Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (APP) for our Pediatric Support Centers located in Columbus on Olentangy River Rd, Altair Parkway in Westerville, and East Main Street in Reynoldsburg, Ohio.
The APP, in collaboration with the Physicians, coordinates the healthcare of ACUTE pediatric patients at these locations. This position monitors the patient’s status and collects data to evaluate and improve care by providing a comprehensive assessment, implementing the appropriate treatment, and evaluating the patient’s progress.
This is a full-time position working a variety of shifts Monday-Friday between 10am -8 pm and some rotating weekends Saturday/Sunday from 9-5 pm.
Essential Functions and Responsibilities:
Develop patient care treatment plans by obtaining medical histories and examining patients during appointments. This includes providing a diagnosis and establishing patient care needs to ensure desired care outcomes.Coordination and care planning with PCP, specialists, and the rest of the care team members. When indicated, facilitate goals of care conversations. Give care to relieve symptoms. Perform routine or annual physical examinations.Treat or refer patients for primary care conditions. Educate patients and caregivers on conditions, outcomes, and treatment plans. Consult with Physicians and clinical staff regarding issues, treatment plans, and progress. Prescribe medication dosages, routes, and frequencies based on individual patient needs. Order, perform, and/or interpret results of diagnostic tests when necessary. Participate in monitoring patient outcomes. Assure quality of care by adhering to professional standards and procedures set by federal, state, and company requirements. Maintain accurate records of assessments, follow-ups, and evaluations.Qualifications
Education, Licensures & Certifications:
Master's Level Degree or above.Must be considered a Certified Nurse Practitioner (CNP) or Physician Assistant (PA) by a nationally recognized board in the state of Ohio – preferably Certified in Pediatrics. Experience working in a primary care setting in Pediatrics as a Certified Nurse Practitioner (CNP) or Physician’s Assistant. Pediatric Nursing experience is required.Knowledge, Skills & Abilities:
Ability to work under pressure. Proficient in clinical practice protocols and delivering primary care.Proficient in Microsoft Office applications. Extensive knowledge of medical terminology.Excellent relationship-building skills. Excellent listening, communication, and documentation skills.Knowledge of principles, practices, and procedures used in the detection, diagnosis, and treatment of common health problems. Knowledge of safety and infection control practices and procedure.Strong problem-solving skills.Possesses a customer-service-oriented mindset.
