    POSITION DESCRIPTION:

    Serve as a primary care provider for geriatric patients located in one or more long-term care facilities / skilled nursing facilities in Terre Haute.


    A low volume of patients are seen per day, allowing ample time to focus on all needs and to provide proper attention and support.


    Overall duties of the role include, but are not limited to:


    - plan, direct, and evaluate medical care

    - examine and treat chronic illnesses

    - examine and treat urgent illnesses, minor injuries, etc.

    - order and interpret laboratory and diagnostic testing

    - collaborate with caregivers and other healthcare professionals

    - work closely with the interdisciplinary care team

    - make referrals as necessary


    The practice uses an EHR on a mobile-based platform and providers are supported by team members who assist with scheduling, orders, referrals, etc.


    Providers share on-call responsibilities, which is low volume phone triage only. Providers are on call no more than 3 nights per month and flexibility in that schedule is offered.

    POSITION DESCRIPTION:

    The nurse practitioner serves as the primary care provider for a dedicated panel of geriatric patients located within a defined geographic territory in the greater Dayton area. Patients are located in private homes (house calls), as well as long-term care facilities (ALFs, ILFs, SNFs) and other group settings.


    Providers see a low number of patients per day, with schedules coordinated by a dedicated team factoring in type of visit, travel time, and patient needs. The schedulers put together daily schedules and routes for the providers to ensure efficiency and appropriate time for every visit.


    Duties of the role include, but are not limited to:


    - health assessments to identify patient needs

    - develop personalized care plans

    - evaluate patient conditions and responses to treatment

    - adjust treatment plans as necessary

    - minor procedures and wound care as settings permit

    - educate patients and families on health management, disease prevention, etc.

    - work with physicians, nurses, and other professionals to coordinate care


    The practice uses a mobile-based EHR with AI functionality to reduce administrative time.


    On-call is phone triage only with low volume. Providers evenly share the on-call responsibility.

    POSITION DESCRIPTION:

    The Nurse Practitioner or Physician Assistant will deliver comprehensive primary care, chronic disease management, and preventive health services to a diverse patient population via in-home visits (house calls) in a designated territory in the New York metro area. The exact geographic boundaries for the position will be mutually determined during the interview process.


    Focusing on geriatric and chronic care populations, the overall duties of the role include, but are not limited to:


    - in-home assessments, physical exams, medical histories, and diagnostic evaluations

    - health screenings and wellness checks

    - preventive care, health education, and counseling

    - develop and implement individualized care plans

    - work in collaboration with patients, families, and interdisciplinary healthcare teams

    - manage chronic conditions (diabetes, hypertension, heart disease, etc.)

    - order, interpret, and follow up on diagnostic tests, labs, and imaging

    - prescribe medications and treatments in accordance with scope of practice

    - administer immunizations

    - address urgent medical needs in the home when possible

    - provide timely follow-up care for patients discharged from hospitals, EDs, or SNFs

    - regularly evaluate patients to prevent readmissions

    - ensure continuity of care with primary care providers, specialists, and community resources

    - participate in quality improvement initiatives

    - provide culturally-sensitive and linguistically-appropriate care at all times

    The Remote Patient Monitoring (RPM) Coordinator is responsible for monitoring patient health data transmitted from connected medical devices, engaging patients to support chronic disease management, and coordinating care with providers and clinical staff.


    This role serves as a critical link between patients and the healthcare team by identifying potential concerns, promoting patient adherence, and ensuring timely intervention when abnormal readings occur.


    Specific duties of the role include, but are not limited to:


    - monitor daily biometric readings (blood pressure, glucose, pulse-ox, heart rate, etc.)

    - review patient alerts and identify abnormal trends requiring intervention

    - contact patients regarding abnormal readings

    - provide education on proper device usage and medication adherence

    - coach patients on healthy lifestyle habits (nutrition, exercise, disease management, etc.)

    - escalate concerning clinical findings to providers according to established protocols

    - assist with patient enrollment into RPM programs

    - coordinate delivery, set-up, and troubleshooting of monitoring devices

    - monitor patient compliance and encourage regular transmission of health data

    - collaborate with providers, care managers, and support staff in continuity of care

    - participate in QI initiatives aimed at increasing engagement and reducing hospitalizations

    - stay current on CMS guidelines for RPM and organizational workflows

Their facilities boast exceptional services in independent living, assisted living, memory care, couples care, rehabilitation services, and more. They are currently seeking two compassionate Assistant Directors of Nursing / Wellness Nurse Managers for their Reston location.



    POSITION DESCRIPTION:

    The Assistant Director of Nursing Services / Wellness Nurse Manager (ADON) reports to and supports the Director of Nursing Services (DON) at the 90-resident Reston facility. This includes support in planning, organizing, developing, directing, and evaluating the nursing services operations.


    This role works closely with residents, families, physicians, and other members of the interdisciplinary team to ensure that residents receive high-quality, patient-centered care with adherence to all federal, state, and local regulations.


    The duties of the ADON role include, but are not limited to:


    - directly supervise nursing personnel in the absence of the DON

    - perform staff nurse duties when required

    - meet with prospective and current residents to discuss any issues or needs

    - review pre-admission medical evaluations for residents

    - assist in collecting resident personal information for admission

    - evaluate resident functioning levels

    - lead performance evaluations for LPNs / LVNs, in partnership with DON

    - participate in regular reviews to identify improvement opportunities to nursing service

    - assist in developing improvement plans when needed

    - participate in resident health maintenance meetings

    - coordinate with physicians and other healthcare providers to update care plans

    - supervise delivery of treatments to residents

    - develop and maintain staff schedules

    - assist in recruiting, interviewing, and hiring nursing staff


    The facility uses the Yardi EHR.



    REQUIREMENTS:

    - active Virginia RN license

    - 2+ years clinical nursing experience

    - experience in SNF or long-term care environments preferred

    - BSN preferred
