Recreation Coordinator
JOB SUMMARY As part of the interdisciplinary team, the Recreation Coordinator assesses participants' needs, interests, and capabilities to develop personalized therapeutic recreation plans. This role involves planning, organizing, directing, and engaging in a comprehensive therapeutic recreation program designed to meet both the general and specific needs of frail elderly participants. Additionally, the Recreation Therapist provides leadership, supervision, and support to center staff.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:

Minimum Qualifications
Education, training, or experience sufficient to plan and implement activities that meet the needs of frail or elderly participants.Preferred Qualifications:
Education: Bachelor’s degree in Therapeutic Recreation, Recreation Therapy, or a Bachelor of Arts in Music with Board Certification (MT-BC) by the Certification Board for Music Therapists (CBMT).Experience:
Experience working as part of an interdisciplinary team in a hospital, nursing home, or community-based setting.1 year of experience working with a frail or elderly population. If this is not present, training will be provided upon hire (if applicable for the role).PRE-EMPLOYMENT REQUIREMENTS:

Licensed Practical Nurse (LPN)
JOB SUMMARY
Under the direction and supervision of the Clinic Practice Manager, administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone/telehealth. Provides education to the participant, and caregiver in regards to care plan, nursing treatments or medications. Communicates participant’s condition to primary caregivers, physician and interdisciplinary team.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
As delegated by Clinic Practice Manager performs basic nursing services in the PACE clinic within scope of practice.In Coordination with the Interdisciplinary Team and the RN Care Manager participates in planning and executing interventions in accordance with the participants plan of care.Performs the administration of prescribed medications, vaccinations, and treatments in accordance with nursing standards and documents appropriately.As designated by the Clinic Practice Manager and meeting the requirements within the scope of practice shall withdraw blood from participants as instructed.Maintains participants’ medical record and fulfills PACE ORGANIZATION charting and reporting requirements.Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians.Triages participants within scope of practice to determine priority of care.Educates participants and family/caregivers regarding medications and treatment instructions Prepares equipment and aids physician during treatment, examination, and testing of participants.Assists other staff in clinic in daily appointments and emergencies. Maintains timely flow of participants.Responds to and refers incoming telephone calls.Assists in the maintenance of the exam rooms for necessary supplies and materials. Ensures cleanliness. Ensure clinic supplies are on the reorder supply list as appropriate. The Executive Assistant to the C-Suite provides high-level administrative, operational, and strategic support to the Chief Executive Officer and Executive Leadership Team of a national Program of All-Inclusive Care for the Elderly (PACE) organization. This role serves as a trusted partner to senior leadership, ensuring seamless coordination of executive priorities, board governance activities, enterprise initiatives, and cross-functional communication across multiple states and PACE centers.
The ideal candidate is highly organized, proactive, politically astute, and capable of managing complex schedules, sensitive information, and competing priorities in a fast-paced healthcare environment. This individual must demonstrate exceptional judgment, professionalism, discretion, and the ability to anticipate executive needs while supporting organizational growth and operational excellence.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Executive Support
Manage complex calendars for the CEO and designated C-Suite leaders, prioritizing strategic initiatives, meetings, travel, and organizational commitments. Manage expense reports for C-Suite leaders in accordance with Travel and Expense policy and procedures.Manage outlook inbox for CEO and assigned C-Suite with the highest level of confidentiality. Healthcare experience strongly preferred. Experience supporting a multi-site or multi-state organization preferred. Experience working with Boards of Directors and governance activities preferred. Registered Dietitian
JOB SUMMARY
Assesses nutritional status of PACE participants. Uses pertinent data to plan and implement appropriate nutrition interventions and communicate the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. Functions as a member of the Interdisciplinary Team. Provides nutrition counseling, education, and therapy to participants; educates and counsels family and other caregivers regarding dietary issues; and conducts initial assessments and periodic reassessments. Participates in the development of the plan of care, assists in the coordination of 24-hour care delivery, and regularly informs the Interdisciplinary Team (IDT) of the nutritional needs of each participant. Provides education to other health care professionals as appropriate.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provide high quality nutritional care and services and serves as a member of the PACE interdisciplinary team (IDT) Conduct an initial comprehensive nutritional assessment upon enrollment and as needed according to PACE procedures regulations. Collaborate with the interdisciplinary team to develop a comprehensive care plan for each participant. Revise goals and approaches to participants’ care in coordination with interdisciplinary team. Provide nutritional counseling to participants, care givers and family members to promote health and wellbeing of participants. Facilitate nutrition-related care plan interventions, such as home delivered meals and supplements. Monitor weight, medical diagnosis, drug nutrient interactions, changes in chewing or swallowing, mood changes, hydration status, intake, skin breakdown and report changes to the interdisciplinary team with recommendations for changes. Works collaboratively with the interdisciplinary team (IDT) to develop and implement the comprehensive plans of care for participants. Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community. Participate in all interdisciplinary team meetings.Assist the interdisciplinary team members, contractors and center staff in understanding the significant nutritional status and needs related to the participant’s health problems. Incorporate cultural and ethnic preferences of participants into individual care plans. Provide direction and input into planning and implementation of nutritional services regarding the cultural and ethnic preferences of participants. Performs the duties of other IDT members based on professional licensing, competencies, and experience as needed. Performs the duties in the role of Food Service Supervisor as assigned. Actively participates in Quality Improvement projects / meetings. Participates in family meetings, staff meetings, in-service and training and orientation programs as required. Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines. Participant Experience Coordinator
JOB SUMMARY:
Responsible for the front desk in the lobby at the PACE center. Greet all participants, families and visitors that enter the center with complete customer service skills. Provide guidance and direction to all participants, families and visitors. Assist with check in of participants as they arrive with identification process and when they leave. Answer the telephone, screen and direct calls, take and relay messages and schedule appointments as requested by the Center Director, Clinic personnel. Monitor security system access alarms, call bell alarms and cameras. Prepare and process routine correspondence, file and store records for efficient retrieval. The Participant Experience Coordinator will assume independent work projects whenever possible in support of the operations of the Center. Is responsible for reporting identified safety issues such as hazardous environment.

Team Assistant
JOB SUMMARY: The Team Assistant plays a vital role in supporting the Interdisciplinary Team (IDT) to ensure operational efficiency and the seamless delivery of services. This position serves as a central point of communication and coordination, ensuring that participants receive timely updates and that all services are provided as ordered in their care plans. The Team Assistant fosters effective collaboration among team members, facilitates accurate documentation, and enhances the overall experience for participants by maintaining a high standard of organization, responsiveness, and professionalism.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Conducts daily chart audits to ensure that services ordered by their assigned IDT have been completed and followed up upon by the appropriate discipline. Attend IDT meetings, keep notes and facilitate as needed.Assist care team manager with grievance resolution and appeals.Participate in quality initiatives as assigned.Answer incoming calls from participants on their IDT panel answering their questions and helping with the participant needs to include but not limited to routing call to the appropriate discipline if available to take the call, taking a message with providing an anticipated return call timeframe and scheduling appointments with the requested IDT member using the scheduling feature in EMR.Assist with obtaining consultation, specialist and outside medical records as needed and enter into EMROther duties as assignedEXPERIENCE AND EDUCATION:

POSITION SUMMARY
The Center Support Aide provides non-clinical support to ensure the PACE center functions smoothly and safely each day. Assists with the setup, organization, and breakdown of personal care spaces, meal service operations, and common areas. Maintains a clean, orderly, and welcoming environment for participants, staff, and visitors. Supports center programming and daily workflows through supply restocking, room preparation, participant escorting within the center, and personal care assistance. Functions as a supportive team member by communicating environmental, medical or operational needs to appropriate staff to promote positive participant experience. Administers personal care assistance when needed to PPTs in Center.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Assist with the daily setup, organization, and breakdown of personal care rooms/center, including arranging tables, chairs, hydration stations, and maintaining supplies needed for meals and/or personal care. Provide non-clinical support during participant activities by escorting participants within the center under staff supervision, including personal care or transfers.Assist with meal service operations, including tray setup, table preparation, meal delivery support, and dining area cleanup. Assist with feeding needs of PPT’s when needed.Maintain cleanliness and order of common areas, including activity rooms, dining spaces, hydration stations, and shared equipment areas, closely following the Infection Control policies and procedures.Restock center supplies such as linens, PPE, personal care supplies, PPT clothing, sanitation supplies, and participant comfort items.Support transportation coordination by helping stage participants for departures and arrivals, escorting wheelchairs only.Monitor the center environment for safety concerns such as spills, broken equipment, cluttered walkways, or supply shortages and promptly report issues to leadership.Foster positive, respectful, and supportive relationships with participants, families, and team members through professional non-clinical interaction.Follow all organizational policies, confidentiality standards, and OSHA safety guideline sat all times.Perform other non-clinical duties as assigned to support the daily operations of the PACE center.Administer personal care such as toileting, showering, personal care, incontinence of care, and colostomy care, as designated within the scope of practice of state certification.Maintain required documentation for toileting, showering, laundry, and water temperatures.May take assignments to cover home care needs of participantsParticipates in IDT for PCA role as assigned by center leadershipEXPERIENCE AND EDUCATION:
Two (2) years of experience working in a hospital, nursing home or community-based setting is preferable. License/Certifications applicable per State requirements. NJ must have CHHA/CNA1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role). Position specific competencies for the Personal Care Specialist will be met prior to assuming participant care.PRE-EMPLOYMENT REQUIREMENTS:

Engagement Navigator
JOB SUMMARY
Under the direction of the Director of Outreach and Engagement (OED), the Engagement Navigator builds trust with prospects and families, assesses needs and program eligibility with care, and guides them through intake and enrollment to ensure participant success. The role involves clearly presenting the benefits of BoldAge PACE, maintaining accurate CRM and EMR documentation, and fostering seamless communication across outreach, clinical, and operational teams. Serving as a knowledgeable and confidential resource, the Engagement Navigator also analyzes data and feedback to strengthen the customer experience and program effectiveness. ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Implement the Outreach and Engagement Plan and associated activities as a member of the Outreach and Engagement DepartmentRespond to all communications as quickly, accurately, and thoroughly as possiblePrioritize providing service and solving problems in all interactions with community contacts and potential enrollees.Provide information about the program benefits, requirements, and eligibility to all interested individuals, groups, family members, and the communityParticipate in the enrollment assessment process to determine eligibility for PACE. Collaborate with other PACE staff and team members in the assessment and initial care planning process.Assist with the Medicaid eligibility determination process with the potential enrollee, their family, state Medicaid office, PACE business office.Coordinate with healthcare providers to obtain health-related records for potential enrollees.Obtain all necessary agreements, permissions, and consents from the potential enrollee. Represent the potential enrollee in meetings with IDT members communicating their needsFacilitate PACE enrollment with the potential enrollee and their families and caregivers, ensuring they understand and agree to the program enrollment.Support and guide the new participants through the first three (3) months of their PACE enrollment. Assist with communications with the interdisciplinary team, answer participants’ questions, address issues as needed.Maintain thorough documentation of intake and enrollment data and activitiesEstablish a “people first” approach in all client encounters and enrollment activities. Strive to exceed expectations for census enrollment targets working with the team to prioritize and execute enrollment activities.Demonstrate accountability for all intake systems and interactions.Participate in outreach activities as needed. Clerical Assistant Remote Per Diem
JOB SUMMARY:
The Clerical Assistant / Video Content Specialist is responsible for producing AI-generated video content using Synthesia and providing administrative support. This role involves creating, editing, and publishing training videos, onboarding materials, and internal communications through the Synthesia platform, while maintaining records and ensuring the secure handling of sensitive digital content. The position requires proficiency with AI video creation tools, strong attention to detail, organizational skills, and the ability to manage digital content production and general administrative tasks efficiently and independently.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Create, edit, and publish AI-generated videos using the Synthesia platform, including avatar-based presentations, training modules, and internal communications.Script and storyboard video content in collaboration with subject matter experts and department leads.Manage the Synthesia workspace, including organizing video libraries, updating templates, and maintaining brand consistency across all productions.Edit and refine video content for clarity, accuracy, and professional quality, incorporating feedback and revisions as needed.Maintain records of video productions, supporting tracking, reporting, and follow-up as needed.Ensure the secure and confidential handling of sensitive documents and digital content.Perform basic administrative duties such as data entry and document organization.Communicate effectively with internal team members regarding video project status, deadlines, and content timelines.Other duties as applicable/assigned.EXPERIENCE EDUCATION AND CERTIFICATIONS:

Social Worker
JOB SUMMARY:
The Social Worker (MSW) provides direct social work and care management services to participants of the PACE program, offering comprehensive support to enhance their quality of life and ability to live independently in the community. Responsibilities include participant screening, case management, counseling, and referrals, as well as planning, organizing, and implementing social work services tailored to participants and their families.
As a liaison between the interdisciplinary team, caregivers, and community agencies, the Social Worker ensures participant needs are effectively addressed, and services are coordinated seamlessly. This role is essential in promoting participant well-being, advocating for their needs, and advancing the mission and goals of the PACE program.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Participate in initial assessments, care planning, and periodic reassessments (at least every six months) of participant cases, ensuring assessments are completed prior to team meetings and adhere to departmental standards.Document participant changes and updates in the medical record accurately and in compliance with industry and departmental standards.Collaborate and communicate effectively with Interdisciplinary Team (IDT) members regarding participant status, needs, and care plans.Maintain up-to-date case management records, including periodic participant reassessments, and ensure comprehensive documentation.Coordinate 24-hour care delivery and provide individual and family counseling to support participants and caregivers.Develop and facilitate group counseling sessions and activities to address participant needs.Refer participants and families to appropriate community resources, acting as a liaison and advocate to ensure access to needed services.Foster effective relationships among team members, promoting team building and cohesion.Consult with staff on the impact of social, emotional, and cultural factors on participant health, and advise on available social services in the community.Participate in program and policy development for social work services and contribute to Quality Improvement (QI) processes.Conduct home visits to participants’ private residences to evaluate and support their needs.Actively participate in IDT meetings and contribute to care planning and team discussions.**NJ: Social work services which fall within the scope of practice defined by the Social Workers' Licensing Act of 1991 (N.J.S.A. 45:15BB-1 et seq.) and the New Jersey State Board of Social Work Examiners shall be provided by a social worker.Perform additional duties and special projects as assigned.EXPERIENCE AND EDUCATION:
Master’s degree in Social Work from an accredited school of social work accredited by the Council on Social Work Education.Member of the Academy of Certified Social Workers (ACSW) or other NASW-recognized certification preferred. Current License in the required state. Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting is preferable. 1 year of experience working with a frail or elderly population preferred.
