Job Summary: A Physical Therapy Assistant is someone who provides coordinated physical therapy to patients of all age groups under the supervision of a licensed Physical Therapist. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.
QualificationsQualifications: Physical Therapy Assistant, licensed per the state requirements; 2 years of Home Health preferred; Good communication skills, assessment skills, and strong clinical documentation skills; Ability to read, write, and speak the English language; Reliable transportation. Valid and current auto liability insurance
Essential functions: Treats patients as directed by the Physical Therapist; Records treatments administered in medical record; Records clinical documentation according to accepted regulatory, corporate and professional guidelines; Participates in weekly case conferences; Participates in interdisciplinary communication to ensure that patients needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress; Provides verbal or written patient status updates to supervising PT prior to reassessment and discharge visits, and as indicated by patient response to treatment; Participates in in-service training programs; Responsible for assuring patient safety; Complies with state regulations and national standards as they apply to the practice of physical therapy; All documentation will be completed and synched by 9:00AM the following day, 7 days per week.

DescriptionThe overall purpose of the certified medication aide position is to administer oral and topical medications to patients/residents.
