    Care Coordinator:

    Are you looking to join a dynamic team focused on providing high quality health care to communities across Brooklyn & Queens? If so, you’ve come to the right place. For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. Catholic Charities provides comprehensive care coordination and treatment services to individuals living with serious mental illness, complex medical needs and substance use needs. Under the NYC Department of Health and Mental Hygiene, our Non-Medicaid Care Coordination Program works with individuals who do not qualify for Medicaid and are living with serious mental illness, to deliver comprehensive, community-based services and ensure clients have access to uninterrupted and coordinated behavioral and physical health services while addressing the social determinants of health that impact daily living. Care Coordinators address a host of issues that impact clients directly such as housing, access to nutritious food, economic security/benefits, medication adherence, linkage with outpatient treatment providers or other community resources and social supports.

    RESPONSIBILITIES:

    The Care Coordinator has overall day-to-day responsibility and accountability for coordinating all aspects of an individuals’ care with complex and/or psychiatric co-morbid conditions and for facilitating their access to the full range of medical and psychosocial services in an efficient and effective manner. Individuals are provided care in their home/community at least twice monthly, and more frequently if needed. Duties of the Care Coordinator focus on integration and coordination of physical health, mental health and overall social needs. The Care Coordinator must become an active participant in all phases of care transition to assure that enrollees received all required mental and medical follow up care and services and re-engagement of patients who have become lost to care. The Care Coordinator electronically monitors and tracks data regarding the individual and alerts all members of the Care Team when follow-up is required.

    • Accountable for engaging and retaining individuals in care, coordinating and arranging for the continuous provision of services, supporting adherence to treatment recommendations, monitoring and evaluating their needs, including prevention, wellness, medical, specialist and behavioral health treatment, care transitions and social and community services where appropriate through the creation of an individual plan of care.

    • In collaboration with the client, their family and/or caregivers, and other service providers develops, manages and coordinates a comprehensive individualized person-centered care plan that coordinates and integrates the continuum of medical, behavioral health services, rehabilitative, long term care and social service needs and clearly identifies the primary care physician/nurse practitioner, specialists, behavioral health care providers, care manager and other providers directly involved in the individual’s care.

    • Ensures the availability of priority appointments for clients to care services including physical, psychiatric, and substance use within their provider network to avoid unnecessary, inappropriate utilization of emergency room and inpatient hospital services.

    • Promotes evidence-based wellness and prevention by linking health home members with resources for smoking cessation, diabetes, asthma, hypertension, self-help recovery resources and other medical services based on individual physical needs and preferences.

    • Tracks and shares client information and care needs across providers by utilizing electronic databases and monitors outcomes and initiate changes in care, as necessary, to address client needs.

    • Reassesses needs for services and reviews clients’ historical or targeted clinical measurements (i.e. number of ER visits and inpatient psychiatric admissions).

    • Identifies potential barriers to successful care and resolutions to those barriers.

    • Completes contact notes, incident reports, and other required documentation and maintains accurate recordings in electronic case files.

    The Care Coordinator is required to utilize technology and various web-based platforms for documenting progress notes and daily work activities. Ability to use equipment such as iPhones, tablets, Surface Pros and easily navigate various technology platforms and reporting systems is a requirement.

    SPECIFICATIONS FOR EDUCATION/CERTIFICATIONS/LICENSES

    • Bachelor’s degree in social work, psychology, or a related health/human services field with 2 years of direct work with the target population or Degree/Certification in Medical and Clinical Assistance or Health Professional field.

    SPECIFICATIONS FOR EXPERIENCE AND TRAINING

    • Combination of skills in the areas of crisis intervention, time management, and psychosocial rehabilitation skills.

    • Ability in linking clients to a broad range of services essential to successfully living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing and financial services).

    • Must have excellent communication skills.

    • Cross-cultural competency, outreach, interviewing, listening, advocating, linking, negotiating, engagement, monitoring and clinical assessment skills are essential.

    • Knowledge of community medical resources and their financial requirements.

    • Excellent computer skills.

    • Ability to read and write in English

    • Knowledge of computer programs (MS Word, Excel, PowerPoint, & Outlook).

    • Good communication skills. • Knowledge of the second language is preferred (Spanish, Russian, Creole, Cantonese).

    POP Management is an in house property management company of Catholic Charities Brooklyn & Queens. POP Management is the largest provider of affordable housing in Brooklyn & Queens. POP Management provides affordable housing to low-income seniors and to those with documented history of homelessness, substance abuse, and chronic mental illness. POP Management oversees and manages 25 senior buildings with 2,400 units and 4 supportive residences with 300 units for formerly homeless adults/veterans consisting of 300 units. The objective of POP Management is to provide high quality housing that responds to the needs of low-income seniors and supportive populations.


    The Handyperson works as an integral member of the on-site maintenance team to ensure a safe work environment is provided and maintained for all occupants and visitors in buildings managed by POP Management, the housing office of Catholic Charities, Brooklyn & Queens.. The Handyperson is responsible to perform routine repairs in and around the property while adhering to a standard that meets all outside monitoring requirements to ensure a safe and clean environment. Additionally, the Handyperson is responsible to participate in the duties of the regular cleaning and sanitizing of the property.  The Handyperson ensures that required safety standards are met.
    * Work closely and cooperatively with the Superintendent to ensure optimum maintenance of the property.

    STATEMENT OF THE JOB 
    Under the direction of the Clinical Director for Behavioral Health, the Supervisor has the responsibility for clinical and administrative supervision in CCNS PROS outpatient services.  Responsibilities also include supervision of staff in the outpatient programs, assuring that program staff carries out their job responsibilities in a professional and ethical manner and maintain availability to staff for consultation during all program hours of operation.
    The Clinical Supervisor will be responsible for overseeing the delivery of care in order to ensure the appropriateness and effectiveness of outpatient services, identification of gaps in service delivery system, representing the Agency at meetings, conferences and public forums, strategic planning and implementation, development and implementation of processes and services that will improve the service delivery system, program development – including monitoring revenue, productivity, outcomes, staff training and development and staff supervision, compliance with contracts, agency policies and procedures, regulatory requirements and audit readiness and new initiatives.
    •    Monitor and oversee all aspects related to the delivery of PROS outpatient services – including program site visits on all staffing work shifts.
    •    Plan, coordinate, identify and implement program outcomes and evaluation, strategic planning, accreditation efforts and program development.
    •    Provide and/or oversee supervision of staff and graduate students.
    •    The position requires the ability to provide coverage and supervision at the programs or in situations which require 24 hour/7 day per week response.
    •     Facilitate program compliance with fire, safety, and health standards as per licensing and or quality assurance requirements and standards.
    •    Perform initial interviews, crisis intervention, and on-going therapy with individuals, & children, run groups, work with families, & couples as dictated by the client’s clinical needs. 
    •    Facilitate staff’s understanding and incorporation of Agency determined Best Practices, Integrated Delivery and/or Evidence Based Practices models as needed.
    •    Identify and report of Agency inefficiencies that negatively impact on program ability to provide services and function in an efficient and professional manner.


    QUALIFICATIONS
    •    Master’s Degree in Social Work from a nationally accredited institution. 
    •    Valid NYS LCSW required.
    •    Excellent clinical & supervisory skills along with three years of mental health &/or substance abuse experience. 
    •    Requires a combination of skills in the following areas: operational oversight and evaluation of program services and staff, personnel management, public relations, governmental relations, leadership development and team building skills.
    •    Time management, organizational, and computer skills. 
    •    Ability to evaluate and intervene in crisis situations. 
    •    Excellent oral and written communication skills. 
    •    Comfortable with public speaking, lobbying and advocacy. 
    •    Flexibility & Creativity. 
    •    Bilingual Spanish speaking preferred.

    STATEMENT OF THE JOB

    •    Planning, coordinating, identifying and implementing program outcomes and evaluation, strategic planning, accreditation efforts and program development.
    •    Provide and/or oversee supervision of staff including ensuring coverage and supervision in situations which require 24 hour/7 day per week response.
    •    Involvement in the RFP process, CQI initiatives and other grant initiatives as needed.
    •    Participate in the Agency’s response as it relates to the NYC designation of being a Tier One Response organization.
    •    Participate in the development of appropriate program budgets and monitoring program adherence.


    QUALIFICATIONS
    •    Master’s Degree in a Human Service-related field from a nationally accredited institution. 
    •    Valid NYS LCSW.
    •    Upon eligibility, obtain/retain Medicare UPIN credential and Managed Care Provider applications required for programs that bill 3rd party payers for services.
    •    Three (3) years of supervisory experience
    •    Experience in providing services to persons with psychiatric or cognitive disabilities or comparable experience.
    •    The position requires a combination of skills in the following areas; administrative and clinical supervision and practice, design, operational oversight and evaluation of program services and staff, personnel management, budget development and control, property management, public relations, governmental relations, leadership development and team building skills.

    STATEMENT OF THE JOB 
    The Agency has embraced the concept of a fully integrated system of care for all consumers of service.  This requires that staff assess the client’s needs holistically and see it as their responsibility to connect the client with services needed both internal and external to the Agency, regardless of the door through which the Consumer enters the system.  
    Under the direction of the Clinical Director, the Licensed Social Worker (LMSW) has responsibility for direct service to clients in the PROS program.  All services are to be delivered in a professional and ethical manner, consistent with best practice tenets.  The Licensed Social Worker will function as part of a treatment team, maintain mandated levels of service, participate in Agency meetings as required and fully comply with Agency policies and procedures.  The Licensed Social Worker will conduct assessments, clinical intervention, classes and groups, provide crisis services, engage families and significant others in participants’ recovery process, promote wellness and inspire hope, assist participants with exploration of barriers to and goals of recovery.  
    The Licensed Social Worker adheres to all standards of contract compliance, audit readiness, Agency policies and procedures and all other regulatory requirements.  Work as a member of a team to help ensure the overall integrity and welfare of the Agency. 
    •    Meet Agency requirements for qualitative and quantitative contacts with participants.  Includes conducting intake interviews, rendering diagnoses in accordance with DSM criteria, providing crisis intervention, providing intervention in various treatment modalities, conducting telephone referrals, providing advocacy, case management and referrals. Provide services in all PROS components.  Develop WRAP plans with participants.  
    •    Meet Agency requirements for qualitative and quantitative documentation.  Includes writing progress notes, recovery and wellness plans, intake assessments, correspondence regarding clients, termination summaries, encounter forms, case notes, group notes and any and all other required documentation. 
    •    Assess the needs of the participant including basic needs such as food, clothing and shelter.
    •    Ensure that referrals are made to appropriate internal and external services as required to meet the needs of the participants and their families.
    •    Follow-up on referrals made on behalf of the participant and respond to all referrals in a timely and professional manner, recognizing the need for effective collaboration in order to insure the highest levels of service provision.
    •    Coordinate physical, medical and psychological needs for clients.
    •    Responsible for all administrative and clinical documentation associated with direct service clients.  Maintain content and format of casework record according to regulatory guidelines.
    •    Provide and/or participate in trainings related to behavioral health services as well as those specific to and required by PROS license.


    QUALIFICATIONS
    •Master’s degree in social work from a nationally accredited institution. 
    •Valid NYS LMSW required.
    •NYS LCSW license preferred.
    •Obtain NPI (LCSW) and Medicare Provider (LCSW) number.
    •Frequently lifts and/or moves up to 10 pounds. 
    •Able to travel to multiple locations within the five boroughs as needed
    * Must be very well organized.
    * Bilingual Spanish speaking preferred.                                            
    * Have excellent oral & written communications skills.                      
    * Must be computer literate & knowledgeable of MS Word, Excel, & Outlook.

    Older Adult Centers promote an active life for seniors. The centers aim to assist them to preserve their independence and health by offering various range of activities and services such as serving breakfast, lunch, health screenings, social services, educational workshops, class exercises, discussion groups, games, art classes, lectures, workshops, organized outings, and special events.

    STATEMENT OF THE JOB

    The Assistant Cook assists the Cook to prepare and distribute congregate meals; cleans kitchen area and appliances; properly stores food items, while maintaining sanitary and heath standards according to DOH and funding source regulations, ensuring the food is stored and served at the proper temperature in accordance with NYC DOHMH regulations. The Assistant Cook will be responsible for oversight of the kitchen inventory and supplies.

    Prepare meals under the guidance of the Cook.Clean and maintain kitchen area and related equipment.Maintain sanitary and safety procedures in kitchen and dining areas.Serve food and ensure proper portion control in accordance to USDA’s Dietary Guidelines for Americans, the New York City Food Standards and DFTA guidelines to seniors adhering to NYC DOHMH regulations for food service establishments.In the absence of the Cook, assume responsibility in oversee and maintain the food related areas and equipment in accordance to according to NYC DOHMH Article 81.Oversee safe handling and temperature control of prepared foods.Receive, date and store all food items.Inspect all food deliveries for freshness and correct amount ordered.Develop meals recipes and introduce to seniors, work on nutritional brochures with cooking recipes, conduct culinary classes.

    QUALIFICATIONS:

    High School Diploma or equivalency preferred but not required.Valid Food Handler’s Protection Certificate.Previous experience in a commercial kitchen preferred.Able to prepare nutritious and appealing meals.Able to operate independently in the Cook’s absence.Able to travel to multiple locations within the five boroughs as needed.Frequently lifts and/or moves up to 25 pounds.

    STATEMENT OF THE JOB
    The Driver will be required to transport older adult clients on regularly scheduled routes to various destinations. Escort and assist infirm clients from their homes to the vehicle and back. Perform clerical functions including data entry, filing, answering phones and distributing mail.
    • Transport clients on regularly scheduled routes from their homes to destinations such as senior centers, supermarkets, shopping malls, medical appointments as determined by the Director or other assigned staff.
    • Escort and assist the fragile clients from their homes to the vehicle and back, if necessary.
    • Ensure the safety of the passengers through the use of seat belts, wheelchair brackets and other safety devices.
    QUALIFICATIONS:
    • High School Diploma or GED preferred.
    • Valid New York State Driver’s License (Class D) and provision of printout of annual Driving Record from Motor Vehicle Bureau.
    • Must obtain defensive driving certificate within 6 months of employment.
    • One (1) year driving experience with no more than one moving violation within a 24-month period, no driving while intoxicated convictions.
    • Knowledge of roadways in the area where program operates.
    • Ability to lift to 25lbs.
