    Brooksby Village by Erickson Senior Living ALL CLIENTS ARE BASED AT 1 LOCATION IN PEABODY MA - NO DRIVING We are flexible for either 24 or 32 hours. This position includes participating in a weekend rotation (which would fall approximately every 5-6 weekends). This is a full work day on Saturday and Sunday with the option of taking off a day during each of the corresponding weeks, as well as being on-call from 8:30am Saturday - 8:30am Monday. The weekend differential is $3/hr and on-call is paid a stipend of $159.

    Join our team as Registered Nurses for Home Care. The RN in Home Care provides nursing care according to established policies and procedures for a home care agency. The RN develops and supervises the implementation of a resident's individualized service plan, observes the resident's condition, and communicates to the Physician and family/responsible party as needed. The RN oversees the resident's care delivered by the Home Support team of LPN's, Aides, and Home Care workers and companions.

    Partnering and collaborating in the initial evaluation to assess the residents' needs to develop and implement a service plan based on the evaluation and in conjunction with the resident, family, physician, and /or authorized practitionerCommunicating any changes in the resident's conditionEnsuring that the resident's individualized plan is implemented and followed by the Home Support agency team of providersDelivering nursing care as outlined in the plan of care and according to acceptable nursing standardsParticipating in care conferences and other resident-related meetingsParticipating in the orientation and training of other agency nurses as well as training any Aide staff development activities

    What you will need

    Must have a minimum of 6 months' experience as an RNExperience in caring for seniors and/or senior with cognitive impairment preferredBasic computer skills required including experience with Microsoft Office, internet and web applicationsExperience with an electronic medical record is preferred

The Clinical Manager is an active member of the clinical leadership team who is responsible for the assessment, coordination, implementation, and evaluation of the delivery of safe and effective person-centered care in their assigned neighborhood. The Clinical Manager will also facilitate, precept, and mentor staff to grow professionally and advance their knowledge and skills.

    Compensation: Salary range $110,000-$120,000 per year with potential for annual bonus up to 8%. Salary based on experience

    How you will make an impact

    Coordinating, implementing, and overseeing the clinical care of guests/residents in skilled nursing/long-term care neighborhoods Maintaining clinical practice and processes in accordance with the Erickson Senior Living Management Policies and Procedures and nursing standards of practice. Ownership of documentation process, including MDS, care plans, incident reporting, and charting requirements Ensuring appropriate staff is on duty based on guest/resident acuity and provides recommendations based on identified needs of the neighborhood. Monitoring all aspects of infection control for staff and guests/residents and collaborating with clinical leadership to develop an appropriate process to ensure effective infection control monitoring, including antibiotic stewardship. Analyzing, developing, and implementing Quality Improvement measures for clinical indicators/benchmarking such as Falls, Pressure Ulcers, Medication Errors, Restraint Utilization, Re-hospitalization, Anti-psychotic utilization, ADL splits/documentation, etc. Collaborating with Staff Development Coordinator to ensure all required training and competency reviews are completed according to federal, state, and local regulations. Participating in clinical leadership meetings such as Wound Rounds, Clinical Operations Meeting, Utilization Review, and Performance Improvement/Risk Management/Safety (PI/RM/S) Committee Provides leadership and guidance during annual survey/complaint investigations as well as other regulatory audits/reviews

    What you will need

    Minimum 3 years' experience as a RN on a nursing unit (skilled nursing, long term care, assisted living or hospital) is required. Current Registered Nurse license for the state in which they operate. Current CPR required Basic computer skills required including experience with Microsoft Office, internet and web applications, Experience with electronic medical record preferred.
