    We are seeking a dedicated and experienced Director of Nursing and Clinical Services to join our Remote Care company. This pivotal role will oversee all remote clinical care services, ensuring the highest standards of patient care are maintained. The Director of Nursing and Clinical Services will have direct supervision of Nursing Care Delivery and Tele-Nutrition teams and will report directly to the Chief Nursing Officer. This role will collaborate closely with quality, product and program development.


    Key Responsibilities:

    Manage and oversee all remote employees under the care team that deliver clinical services, including nursing, Tele-Nutrition. Additional care delivery teams and services may be added as expanded coverage occurs.Ensure compliance with healthcare regulations and standards.Develop and implement clinical protocols and procedures.Lead and mentor clinical teams to achieve excellence in patient care.Collaborate with the Chief Nursing Officer and other senior leadership to align clinical services with organizational goals.Collaborate with the Director of Program Development and Director of Quality and education to ensure all aspects of care team development and program delivery are safe, effective and efficient.Collaborate with product and engineering to ensure needs of the care team are met and accounted for in planning and strategic development of organizational growth.Monitor and evaluate the performance of clinical services and implement improvements as needed, including implementation of KPIs set for clinical services.Stay current with industry trends and advancements in remote healthcare delivery.Ensure productivity and code completion meet standards and expectations for growth and development.Design and implement a scalable care coordination service line for remote care supporting Providers, Health Systems and Payers in providing:

    - Remote Patient Monitoring (RPM)
    - Chronic Care Management (CCM)
    - Principal Care Management (PCM)
    - Advanced Primary Care Management (APCM)
    - Tele-Nutrition

    Additional duties and responsibilities as assigned.Requirements:

    Qualifications:

    Minimum of 5 years of clinical experience.At least 3 years of leadership or management experience.Experience with remote care and Current Procedural Terminology (CPT) codes associated with remote care delivery/services.Firm understanding and comprehension of compliance and regulatory requirements for clinical services associated with remote care delivery.Active, Unrestricted Registered Nursing License in one of the following required for hire: Massachusetts, New York, Connecticut. (Additional licenses required as identified)Graduate of an accredited professional nursing program.Bachelor’s degree in nursing or other health related field required, Masters preferred.Willingness to obtain additional state licensure as required throughout employment.

    Key Responsibilities:

    Provide strategic administrative support to the executive team based in multiple Brook office.Manage the administration of the Seattle office, and support / provide assistance to the other Brook offices as needed.Coordinate and oversee administrative duties to ensure efficient office operationsBuild and design PowerPoint decks to help put together proposals and presentationsBe a true strategic executive partner anticipating the needs of our executive team, and weighing the impact of changing priorities which may require independent decision making.Calendar and Meeting Management: Coordinate the executive team's calendar, scheduling of appointments, meetings, and travel arrangements.Prepare meeting agendas, PowerPoint presentations, and documentation. This role functions as a remote patient monitoring nurse with an individual patient case load. Accountable for the application of care pathways for remote clinical nursing practice and patient care coordination for chronic disease diagnoses. There is responsibility to manage the resources with mindfulness regarding adherence of regulatory and professional standards, including Health Insurance Portability and Accountability Act (HIPAA) as well as any additional security requirements identified. It is imperative to foster collaborative relationships within all areas of the interdisciplinary workings of the contracted clinic/health care provider and Brook as a partnership.

    Job Overview


    Brook Health is seeking a dynamic, strategic, and experienced Vice President of Customer Success to lead and scale our Customer Success function. This leader will be responsible for building a high-performing team and defining the systems, strategies, and processes needed to deliver exceptional value to our provider partners. The role is ideal for someone who thrives in fast-paced, high-growth environments and has a deep understanding of the healthcare landscape—particularly provider organizations and clinical workflows.


    This position will initially report directly to the CEO and will later transition to report to the Chief Revenue Officer (CRO) as our commercial organization evolves.

    Key Responsibilities

    Build and scale a Customer Success organization that is structured, high-impact, and aligned with Brook’s mission and growth goals.Develop and retain top-tier talent, fostering a culture of performance, ownership, and innovation.Define and manage the provider customer journey, ensuring seamless onboarding, adoption, and ongoing success.Drive measurable improvements in customer outcomes, engagement, satisfaction, and expansion.Establish scalable systems, tools, and playbooks to support a consistent and repeatable customer success process.Act as a strategic partner to provider organizations, integrating Brook’s solutions into clinical workflows and operational systems.Serve as a key voice of the customer, driving cross-functional collaboration and informing product and service strategies.Represent Customer Success as a member of the broader leadership team, contributing to overall company direction.

    Qualifications
