    GENERAL PURPOSE OF THE JOB

    The purpose of the center manager position is to lead, develop, implement, oversee and manage the operations, future growth and development of the allcoveTM center.

    The center manager works closely and collaboratively with a range of stakeholders to ensure all activities are in accordance with the available funding, and direction provided by the lead agency, Community Consortium partners and the Youth Advisory Group.

    The center manager has experience at managing multidisciplinary teams, as well as experience in establishing collaborative agreements for integrated services that meet the needs of the communities who access services.

    The center manager can also provide clinical leadership, oversight, and supervision to licensed and pre-licensed mental health providers.


    STANDARD REQUIREMENTS

    Is knowledgeable of HIPAA and Patient Rights and ensures an atmosphere that allows for the privacy, dignity, and well-being of all in a safe, secure environment. Supports and cooperates with specific procedures and programs for:Safety includes universal precautions and safe work practices, established fire/safety/disaster plans, risk management, security, reporting and correcting unsafe working conditions, equipment repair, and maintenance needs.Confidentiality of all data, including Client, Employee, and Operations data.Quality Improvement and compliance with all regulatory requirements.     ESSENTIAL FUNCTIONS

    Leadership

    In accordance with funding and oversight requirements, lead, develop, oversee and evaluate implementation of service delivery to young people and their family and friends. Communicate, promote and progress the vision and strategic plan of the allcove center amongst Community Consortium partners and the broader community including youth community, adult-ally, youth mental health, marginalized and traditionally under-resourced communities, including BIPOC communities. Represent the lead agency and the allcove center at events, conferences and seminars. Serve as a linkage to the Community Consortium, engage with their strategic advice and collaborate with them in development of strong community partnerships. Engage relevant community members, organizations, and partners in an integrated model of care, promoting multidisciplinary teamwork and participation in the allcove program at a local level. Convene various committees associated with the allcove program and youth health services, organize and participate in committee meetings, including preparation of meeting agendas, minutes, papers and correspondence.

    Management

    Oversee the day-to-day operation of the allcove center.Lead and oversee the recruitment and selection of any new staff (including service partner and contracted staff), ensure they are orientated to the procedures and operations of the service.Manage and supervise staff, monitor staff performance and development, and address staff performance issues effectively according to documented policies and procedures. Work collaboratively with service partner and contracted staff to ensure that any difficulties are identified and resolved to maintain the sustainable and ongoing delivery of their services. Participate in and support other members of the allcove center to participate in programs, education and training, and other opportunities offered by the Central allcove Team, the allcove network, and the larger array of youth mental health services. Shape and structure a working environment that is conducive to authentic youth engagement, where all staff understand what is expected of them and how their efforts contribute to organizational success.Ensure center’s team is in alignment with trademark license and brand guidelines.

    Business Management

    Develop the allcove center strategic plan, business plan and annual work plan in conjunction with the lead agency senior management, consortium partners ensuring deliverables and key performance indicators are achieved. In conjunction with lead agency senior management, manage the allcove contracts and funding agreements and all their deliverables including: Acting as the key contact for all matters concerning the contracts and delivery of the allcove program. Managing the allcove budget. Completing and submitting all reporting in relation to the contracts. Establish appropriate processes and structures and develop, document, implement and review policies and procedures that ensure the youth centered, community-driven services of the allcove center. In conjunction with management, be an active participant in the financial management of the allcove center. Ensure financial responsibility and accountability.

    Quality and safety

    Oversee the implementation of relevant and innovative quality systems and contribute to their study and evaluation to ensure service integrity and quality.Ensure adherence to relevant quality and safety professional and healthcare standards and mandatory education related to risk management, occupational health and safety and other relevant areas.Monitor, evaluate and maximize data collection and compliance with the dataset requirements from allcove Central allcove Team, and any other funding body.Recognize and manage risk and ensure that actions are taken to prevent and minimize harm to consumers and the workforce.Respond to or provide support to all staff, including peer support, supported education and employment, and substance use staff, during critical incidents and high-risk situations, both clinically and operationally.Ensure all complaints and incidents are managed in accordance with lead agency and allcove center policies and procedures.

    Clinical Oversight

    Provide weekly clinical and administrative supervision to therapists and clinical interns. Ensure documentation is accurate and submitted within established time frames from therapists.Display sensitivity to the cultural and linguistic needs of clients and families served.Interact effectively with internal programs, clinical/support staff, and external sources such as schools or community agencies.Ensure staff have the knowledge, tools, and skills for job success, including quality client care and documentation.Use the electronic health record system and agency quality services measures reports actively during in-person supervision.Complete and submit written reports reviewing supervised chart documentation and client records.Meet agency billing requirements for reimbursable services and provide services per programmatic and contractual requirements.Attend and actively participate in staff meetings, contributing to program development and staff training.Review and maintain audit-ready standards and conduct peer audits.

    Relationships

    Develop and maintain positive and effective working relationships with a broad range of people and organizations including:Young people that access the center and their families.Central allcove Team.State level partners and consultants.Youth advisors. Community Consortium partner organizations and staff.Co-located partner organizations.Local youth.Health and community service providers.Staff within government agencies. Neighbors of the allcove center.Other allcove center network staff.Center staff and staff from partner organizations, including peer support, supported education and employment and substance use staff. Develop and maintain key internal relationships with lead agency staff, allcove center staff, contractors and the Youth Advisory Group. Build and maintain effective relationships with key stakeholders involved in the provision of primary care, behavioral health and psychosocial services to young people.

    QUALIFICATIONS

    A broad understanding of the challenges and experiences of youth including specific local community factors.Must have practiced psychotherapy or provided direct clinical supervision of qualifying supervisees who perform psychotherapy for at least two (2) years within the five (5) year period immediately before commencing supervision in community or behavioral health. Minimum of two years post-licensure experience. Demonstrated capacity to effectively engage in leadership and management processes, including multidisciplinary and/or cross sector initiatives, and partnerships with a wide range of professionals, organizations and stakeholders. Demonstrated experience at managing and developing operational systems for a health or community services organization, including effectively planning and allocating resources to maximize operational efficiency and meet funding deliverables. Experience in direct service provision to young people, including providing mental health services or programs, and appropriate expertise in managing crisis and high-risk situations. Previous experience partnering with youth on developing health care systems and services, prioritizing the importance of youth voice and shared decision making. Proven track record in building and maintaining effective working relationships with a range of internal and external stakeholders. Demonstrated understanding of the principles of accountability systems and continuous quality improvement processes, including the utilization of data systems, the development and implementation of policies and procedures, and the use of evaluation and risk management tools. Detailed understanding of the California health care system, particularly the areas of behavioral health and primary care, including integration, as well as experience with the range of professionals and organizations that work within or in proximity to integrated health care service systems. Experience in working with vulnerable and underserved populations. Demonstrated experience in managing all aspects of staff and team performance, including recruitment, staff orientation, supervision, performance reviews, staff disciplinary action, performance development and team building. Highly developed verbal and written communication skills. Exceptional interpersonal skills with the ability to establish and maintain effective relationships with a diverse range of people and professionals. Ability to work both independently and collaboratively as a productive team member. Advanced computer skills including word processing, spreadsheets and database applications. Knowledge of principles and techniques of social, cultural, and intergenerational dynamics of Asian immigrants and refugees. Reliable Transportation. Successfully meet the post-offer/pre-hire physical, and fingerprint, requirements and receive an eligibility status for the Level II Background Check required by the State of California.

    About This Role - Physician

    Primary care physicians are usually the first point-of-contact for people seeking medical care. These doctors must be able to assess, diagnose, and treat a wide range of conditions, ranging from broken bones to more exotic illnesses. Physicians often care for families across generations and are an integral part of many people's lives. In cases where more specialized medical care is needed, patients may be referred for more focused care.


    Duties and responsibilities

    Provide the full spectrum of outpatient primary care for adults and children. Record patient history, perform or request diagnostic tests and make diagnoses. Prescribe appropriate treatment, administering vaccinations and performing follow-up examinations, as well as educating patients in disease prevention and health maintenance. Provide individual patient instruction in accordance to prescribe treatment plan. Prepare medical records, clinical reports and correspondences. Supervise allied health professionals, such as nurses, physician assistants and medical assistants as assigned. Attend conferences and seminars when requested and comply with continuing education requirements.Perform other duties as assigned.

    Qualifications

    Graduated from an accredited medical school. Valid California Practice License. Continuing Medical Education (CME) credit to maintain license and board certification after residency. DEA registration to prescribe controlled substances. Bi-lingual English and Chinese (Preferred, not required). Valid CA Driver’s License and Proof of Insurance.

    Provides support for the Operational Leadership by delivering superior level administrative clerical support. The Project Assistant to Operational Leadership will implement administrative systems, procedures, policies and contribute to administrative projects. The Project Assistant will also utilize critical thinking skills in the performance of office support and operational/supervisory support functions. This position is also responsible for creating presentations, preparing accurate reports and managing meeting notes. The Project Assistant will work independently in carrying out assignments to completion with time sensitive deadlines. Must have excellent communication skills and be able to interact with Executives, Managers, Providers and employees in all departments and levels.     Duties and responsibilities

    We are seeking a compassionate and dedicated Day Center Aide to join our team at CSC PACE Center.     Duties and Responsibilities

    Participant Care Responsibilities

    Assist participants with activities of daily living (ADLs), including: Ambulation and transfers Toileting and incontinence care Grooming and personal hygiene Feeding and hydration assistance Dressing assistance as needed Obtain and document vital signs, weights, intake/output, and other observations as directed by licensed nursing staff. Observe and promptly report changes in participant condition, behavior, or safety concerns to the nurse or interdisciplinary team. Assist with restorative care, mobility exercises, and fall prevention measures under the direction of nursing staff. Maintain participant dignity, privacy, and confidentiality at all times. Support safe participant transportation within the center and assist with wheelchair safety and positioning.

    Day Center and Activity Support

    Assist the Activities Department with setup, facilitation, supervision, and cleanup of group and individual activities. Encourage participant participation, engagement, and socialization during activities and meals. Escort participants to meals, activities, appointments, and rest areas as needed. Provide supervision and redirection to participants to promote a safe and supportive environment. Assist with meal service, snack distribution, and monitoring participants during dining.

    Cleaning and Environmental Responsibilities

    Clean and sanitize tables, chairs, activity areas, equipment, and commonly touched surfaces in accordance with infection control policies. Assist with maintaining cleanliness and organization of the day center, participant areas, nourishment stations, and activity rooms. Properly dispose of trash, soiled linens, and biohazard materials according to facility procedures. Restock participant care supplies, PPE, linens, and activity materials as needed. Report environmental safety hazards, maintenance concerns, or supply shortages promptly.

    Compliance and Safety Responsibilities

    Follow all infection prevention and control policies, OSHA requirements, and safety procedures. Maintain active CNA certification and required trainings. Adhere to participant rights, HIPAA confidentiality requirements, and PACE program policies. Participate in staff meetings, trainings, competency evaluations, and emergency preparedness activities. Support the interdisciplinary team in delivering quality participant care and services.Other duties as assigned.


    Qualifications

    Education:

    The Speech Therapist (Speech-Language Pathologist) is responsible for evaluating, diagnosing, and treating communication, cognitive-linguistic, and swallowing disorders for participants enrolled in the Program of All-Inclusive Care for the Elderly (PACE). The Speech Therapist works collaboratively with the Interdisciplinary Team (IDT) to develop and implement individualized care plans that support participants in maintaining the highest practicable level of independence, safety, and quality of life while remaining in the community.

    This is a fully on-site position based in Los Angeles, CA. Remote work is not available. Applicants must be able to work on-site and be willing to relocate if currently residing outside the area.


    Duties and Responsibilities

    Conduct speech, language, cognitive, voice, and swallowing evaluations for PACE participants. Develop individualized treatment plans based on participant needs, physician orders, and IDT recommendations. Provide skilled therapy services for communication disorders, dysphagia, cognitive deficits, and related conditions. Assess participants for aspiration risks and recommend appropriate dietary modifications and swallowing precautions. Collaborate with dietary staff, nursing staff, providers, rehabilitation team members, and caregivers regarding safe feeding practices and communication strategies. Participate actively in Interdisciplinary Team (IDT) meetings and care planning discussions. Educate participants, family members, caregivers, and staff regarding therapy goals, treatment plans, adaptive techniques, and safety recommendations. Document evaluations, treatment plans, progress notes, reassessments, and discharge summaries in accordance with CMS, state, federal, and organizational requirements. Monitor participant progress and modify treatment plans as clinically indicated. Recommend and train participants on the use of adaptive communication devices and assistive technologies when appropriate. Ensure compliance with infection control, participant rights, safety standards, and confidentiality requirements. Participate in quality improvement activities, audits, and regulatory compliance initiatives. Coordinate care transitions and discharge planning related to speech therapy services. Maintain productivity standards and timely completion of documentation. Perform home visits as needed to assess participant safety and functional needs. Other duties as assigned.


    Qualifications

    Education:

    Master's degree in Speech-Language Pathology from an accredited program preferred. Current and unrestricted Speech-Language Pathologist license in the State of California. Certificate of Clinical Competence (CCC-SLP) preferred. Current CPR/BLS certification required or able to obtain within 90 days of hired.
