will actively engage local stakeholders, deliver consistently exceptional customer service, and champion programs and services that expand access, awareness, and impact.     Qualifications:

    Minimum one-year experience working in-patient or outpatient psychiatric facilities.

    Effective Communication skills, including written, verbal, and interpersonal communication for diverse audiences.

    Proven expertise in Customer Service and working with various stakeholders to provide exceptional service and support.

    Job-Specific Duties:

    Build and grow referral relationships (physicians, allied health, community partners); identify new sources.

    Provide community education, referral support, and coordinate tours.

    Support patient interventions as needed and follow up with admitted clients to sustain engagement.

    Complete at least 20 in-person referral contacts/week and stay current on community issues/trends; report updates daily to the Program Director.

    Understand and support the outreach workflow (intake, screening, insurance verification, admission, follow-up).

    Attend weekly CHC unit visits and weekly staff/treatment team meetings.

    Manage priorities, return calls within 1 hour, and escalate significant issues promptly to the SVP.

    Coordinate transportation, complete documentation/insurance verification, and provide pre-admission patient orientation (including copays/deductibles).

    Perform other duties as assigned.

    Job Summary

    College Medical Center Phoenix Partial Hospitalization Program (PHP) is seeking an Outpatient Registered Nurse to support our expanding PHP services. A nursing degree/diploma from an accredited school is highly preferred. The Outpatient RN must have a minimum of one year of clinical experience in psychiatric nursing within an outpatient setting and the skills to provide care to PHP clients. This is an opportunity to be part of a growing PHP program.

    Qualifications:

    Current Arizona Registered Nurse (RN) license.

    Current AHA BLS Healthcare Provider certification.

    Graduate of an accredited school of nursing (degree or diploma).

    Minimum one (1) year of clinical experience in psychiatric nursing, preferably in an outpatient setting.

    Proficiency in timely admissions processing, documentation, and executing provider orders.

    Ability to recognize and respond to changes in patient condition.

    Experience with milieu safety and basic nursing procedures (vital signs, medications, injections).

    Demonstrated ability to work collaboratively in a multidisciplinary team.

    Strong communication, assessment, and customer service skills.

    Preferred Qualifications

    Prior outpatient PHP/IOP experience.

    Qualifications

    Job Duties

    Facilitate group treatment modalities within the program's scope of practice

    Conduct initial assessments of new patients and develop initial treatment plans

    Develop comprehensive master treatment plans using assessment data and input from the treatment team

    Accurately document progress notes within 24 hours of service or by the next business day, referencing identified problems on treatment plans

    Document incidents or unusual occurrences promptly

    Complete discharge summaries for assigned patients in a timely manner

    Demonstrate competency in working with adult patients (ages 18+), including completion of psychosocial assessments according to established guidelines
