    POSITION SUMMARY:

    Supervises planning and administration of all nursing care working directly with assigned nursing staff in the provision and maintenance of high quality patient care. Responsible for organizing and managing the shift, guiding and supervising the nursing staff and assisting the Director of Nursing with administrative and personnel functions, assuming responsibility for the nursing service department in her absence. Maintains confidentiality of information regarding residents, families, health care personnel and Bowdle HealthCare Center.

    REQUIREMENTS AND QUALIFICATIONS:

    Education: Graduate of an accredited school of nursing.

    License/Certification: Current RN license from South Dakota State Board of Nursing.

    DUTIES AND RESPONSIBILITIES:

    1. Assume full responsibility for management of the nursing unit, working jointly with Director of Nursing to plan nursing care policies, procedures, and standards of patient care- making appropriate recommendations for improvement of health care in the facility.

    2. Assist Director of Nursing with performance evaluation for nursing staff, especially new staff prior to completion of probationary period. Provides documentation for Director of Nursing regarding needed counseling or disciplinary action making effective recommendations.

    3. Provides leadership and direction of nursing service personnel for professional orientation, growth, and development. Inspires team work on all shifts.

    4. Assume responsibility for coordinating each patient's medical and nursing regime; discussing patient’s condition and sharing pertinent information with others involved in patient’s care.

    5. Demonstrates ability to perform basic nursing process (observation, assessment, nursing diagnosis, planning of patient care, implementation of care plans, and evaluation of same).

    6. Communicates with family and health professionals regarding care to patients, provides educational and teaching efforts directed to patients and their families.

    7. Assumes responsibility for safety and comfort for both staff and patient and demonstrates regard for patients. Handles confidential data in a professional manner.

    8. Establish work assignments and maintain staffing pattern assisting Director of Nursing in making adjustments in schedule due to absence of scheduled employees, and other emergency situations.

    9. Fosters frequent and open communications with nursing staff and Director of Nursing.

    10. Demonstrates effective execution of physician orders, reporting of infectious and/or communicable diseases, and assists staff with incident, accident, and error reports as necessary.

    11. Assumes a positive leadership role among the staff by consistent willingness to demonstrate and update nursing care measures and by ability to recognize and evaluate the emotional climate of the nursing unit.

    12. Have a solid knowledge of legal aspects of nursing through appropriate assignment of nursing responsibilities and through teaching and supervision of staff.

    13. Ensures that physician orders for treatments, medications, and procedures are properly implemented on each shift worked.

    14. Display a positive attitude toward all departments and clinical areas to help coordinate smooth delivery of all resident care services. Encourage similar attitude among staff.

    15. Knowledge of emergency procedures, fire internal/external disaster plans, and participates in drills.

    16. Assumes responsibility for professional and self-development, attends in services and workshops to enrich personal knowledge, growth, and skill in providing clinical care for patients.

    POSITION SUMMARY:

    Organizes and conducts medically prescribed physical therapy programs with Bowdle Healthcare Center and its outreach services to restore function and prevent disability following disease, injury, or loss of a body part; helps the patient develop or restore function and achieve maximum level of independence while learning to live within the limits of their capabilities.

    REQUIREMENTS AND QUALIFICATIONS:

    Education: Doctorate in Physical Therapy from an accredited school of physical therapy.

    DUTIES AND RESPONSIBILITIES:

    Evaluates patient needs and develops realistic and appropriate short and long-term goals; discusses evaluation results and goals with patients, family, primary medical provider and other concerned persons as appropriate.

    Demonstrates ability to assess a clinical situation from various perspectives, considers alternatives and choose an appropriate course of action.

    Demonstrates creativity, skill and efficiency in planning, designing and implementing treatment programs, including home programs, for patients with all types of disabilities and multiple medical problems.

    POSITION SUMMARY:

    The Activity Aide assists the Activity Coordinator in providing meaningful activities designed to meet the interests, physical, spiritual, mental, and psychosocial well-being of each resident, and reports residents’ progress to Activity Coordinator. The Activity Aide will ensure the safety and overall well-being of all residents.

    REQUIREMENTS AND QUALIFICATIONS:

    Education: Eighth grade education or equivalent

    License/Certification: None upon hire, must attain CNA certification within 3 months of employment.

    Experience: None required. Experience in a recreation setting preferred.

    Personal Specifications: Must have a basic understanding of the needs of the ill elderly and have an empathetic approach to illness. Must have a willingness and an aptitude for working with a variety of people. Must have a basic knowledge of a variety of activities. Must exhibit the ability to give instruction to others and to lead. Must exhibit a spirit of cooperation and have a pleasant, friendly personality.

    DUTIES AND RESPONSIBILITIES:

    1. Provide one to one activities and sensory stimulation with passive non-alert and passive alert residents, confused residents, and residents who prefer not to participate in group activities.

    2. Leads group activities as directed by the Activity Coordinator. Assist in planning activities when requested. Performs other tasks within the activity department as directed by the Activity Coordinator.

    3. Provides coverage of the activity department when the Activity Coordinator is absent.

    4. Must be able to work courteously and cooperatively with residents, staff, and visitors. Reports to Activity Coordinator on resident’s responses and/or progress to specific approaches utilized. May be requested to maintain a written record of same.

    5. Must be able to work with older adults having limited capacities and various ailments.

    6. Will be required to keep all professional and medical information confidential. Abides by established policies of the facility and licensing agencies.

    7. Must be able to work flexible hours which will include some evenings and weekends. Is aware of the appropriate chain of command.

    8. Recognizes abnormal signs and symptoms which signify a physical change in condition which would require nursing intervention.

    9. Reports to Activity Coordinator on the functions of Activity Service under his/her direction.

    10. Attends required inservice programs and staff meetings.

    11. Aides Dietary department with meals in whatever capacity able.

    PHYSICAL ACTIVITY REQUIREMENTS:

    Primary Physical Requirements

    LIFT up to 10 lbs.: Frequently required to lift various supplies.

    LIFT over 50 lbs.: Occasionally to transfer resident from wheel chair with assistance of others and transfer belt.

    CARRY up to 10 lbs.: Frequently required to lift and carry supplies used for recreational activities.

    REACH above shoulder height: Required to reach cupboards or shelves for supplies.

    REACH at shoulder height: Required for various jobs directing or demonstrating activity.

    REACH below shoulder height: Required to assist resident in wheel chair.

    PUSH/PULL: Required to assist people in wheel chairs/Geri-chairs daily.

    Hand Manipulation

    Grasping: Required constantly while documenting and while working with hobbies, crafts, exercise, etc.

    Fingering: Required for day to day activities.

    Controls & Equipment: May be required to operate an automobile, audio/video equipment, and taking phone calls.

    Other Physical Considerations

    Bending: Frequently required while working with seated residents at table to monitor or assist progress of resident.
