How you'll contribute
The PHP Therapist is responsible for delivering therapeutic services to individuals in the Partial Hospitalization Program, including group and individual therapy, discharge planning, and communication with interdisciplinary and external stakeholders. The role supports patients with psychiatric, emotional, and/or substance use disorders throughout their treatment journey.
Essential Functions:
Conduct individual and group therapy sessions to educate and treat patients experiencing psychological, emotional, or substance use issues.Actively participate in treatment planning, discharge, transition, and after-care processes.Provide family therapy sessions as needed to support continuity of care and reduce barriers to treatment.Support with patient vitals and urine drug screening (UDS) as needed.Communicate proactively with patients, families, and referral sources in accordance with HIPAA and 42 CFR Part 2.Collaborate consistently with interdisciplinary teams including physicians, UR, and nursing staff.Support UR team functions and provide treatment updates or documentation for authorizations.Complete required clinical paperwork including psychosocial assessments, Columbia Suicide Risk Scale, and progress notes.Respond appropriately to patient crises and participate in de-escalation procedures.Demonstrate knowledge of confidentiality, HIPAA, and clinical regulations (TJC, CARF, Medicaid, etc.).Assist care coordination with discharge planning and continuity of care.Complete all documentation thoroughly, accurately, and within facility timelines.Perform additional duties as assigned by leadership.

What we're looking for
Applicants should have a master's degree in social work or counseling. Additional requirements include:
Prior experience with psychiatric and chemical dependency patients. Current clinical or social work license as required by state regulations. CPR and De-escalation certification required or obtain within 30 days of hire. Current unencumbered clinical license per state of practice guidelines.

An Intensive Outpatient Program Therapist (IOP) who excels in this role:
Conducts individual and group therapy sessions to educate patients regarding psychological, emotional, or substance abuse problemsDisplays active involvement in treatment planning processDisplays active involvement in the discharge, transition, and after-care planning treatment processAs therapeutically necessary, family session counseling provides to patients to ensure appropriate communication and involvement of family members and support groupsActively communicates with clients, families, and outside referral sources, functioning within the scope of HIPAA and 42 CFR Part 2Demonstrates proactive communication with those involved with the patient's treatment, documenting all correspondence held with patients and other stakeholdersSupport with vital and UDS capture as neededOngoing and consistent collaboration with the interdisciplinary team including physicians, utilization review and nursing staff membersDemonstrates active communication with team membersEffectively communicates to UR on treatment and LOS issues, supporting in UR functions/authorizations/denials as needed and requestedCompletes appropriate paperwork for clients in a timely, accurate, and complete mannerCompletes required assessment paperwork including patient assessment scales, Columbia, psychosocial/LOC assessments (not applicable at all facilities), psychosocial/LOC updates, and assigned assessment paperworkDemonstrates appropriate crisis intervention and de-escalation skillsActs as member of team when a crisis occurs, assisting others to ensure patient safetyEnacts the code system per company policy and trainingCompletes accurate assessments of patients utilizing clinical skillsAssists care coordination department as requestedOther duties as assigned
