    BreakThru Services, LLC. is a DBHDS-licensed behavioral health organization providing Applied Behavior Analysis therapy, therapeutic consultation, and mental health skill-building services across Virginia, Delaware, and South Carolina. We are expanding our service offerings to include Guardianship and Conservator Services under a contract with the Virginia Department of Behavioral Health and Developmental Services (DBHDS), and we are seeking dedicated professionals to join our growing team. Our mission is to provide person-centered, dignity-affirming services to individuals with intellectual and developmental disabilities and their families.

    BreakThru Services is seeking a compassionate and detail-oriented Licensed Clinical Social Worker or equivalent QMHP-credentialed professional to serve as a Guardianship Coordinator. In this role, you will serve as the day-to-day guardian for individuals with intellectual and developmental disabilities who have been court-assigned to BreakThru Services. You will be responsible for conducting regular client visits, coordinating healthcare and community services, managing benefit applications, and ensuring the safety and welfare of individuals under your guardianship. This is a meaningful, high-impact role serving one of Virginia's most vulnerable populations.

    Key Responsibilities:

    Serve as the court-appointed guardian and/or conservator for individuals with IDD who have been assigned to BreakThru Services by the Virginia Circuit Court

    Conduct in-person client visits at a minimum monthly to assess living conditions, health status, social engagement, and overall wellbeing

    Make day-to-day guardian decisions regarding healthcare, personal welfare, daily activities, community participation, and safety on behalf of incapacitated individuals

    Coordinate benefit applications and enrollment including SSI, Medicaid, food assistance, housing assistance, and other entitlement programs

    Coordinate discharge planning for individuals transitioning from state hospitals, psychiatric facilities, or other institutional settings to community-based living

    Collaborate with support coordinators, physicians, residential providers, day programs, and family members to ensure continuity of care

    Maintain accurate and complete guardianship records including visit logs, benefit status, medical records, court documents, and financial records

    Prepare and submit annual reports to the Virginia Circuit Court in compliance with Virginia Code Title 64.2 requirements

    Report any concerns regarding abuse, neglect, or exploitation to Adult Protective Services and the Program Director immediately

    Attend court hearings, team meetings, and guardianship planning conferences as required

    Complete Virginia DARS Guardian Training and maintain certification as required by law


    Required Qualifications:


    Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Clinical Psychologist, or equivalent QMHP-level licensure in Virginia

    Minimum of 3 years of direct clinical or case management experience working with individuals with IDD, serious mental illness, or complex support needs

    Strong knowledge of Virginia social services systems, Medicaid waiver services, community resources, and disability support networks

    Completion of Virginia DARS Guardian Training (or willingness to complete within 30 days of hire)

    Excellent organizational and documentation skills — experience maintaining court-compliant records

    Valid Virginia driver's license and reliable personal transportation — field-based travel required

    Ability to pass a comprehensive background check, including criminal history and child/adult abuse registry clearances


    Preferred Qualifications:


    Prior experience serving as a guardian, conservator, or in a fiduciary capacity for incapacitated adults

    National Certified Guardian (NCG) credential or willingness to pursue certification

    Familiarity with Virginia Circuit Court guardianship filing procedures and annual reporting requirements

    Bilingual (English/Spanish) a plus

    Experience working with DBHDS-licensed providers or Virginia DD Medicaid waiver programs
