POSITION SUMMARY
The Family Nurse Practitioner (FNP) or Physician Assistant (PA) will be responsible for direct patient care and for other administrative duties as designated by the Director of Primary Care & Quality. The Medical Doctor will provide the overall direction for all patient care activities, and the Director of Primary Care & Quality will provide you with administrative supervision.

Primary Care Provider providing complete patient history, physical examinations, diagnosis, and plan for treatment of primary, secondary, and tertiary care services as outlined in, and in compliance with the standardized protocols for Nurse Practitioners and Physician Assistants.Primary care delivery includes communicating with patients or guardians of patients regarding care plans, results of diagnostic studies, and answering patient and family questions. Also included is organizing and delivering health education for patients, including preventative health information and disease-specific education. The primary care provider coordinates patient referrals for specialized health care to public health care agencies. Reviews patient file/record, including allergies, problems, medications, and immunization status. Orders or executes various tests, analyses, and diagnostic images to provide information on a patient's condition. Analyzes reports and findings of tests and examinations, and diagnoses the condition of patients. Administers or prescribes treatments. Determines and prescribes medication, dosage and schedule given the patient’s condition and allergies. Prescribes vaccinations to immunize patients from communicable diseases. Provides prenatal care to pregnant women. Provides postnatal care to mothers and infants. Perform the care services listed below (or be willing to learn those services). Serves as a resource for community agencies. Use technology tools for research, data entry, record keeping, and business communication via email. Meet all clinical obligations of the medical practice: Accrediting Agency compliance, CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, etc. Follow all internal policies and procedures; be accountable for completion of all operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentation. Demonstrate ongoing support and respect for administration in all communications, either privately with staff or in a group setting. Address any administrative issues directly and in private with those concerned. Time management: Routinely see patients in the time allotted, adjusting to daily patient flow issues to remain on schedule.Work productivity must demonstrate the ability to see a minimum caseload of 18-20 patients per day (8-hour shift), effectively work patients into their schedules as caseload demands and respond to a variety of unforeseen daily circumstances, including emergency care of patients. Reporting: FNPs report directly to the Medical Doctor and his/her designees, which include the Chief Executive Officer.Complete patients' charts and submit medical bills daily. Must be flexible, adapt to change, and serve as a role model for staff through interactions; should at times think outside the box, providing possible resolutions/solutions to issues/concerns, rather than simply pointing out a problem area.Comply with all HIPAA rules and regulations. POSITION SUMMARY
The Call Center Associate is responsible for ensuring a positive and effective experience for every patient who has telephone contact with Brighter Beginnings. They answer calls in a timely manner and identify and address patients’ needs. This requires exceptional critical thinking, customer service, organizational, and time management skills. Duties include, but are not limited to, receiving incoming calls, answering patients’ questions, sending patients’ messages to their medical providers, and all aspects of appointment scheduling. The Call Center Associate is expected to meet specific performance targets related to caller wait times, call handling time, high-quality customer service, and quality of telephone encounters created.

Required Qualifications:
• High School Diploma or GED
• Experience and/or interest in health care
• Bilingual in Spanish/English

Preferred Qualifications:

The Call Center Associate will be trained to be backup personnel for the Behavioral Health tasks below:
• Review ACCESS LINE referrals on ccLink. Create patient charts as needed, run insurance enrollment, and schedule intakes with BH providers.
• Support 4 BH provider schedules; Fill BH appointment slots on a daily basis
• Daily confirmation of patient appointments.
• Keep track of patients willing to have last-minute sessions (telehealth) and fill any last-minute cancellation slots.
• Review BH caseload for all 4 providers to ensure patients are staying on the calendar or closed out.
• Send close-out letters to patients as needed.
• Follow up on ROI requests for BH patients.
• Review PHQ-9 and GAD7 referrals to ensure BH intake and follow-up.
• Ensure measurements (GAD7, PHQ9, PRAPARE, CAGE-AID, Columbia) are documented in patient charts (using SMART FORM).
• Ensure consents and custody for minors are documented in the patient chart.
• Review BH referrals for a higher level of care and/or psychiatry. Work with the referrals team to ensure the patient is being connected with services.
• Review BH referrals for insurance support with the insurance team to ensure issues are resolved, and the patient is scheduled for BH sessions as needed.
• CARES 2.0 patients: ensure patients are informed their insurance only covers twice per month, and otherwise schedule with the insurance team to establish self-pay.
• Review BH referrals for case management and provide light case management, as well as refer any patients who qualify for the BB ECM program.
• Create/Send Google invites for BH Sessions (for all BH providers except Lila)
• Support in collateral contacts to outside providers
• Support with updating Excel and eCW tracking of BH services
• Other tasks as assigned.

This is a full-time Medical Assistant position based at our Community Health Clinic in Richmond, with twice montly at our Oakland site, and occasionatlly at the Antioch site. You will be directly serving the community, providing affordable family healthcare to individuals and families who need it most. POSITION SUMMARY
The Family Practice Physician (Physician/MD/DO) provides primary and continuing medical care for patients seen at the Brighter Beginnings Family Health Clinics (BBFHC). Physicians take account of physical, emotional and social factors when diagnosing illness and recommending the required treatment. Patients may be referred to hospital clinics for further assessment and possibly for treatment, working with the Brighter Beginnings Family Health Clinic providers to ensure quality outcomes.

Provide Holistic Care to Patients: Family physicians see patients at our Family Health Clinic sites as well as provide telehealth services when necessary or desired. Their job is to attend to a panel of patients as their assigned Primary Care Provider and provide comprehensive medical care. They order prescriptions or additional therapy for patients when necessary.Order and Interpret Diagnostic Tests: Family practice physicians request, perform, and interpret diagnostic and clinical tests. They analyze test images and results in light of their patient’s medical history to inform their examinations and make accurate diagnoses. Family physicians then explain test results to patients and implement a care plan.Advise Patients on Needed Health Changes: Patients look to BBFHC Family Medicine Physicians to provide expert advice concerning disease prevention. They may suggest critical changes in diet, activity levels, and hygiene. The lifestyle changes physicians recommend help patients improve their quality of life.Monitor Patient Progress and Records: Another critical activity for the BBFHC Family Practice physician is keeping accurate and detailed records that document their observations from medical exams. Over time, Physicians monitor patient progress and treatment responses. They continually evaluate their patients’ overall health and adjust individual treatments as necessary.Collaborate with Other Healthcare Professionals: Consulting with additional medical and nonmedical specialists about patient needs is another vital task for our physicians. Other healthcare professionals can offer different perspectives on client diagnoses and treatment options. Physicians commonly coordinate care with physical therapists, pharmacists, psychologists, nurses, and social workers. They also refer patients to other specialists when further diagnosis or treatment is needed.Professional Expectations:
