    CDR Health Care Inc. is seeking licensed Advanced Practice Registered Nurses in the Sanderson, FL area to provide healthcare services at a detention facility. The APRN is responsible for independently managing health problems and coordinating health care for patients in accordance with State and Federal guidelines and regulations and the nursing standards of care. We are looking for professional Nurse Practitioners who can deliver primary medical care to a wide variety of patients at our site.

    Serve as a resource for patients in need for care information, counseling, and guidance for any patient conditions in an outpatient setting Provide patient education about clinical conditions, diagnostic testing, preventative care, and prescribed treatments. Facilitates staff, patient and family decision making by providing educational toolsPerform initial patient screenings, obtain past medical and medication hx, completes ROS, Functions independently to perform age-appropriate history and physicalOrders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.Analyze test data and determine the need for follow-up appointments and further treatment optionsCollaborates with multidisciplinary team members by making appropriate referrals.Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities. Update patient records and check records for accuracy at each patient appointmentDeliver quality care while maintaining all company metrics pertaining to productivityPromoting a safe environment

    Requirements

    Current Advanced Practice Registered Nurse (APRN) Licensure in FloridaGraduate of an accredited school of nursing Graduate of an accredited Nurse Practitioner ProgramFNP BC Family Nurse Practitioner certification for FloridaMinimum 2 years’ experienceIM/IV skills along with experience performing basic procedures Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS); PALS provider card Must be familiar with the EHR medical charting systemMaintain Strong and current infectious disease knowledge and practicesStrong written and interpersonal communication Ability to perform independently and as part of a team.Ability to handle high stress situationsFull immunization record must be presented before hire, Covid vaccine not included.Bilingual (Highly Preferred not Required)

    CDR Health Care Inc. is seeking Certified Nursing Assistants (CNAs) based in the Northern Florida area who are willing to travel to Sanderson, FL, to provide healthcare services at a detention facility. This role will provide essential support to patients by assisting with daily living activities and helping licensed medical staff during clinical procedures. The position involves monitoring patient conditions, maintaining a safe and clean environment, facilitating patient mobility, and ensuring effective communication within the healthcare team to promote high-quality, compassionate care.

    Please note that this is not a travel assignment; local candidates based in Sanderson, Lake City, or Jacksonville are preferred.

    Essential Functions:

    Monitor and document vital signs, behavioral changes, and physical condition; promptly report concerns to nursing or medical staff.Assist with in-clinic patient transport, including moving individuals between exam rooms, infirmary beds, and procedure areas.Reposition patients in medical beds to prevent pressure injuries and promote comfort.Support patients under medical observation by providing supervised mobility assistance as part of recovery or treatment plans.Maintain infection control standards by routinely cleaning medical equipment and adhering to PPE and sanitation protocols.Assist licensed nursing staff during intake exams, sick call visits, and minor procedures by retrieving supplies and helping with patient positioning.Other duties that may arise from time-to-time and/or are commensurate with the title and position.

    Requirements

Key duties include conducting physical assessments, performing diagnostic screenings, administering treatments, maintaining accurate medical documentation, and responding to on-site medical emergencies. The position also involves supervising support staff, upholding infection control protocols, and resolving operational challenges to support a safe, efficient, and patient-centered care environment.

    Essential Functions:

    Oversee daily site operations to ensure adequate staffing, supplies, and equipment are consistently available.Serve as the primary liaison between the site, CDR Health, and the medical leadership team.Identify and resolve operational issues such as parking logistics, surge capacity, and staffing shortages.Maintain quality assurance by training staff and monitoring the effectiveness of site testing procedures.Provide break relief for clinical staff and assist with specimen collection as needed (PRN).Supervise CNAs and LPNs to ensure high standards of care and smooth workflow in the medical unit.Educate patients on their medical conditions, medications, and treatment plans in a clear, compassionate manner.Maintain accurate and up-to-date medical records in the electronic medical record (EMR) system.Conduct initial medical intake screenings for new detainees, including health histories, vital signs, and TB risk assessments.Perform comprehensive nursing assessments for patients admitted to the medical ward.Administer medications per provider orders and MAR, including injections, wound care, and chronic disease management.Triage complaints during sick calls and determine appropriate clinical action or provider referral.Monitor and manage high-risk patients, including those under detox protocols, suicide watch, or other observation categories.Respond to in-clinic emergencies, delivering ACLS/BLS care and coordinating further treatment when necessary.Enforce infection control standards, including sanitation practices, PPE compliance, and isolation protocols.Document patient refusals of care, medication, or observation and escalate concerns when necessary.Participate in suicide watch procedures, including direct monitoring, timely documentation, and mental health referrals.Other duties that may arise from time-to-time and/or are commensurate with the title and position.

    Requirements

    Associate’s or Bachelor’s degree in Nursing required; Master’s degree in Nursing is a plus.Minimum of 3 years of experience as a Registered Nurse preferred.Current, active RN licensure in good standing in the state of FL or a compact license.Current BLS certification at the medical provider level, with the ability to deliver appropriate patient care in urgent or emergent situations while awaiting EMS.Recent clinical experience within the last 6 months, including proficiency in specimen collection.Strong organizational and time-management skills.Ability to wear personal protective equipment (PPE) for extended periods without restriction.Excellent verbal and written communication skills, with the ability to interact effectively with diverse populations and in varied settings.Demonstrated ability to make sound clinical decisions under pressure.Capable of multitasking and thriving in a fast-paced environment while meeting deadlinesAble to remain calm and effective in high-pressure situations.Flexible availability, including late afternoons, evenings, weekends, and overtime as needed to respond to emergencies or critical incidents.Willingness to relocate if needed.
