About the Role:
The Nurse Practitioner PRN at the Tucson Hospice Branch plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses. This position involves providing advanced clinical assessments, developing and implementing individualized care plans, and managing symptoms to enhance patient comfort and quality of life. The Nurse Practitioner will collaborate closely with interdisciplinary teams, including physicians, nurses, social workers, and chaplains, to ensure holistic support for patients and their families. Flexibility and adaptability are essential, as this PRN role requires responding to fluctuating patient needs and schedules. Ultimately, the Nurse Practitioner contributes to the mission of hospice care by promoting dignity, respect, and comfort during the end-of-life journey.
Position Summary
The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate- a proven clinician responsible for helping shepherd the care of the seriously ill. The ideal candidate will have a personality type that is predispositioned to thrive in an unstructured environment and is ok with reacting to patient needs while being a supportive team member with excellent ability to communicate with team members.
As a Hospice Nurse Practitioner, you will:
Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
Perform Face-to-Face visits at the patient's place of residence.
Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
Knowledgeable of hospice criteria.

Current STATE RN and Advanced Practice Nurse License
Valid Driver’s License
Valid Auto Insurance
AHA CPR Certification
HomeCare HomeBase experience preferred
