Guide interdisciplinary care planning and discharge processes for optimal patient results. Serve as a patient and family advocate while building strong relationships with payers and healthcare partners. What You'll Bring: Active RN license in Florida and BSN or higher. At least 3 years of hospital-based Case Management (acute care, inpatient rehab, or LTACH). 2+ years of leadership experience (supervisor level or above).
