    Job Summary and Qualifications

    Summary:

    The Licensed Practical Nurse (LPN) within the PACE program plays a vital role in supporting our participants through comprehensive medication oversight and team coordination. Guided by the PACE model of care, which emphasizes an interdisciplinary, participant-centered approach focused on promoting independence and allowing individuals to remain safely in their homes and communities, the LPN serves as an essential member of the clinical care team. This position is responsible for ensuring medications are administered and documented accurately, assessing changes in condition, and providing hands on care. In collaboration with providers, pharmacy, caregivers, and the interdisciplinary team, the LPN supports communication, care coordination, and continuity of services to promote optimal participant outcomes and enhance quality of life.

    What qualifications you will need:


    Essential Accountabilities:

    Provides direct patient care in an outpatient setting, evaluates outcomes and adjusts nursing care processes indicated to adhere to legal, ethical, and clinical standards.

    Performance Criteria:

    1. Evaluates and records patient vitals
    2. Assists provider with therapeutic and diagnostic procedures
    3. Administers oral/injectable medications as ordered and monitors patient's response
    4. Delegates to other licensed and unlicensed staff according to scope of practice and competency.
    5. Delivers quality customer service to all patients and family members.
    6. Triages participants and addresses their needs and concerns. Makes clinical judgement based on their symptoms to determine if the need to see the physician is necessary. Treatment determined by urgency of need.
    7. Coordinates care as indicated with specialist providers and nursing facilities
    8. Coordinating with care team to implement goals and interventions as it relates to the clinical team
    9. Assessing participants medical needs and interventions to create success in the community and creating plan of care to support needs

    10. Coordinator of care, orders, and interventions as prescribed with SNF Partners.

    11. Collaborates and consults with patient, colleagues and interdisciplinary team to achieve desired outcomes.

    Performance Criteria:
    1. Regularly documents and reports data, patient response and progress.
    2. Individualizes patient teaching and care plan based on patient's needs.
    3. Helps to achieve desired outcomes by serving as an advocate for patients and families.
    4. Identifies problems and implements measures to meet needs of patients and families
    throughout continuum of care.

    Basic Cardiac Life Support must be obtained within 30 days of employment start date(LPN/LVN) Licensed Practical or Vocational Nurse must be obtained within 2 years of employment start dateAssociate Degree
