    The Care Management for High-Risk Pregnancy Case Manager will perform intermediate-level care coordination independently, providing education, referrals, and support to pregnant women with identified risk factors. The Case Manager will collaborate with Prepaid Health Plans (PHP), the Pregnancy Medical Program (PMP), OB providers, and community partners to ensure program performance standards and quality measures are achieved.

    Primary Responsibilities:

    Person-centered interactions in-person at Medical Home, home visits, telehealth, or phone call Assess member needs, develop person centered care plans and goals related to the risk factors identified Provide member education, referral, and follow-up based on need, risk or condition Coordinate service plans with other agencies as required per member need Complete documentation in Virtual Health on a daily basis Make referral for service to CHA, ancillary agencies and community resources

    Education/Experience: Graduation from an accredited four-year college or university with a bachelor's degree in social work, a master's degree in social work, a B.S. degree in nursing, or graduation from an accredited school of professional nursing with two years of professional experience, one of which is preferred in public health or hospital maternity setting with experience in case management

    License: Valid license to practice as a Registered Nurse in North Carolina, Valid NC driver's license, current CPR certification

    The Public Health Nurse II will provide program / client services in Communicable Disease (CD) areas which include communicable disease follow-up, community education and outreach, HIV testing and counseling, immunizations, international travel consultations, outbreak management, and tuberculosis using federal, state, and local laws to support public health intervention and practice.

    Primary Responsibilities:

    Management and delegation of hepatitis program; including investigation, follow-up, case management, and state auditsManagement and delegation of rabies monitoring, investigation, and follow-up Begin STI Enhanced Role RN certification at next available cohort and oversee STI program including testing, treatment, and state audits once ERRN certified Investigation and follow-up of communicable diseases and input information into NCEDSSEvaluate immunization records and administer vaccines as indicated/needed per standing orders, in conjunction with CDC, NCDPH guidelines, and NC Administrative Codes Assist with TB follow-up and case management Provide education and clinical services at occasional on-site and off-site community events

    Education/Experience: Graduation from an accredited nursing program. BSN preferred. Public health clinical rotation and previous public health or communicable disease experience strongly preferred. Bilingual in English and Spanish preferred.

    License: Must have a current license to practice as a Registered Nurse in North Carolina. Valid NC driver's license. Non-BSN nurses must also complete the introduction to Public Health Nursing course within the first six months of employment.

    Primary Responsibilities:

    Provide case managementFollow peer support code of ethics Conduct intakes and enrollment activitiesDevelop care plans for clients Assist team with daily data tracking and monthly reports Document appropriately in electronic health record Assist with client needs and goals Accompany clients to meetings and participate in meetings of community groups to advocate for client needs

    Education/Experience: High school diploma or equivalent required. Certified as a Peer Support Specialist or currently working toward certification as a Peer Support Specialist by the governing agency required. Must be over the age of 18 and living in recovery from mental health/substance use disorder or lived incarceration experience. Prior case management experience preferred.
