Job Title/Position: Bereavement Coordinator
Reports To: Clinical Director
JOB DESCRIPTION SUMMARY
The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement
program to meet the needs of families/caregivers from Capital Hospice and Palliative Care LLC
for up to one (1) year following the death of the individual hospice patient.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Coordinates the assessment and delivery of grief counseling needs and services
(one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are
provided to family/caregivers.
2. Plans for Bereavement Program development, expansion and refinement through annual
evaluation of services.
3. Provides bereavement support to hospice personnel coping with work related grief through
one-to-one counseling, supervision of interns providing one-to-one counseling, and referral
to community resources.
4. Provides bereavement information and referral services to callers from the community.
Expands and maintains community resource list as needed.
5. Assists as needed in evaluating and planning services to meet hospice personnel support
needs (i.e., yearly retreats, weekly support groups).
6. Functions independently, but seeks and accepts guidance from other members of the
interdisciplinary group, or from member of the community.
7. Participates in hospice activities such as in-service education, hospice personnel meetings,
and relevant committees.
8. Advances professional knowledge and skills.
9. Actively participates in quality assessment performance improvement teams and activities.
10. Accepts and performs other related duties and responsibilities as required.
POSITION QUALIFICATIONS
1. Bachelor's degree or equivalent theological degree from college or university. Experience
with families/caregivers and patients through death/dying issues – OR – Master's degree in
Social Work with qualifications of experienced clinical grief counseling.
2. Ability to work as a team member.
3. Knowledge and commitment to hospice philosophy.
4. Demonstrates good verbal and written communication, and organization skills.
5. Understanding of grief response and experience with counseling individuals and/or in
groups regarding loss.
6. If making home visits, must be a licensed driver with an automobile that is insured in
accordance with state or organization requirements and is in good working order.
Applicant will be responsible for assessing the psycho-social needs of patients and for providing information and identifying resources to assist the patient and family with issues relating to terminal illness, disease processes and needs within the home. Knowledge and/or experience with end-of-life care and licensed in the state of Illinois.

Assists the patient, significant others, physician, and health care team to understand significant personal, emotional, environmental, and social factors related to the patient's health status on as needed basisContributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis.Instructs health care team members on community resources available to assist patients on a as needed basis.Visits patient according to Plan of Care; completes a clinical note for each visit; and submits clinical notes to the agency on a daily basis.The MSW will give daily report to the Administrator regarding patient visits to ensure continuity of the Care Management process.Plans for continuity of care with hospitals and community agencies on an as needed basis. Assesses social and emotional factors related to patient's illness to determine ability to cope with daily living problems on as needed basis.Assists the patient and significant others to understand, accept and follow medical recommendations on as needed basisQualifications and Skills
Master Degree in Social Work.Minimum 1 year experience in hospice.Must be a licensed social worker in the state of Illinois.Current CPR Certification.Must have a valid drivers license and current automobile insurance.Basic understanding of age-related differences in caring for and communicating with patients of this type.Must possess the ability to work independently without supervision.Effective problem-solver and use good judgment in difficult situations.Applicant will be responsible for assessing the psycho-social needs of patients and for providing information and identifying resources to assist the patient and family with issues relating to terminal illness, disease processes and needs within the home.
