Physical Therapist
The Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for the elderly) organization (PO) that uses an interdisciplinary team approach for care planning and implementing purposeful high quality, affordable, and integrated health care services to the elderly. Our elderly meet PACE requirements as prescribed by CMS and are referred to as participants. THE POSITION: Under the supervision of the Rehabilitation Supervisor, the Physical Therapist provides restorative and rehabilitative services in conjunction with the total plan of care.
The hourly rate for the Physical Therapy role at Center For Elders Independence is $ 49.18 to $73.77 per hr. Hourly rate based on the market for the Physical Therapy position, as well as experience, skills, abilities and work history.

DUTIES AND RESPONSIBILITIES:
Collaborates with the primary care provider team and the interdisciplinary team in evaluating agency participants by applying muscle, nerve, joint and functional ability tests.In conjunction with the interdisciplinary team, participates in the development and implementation of the participant plan of care to maximize a participant’s ability to be independent, balanced against their ability to be safe.Instructs participants and caregivers in the care and use of mobility-related devices such as wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.Works with vendors to obtain rehabilitative equipment and supplies for participants.Instructs the interdisciplinary team and family on the participant’s physical therapy program and aspects of physical therapy in which they may work with the participant.As directed by the plan of care treats participants to relieve pain, develop or restore function and maintain maximum performance, using physical means, such as exercise, soft tissue and joint mobilization, functional training, gait training, neuromuscular re-education, heat, sound, water, light, or electricityConducts initial assessments of all participants.Completes reassessments on participants per CEI policy.Observes and records, and reports regularly to the interdisciplinary team, on the participant’s response to physical therapy and any changes in the participant’s condition.Instructs, supervises and evaluates therapy program aides in the implementation of the participant’s individualized rehab plan of care.Provides in-service training to other members of the health team.Attends the interdisciplinary team conferences and actively participates in the development of the plan of care.Evaluates participants’ home environments and makes appropriate recommendations as needed.Attends continuing education and actively participates in meetings and trainings as required. QUALIFICATIONS:
Possession of a minimum of a Bachelor’s degree and is a graduate of a program of Physical Therapy approved by the Commission on Accreditation in Physical Therapy Education. Currently licensed as a Physical Therapist by the Physical Therapy Board of California.Preferred, one (1) year of experience working as a Licensed Physical Therapist with the frail elderly population.Current CPR and First Aid certification on file.A valid California driver’s license, current vehicle insurance, and access to a reliable vehicle is required.Ability to travel to multiple work sites and participants’ residences, required.Ability to perform job duties in a variety of clinical settings, including standing, walking, and moving between treatment areas throughout the workday.Ability to assist and safely support patients during transfers, positioning, and mobility activities.Ability to lift, carry, push, or pull equipment or materials weighing up to approximately 25 pounds on a regular basis, and occasionally up to 50 pounds with assistance.

The Position: The Personal Care Attendant II provides assistance to CEI participants in the participant’s home or day center. The Personal Care Attendant II demonstrates knowledge and skills necessary to provide caregiving services, including personal care and ADL/IADL care to frail elderly people who need help to continue living safely in the community. The Personal Care Attendant (PCA) II provides care and services as determined by the participant’s care plan in accordance with PACE Regulations.
The salary range for the Home Care PCA II role at Center for Elders Independence is $22.50 - $33.76 per hr. Salaries are based on the market for the Home Care PCA II position, as well as experience, skills, abilities and work history.
Department Duties and Responsibilities:
Day Center:
Records services in required logs or electronic health record.Assists individual and group activities, as necessary.Accompanying participants to outside medical appointmentsResponsible for ensuring that all wander risk participants are wearing code alert devices.Obtains and maintains food handler's certification.Assists with food service program, including serving snacks and meals at the day center, checking food temperatures, reviewing diet orders.Performs kitchen duties to include washing dishes, sanitizing work area, maintaining supply inventory, and checking for food expiration dates.
Home Care:
Records vital signs if requested.Provides medication prompting which may include supervision and double check of participant self-administration of insulin.Emptying and cleaning of foley catheters, ostomies and urostomiesDressing and grooming the participant in preparation for day or night. May include providing shaving, oral and skin care, monitoring skin condition, positioning, and turning of participant.Assists participants to complete home exercise program if required.Plans and prepares light meals, followed by clean-up, safely storing and organizing delivered meals and monitoring food expiration dates.Performs light housekeeping including dusting, vacuuming, taking out garbage and recyclables.Performs laundry services including washing and folding laundry, straightening/making beds and changing linens as needed.Follows the home care service plan to complete tasks.

Home Care Coordinator

The Position: Under the direction of the Director of Home Care, the Home Care Coordinator (HCC) conducts home safety evaluations in the participant’s place of residence to determine home care and DME needs. The HCC is an integral member of the Interdisciplinary team and contributes to participants plan of care to ensure that participants are receiving care that is appropriate for their safety and maximum independence in their homes.
Duties and Responsibilities:
Participates as a member of the Interdisciplinary Team (IDT), responsible for care evaluation, planning, management and communication of in-home care needs. Includes ongoing evaluation of home environment in relation to the changing needs of the participant. Completes the initial, annual and semiannual screenings and evaluations of participants timely and as assigned.Assesses changes in participant’s level of care and home care needs based on results of the home care assessment and service plan tool and presents to the IDT for discussion and approval of any proposed change in the Plan of Care.Evaluates participants’ physical and mental health within their scope of practice. Collaborates with Personal Care Attendants on delivering the service plan of participants and reports any concerns in a timely manner.Attends team meetings, IDT meetings, Home Care meetings, family conferences, and other meetings as required by the Center.Participates in continuing education classes and any required staff and training meetings.
