    PCP Nurse Practitioner

    JOB SUMMARY

    Delivering primary care and continuous healthcare services to PACE participants, utilizing the knowledge and skills required to assess, plan, and provide care in alignment with PACE Program standards. Works closely with the clinical care team to conduct assessments, reassessments, develop care plans, and deliver comprehensive primary care.

    ESSENTIAL DUTIES AND RESPONSIBILITIES: Provide high-quality primary care with a strong focus on individualized evaluation, starting with a comprehensive history and physical exam, rather than defaulting to broad diagnostic testing panels. Collaborate closely with the IDT before referring participants to specialists, ensuring referrals are necessary and that continuity of care is maintained. Collaborate with the Medical Director to manage specialist referrals, initiate appropriate treatment prior to referral when indicated, review specialist notes promptly, and develop and communicate participant care plans that incorporate clinical judgment and consult findings to ensure comprehensive care. Manage and monitor all of your participants' medications, aiming to exceed expectations in medication reconciliation, and medication interactions. This can be done in conjunction with the Medical Director and our collaborating Pharmacist vendor. Stay actively informed of participant status during hospitalizations and facility stays, providing appropriate guidance to ensure safe transitions and care continuity. Assume shared responsibility for all participants in the center, regardless of individual assignment, supporting the IDT model of team-based care. Participate in and contribute to daily IDT meetings, offering clinical insights and collaborating on participant-centered care plans. Ensure accurate, timely completion of all participant assessments and documentation in compliance with policy, regulatory, and quality standards. Review and respond to labs, imaging, and consults in a timely manner, incorporating findings into ongoing care plans and communicating changes with the IDT. Educate participants and caregivers on self-care strategies, medication use, and wellness practices, empowering them to participate in their own care. Use the Emergency Department judiciously, prioritizing PACE resources unless symptoms are potentially life-threatening. Accurately document diagnoses with attention to proper coding practices, supporting risk adjustment and quality reporting metrics. Provide coverage for the 24/7 on-call rotation, addressing participant needs promptly and ensuring proper hand-offs and communication. Contribute to discharge planning, supporting smooth transitions across care settings. Assist with staff training and mentoring, contributing to clinical policy development and best practice implementation. Participate in ongoing performance and quality improvement initiatives. Demonstrate flexibility by stepping into other roles, as allowed by licensure and experience, when needed to support the team. Maintain and report clinical resource needs to the Medical Director, supporting effective operations. Consistently uphold all safety, privacy, and compliance standards.
    EXPERIENCE AND EDUCATION: MD, DO, APRN, or PA NJ: Each nurse practitioner shall be so certified by the New Jersey State Board of Nursing. Each physician shall be licensed or authorized by the New Jersey State Board of Medical Examiners to practice medicine in the State of New Jersey. Current State license State CDS Certification and DEA Registration Board Certified in Geriatrics preferred BLS required (must have within 90 days of employment) 2 years experience providing direct patient care (preferred) 1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
    PRE-EMPLOYMENT REQUIREMENTS: Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance. Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.     Licensed Practical Nurse (LPN)

    JOB SUMMARY

    Under the direction and supervision of the Clinic Practice Manager, administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone/telehealth. Provides education to the participant, and caregiver in regards to care plan, nursing treatments or medications. Communicates participant's condition to primary caregivers, physician and interdisciplinary team.

    ESSENTIAL DUTIES AND RESPONSIBILITIES: As delegated by Clinic Practice Manager performs basic nursing services in the PACE clinic within scope of practice. In Coordination with the Interdisciplinary Team and the RN Care Manager participates in planning and executing interventions in accordance with the participants plan of care. Performs the administration of prescribed medications, vaccinations, and treatments in accordance with nursing standards and documents appropriately. As designated by the Clinic Practice Manager and meeting the requirements within the scope of practice shall withdraw blood from participants as instructed. Maintains participants' medical record and fulfills PACE ORGANIZATION charting and reporting requirements. Observes, records, and reports participant's condition and reaction to drugs and treatments to physicians. Triages participants within scope of practice to determine priority of care. Educates participants and family/caregivers regarding medications and treatment instructions Prepares equipment and aids physician during treatment, examination, and testing of participants. Assists other staff in clinic in daily appointments and emergencies. Maintains timely flow of participants. Responds to and refers incoming telephone calls. Assists in the maintenance of the exam rooms for necessary supplies and materials. Ensures cleanliness. Ensure clinic supplies are on the reorder supply list as appropriate. Responsible for monitoring expiration dates of medications and clinic supplies monthly as assigned. Effectively communicates in interdisciplinary team meetings, clinic meetings or any other meetings as assigned. May Assist in the scheduling of specialist appointments and assisting in hospital admissions and calling report to unit, as assigned or requested Follows all BoldAge Policies and Procedures and OSHA safety guidelines. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions. Maintains safe working environment. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.     EXPERIENCE AND EDUCATION: Graduate of an accredited school of nursing. One year working as an LPN strongly preferred. Current LPN/LVN license in good standing to practice in the required state. ( NJ: New Jersey State Board of Nursing). BLS required (must have within 90 days of employment) IV Certification preferred. 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring.

    Registered Nurse Care Manager

    SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. In collaboration with the interdisciplinary team (IDT), develops plans of care to meet participants' needs. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. Collaborates and communicates with the primary care provider, clinic staff, and other members of the IDT. Provides care to participants in the clinic, center, and participant homes as needed.

    ESSESNTIAL DUTIES AND RESPONSIBILITIES: Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT). Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record ). Participate in 24/7 "on-call" process for triage of participants and their needs. Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse. Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed. Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed. Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies. Notify participants of normal test results. Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements. Implement nursing-related care plan interventions. Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety. Review and revises goals and approaches to participants' care in coordination with participant , family, caregiver and interdisciplinary team. Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community. Evaluates the competence of CNAs and Home Care Aides and delegates tasks and duties to them as indicated. Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families.     Registered Dietician

    JOB SUMMARY

    Assesses nutritional status of PACE participants. Uses pertinent data to plan and implement appropriate nutrition interventions and communicate the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. Functions as a member of the Interdisciplinary Team. Provides nutrition counseling, education, and therapy to participants; educates and counsels family and other caregivers regarding dietary issues; and conducts initial assessments and periodic reassessments. Participates in the development of the plan of care, assists in the coordination of 24-hour care delivery, and regularly informs the Interdisciplinary Team (IDT) of the nutritional needs of each participant. Provides education to other health care professionals as appropriate.

    ESSENTIAL DUTIES AND RESPONSIBILITIES Provide high quality nutritional care and services and serves as a member of the PACE interdisciplinary team (IDT) Conduct an initial comprehensive nutritional assessment upon enrollment and as needed according to PACE procedures regulations. Collaborate with the interdisciplinary team to develop a comprehensive care plan for each participant. Revise goals and approaches to participants' care in coordination with interdisciplinary team. Provide nutritional counseling to participants, care givers and family members to promote health and wellbeing of participants. Facilitate nutrition-related care plan interventions, such as home delivered meals and supplements. Monitor weight, medical diagnosis, drug nutrient interactions, changes in chewing or swallowing, mood changes, hydration status, intake, skin breakdown and report changes to the interdisciplinary team with recommendations for changes. Works collaboratively with the interdisciplinary team (IDT) to develop and implement the comprehensive plans of care for participants. Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community. Participate in all interdisciplinary team meetings. Assist the interdisciplinary team members, contractors and center staff in understanding the significant nutritional status and needs related to the participant's health problems. Incorporate cultural and ethnic preferences of participants into individual care plans. Provide direction and input into planning and implementation of nutritional services regarding the cultural and ethnic preferences of participants. Performs the duties of other IDT members based on professional licensing, competencies, and experience as needed. Performs the duties in the role of Food Service Supervisor as assigned. Actively participates in Quality Improvement projects / meetings. Participates in family meetings, staff meetings, in-service and training and orientation programs as required. Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines. Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families. Documentation of care that reflects excellence as demonstrated by clear, concise, comprehensive data. Practices standard precautions and follows PACE Program Infection Control protocols. Position specific competencies for the Dietitian will be met prior to assuming participant care. NJ: Dietary counseling which falls outside of the scope of practice defined by the State of New Jersey Nursing Practice Act and the New Jersey State Board of Nursing shall be provided by a dietitian. Performs other duties as required or requested.
    EXPERIENCE AND EDUCATION Bachelor's degree from an accredited college with major studies in food, nutrition, or dietetics. Passing the Commission on Dietic Registration (CDR) exam. Minimum of 1 year working with the elderly population required. If this is not present, training will be provided upon hire (If applicable for the role). Minimum of two (2) years clinical nutrition experience preferred Serve Safe Certified preferred NJ: Each dietitian shall be registered or eligible for registration by the Commission on Dietetic Registration.

Ensure accurate, timely completion of all participant assessments and documentation in compliance with policy, regulatory, and quality standards. Review and respond to labs, imaging, and consults in a timely manner, incorporating findings into ongoing care plans and communicating changes with the IDT. Educate participants and caregivers on self-care strategies, medication use, and wellness practices, empowering them to participate in their own care. Use the Emergency Department judiciously, prioritizing PACE resources unless symptoms are potentially life-threatening. Accurately document diagnoses with attention to proper coding practices, supporting risk adjustment and quality reporting metrics. Provide coverage for the 24/7 on-call rotation, addressing participant needs promptly and ensuring proper hand-offs and communication. Contribute to discharge planning, supporting smooth transitions across care settings. Assist with staff training and mentoring, contributing to clinical policy development and best practice implementation. Participate in ongoing performance and quality improvement initiatives. Demonstrate flexibility by stepping into other roles, as allowed by licensure and experience, when needed to support the team. Maintain and report clinical resource needs to the Medical Director, supporting effective operations. Consistently uphold all safety, privacy, and compliance standards.

    EXPERIENCE AND EDUCATION: MD, DO, APRN, NP, or PA Current State license State CDS Certification and DEA Registration Board Certified in Geriatrics preferred BLS required (must have within 90 days of employment) 2 years experience providing direct patient care (preferred) 1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
