    Job Description

    Position Summary

    Provide personalized care and support to clients in the comfort of their homes, ensuring a high-quality, compassionate service that meets the individual needs of each client. Work closely with the client and their family to promote independence, safety, and overall well-being.

    Key Responsibilities

    Personalized Care: Assist clients with daily activities such as bathing, dressing, grooming, and personal hygiene, while encouraging their independence.Nutritional Support: Plan and prepare healthy, nutritious meals tailored to the client's preferences and dietary needs. Assist with feeding if necessary.Household Support: Light housekeeping duties including laundry, cleaning, and organizing to maintain a safe, comfortable, and welcoming home environment.Companionship: Provide meaningful social interaction, engaging conversation, and emotional support to improve the client’s overall quality of life.Health Monitoring: Observe and report any changes in the client's condition, alerting the supervisor or healthcare professionals as necessary.Documentation: Maintain accurate records of services provided, including dates and times, ensuring clear communication with the healthcare team.Specialized Care: Under the guidance of a physician or healthcare professional, assist with more specialized tasks as outlined by the Colorado Nurse Aide Practice Act.Other Duties: Assist with a range of tasks as needed, ensuring the well-being and comfort of the client.

    Qualifications

    Basic Qualifications & Experience

    High School Diploma or equivalent.6 months experience working in an adult day, healthcare or home care environment that serves senior adults.Colorado State Certified Nursing Assistant License, in standing.Must possess a valid Colorado driver’s license in good standing.Must possess current vehicle registration and proof of insurance coverage and reliable automobile.Must be able to read, write and speak the English language.Must possess excellent customer service skills.

    Working Conditions/Physical Requirements

    Position requires medium-heavy physical work. Heavy lifting, pushing or pulling objects up to 100 pounds of force occasionally and/or up in excess of 45 pounds of force repeatedly to move objects.Stands and walks constantly during working hours with occasional sitting, kneeling, squatting, and reaching above shoulder height and fine motor control with fingers. Also requires grasping.Exposure to blood borne pathogens, body excretions and communicable diseasesIs subject to frequent interruptions.Interacts with residents, family members and visitors frequently.

    Essential Duties

    Assists in writing, designing, distributing and evaluating the monthly life enrichment calendar.Inventories and purchases and stocks supplies and equipment as needed.Drives the bus for scenic rides and large and small group outings.May serve on various committees as directed.Serves as Director in Directors absence. May provide the Director support in various functions such as timecard administration, care conferences, budget compliance etc. Observes and communicates information to appropriate staff regarding residents needs, desires for, and responses to programs and activities.Compiles data for reporting purposes and performs various clerical tasks.Other duties as assigned.
    Qualifications

    Basic Qualifications & Experience

    High School Diploma or GED. Activity Professional coursework preferred.1-2 years experience in recreational programming.Colorado drivers license in good standing.Must be able to read, write and speak the English language.Must possess excellent customer service skills.
