    Occupational Therapist (OT) - Madison, SD

    Blue Stone Therapy is seeking an Occupational Therapist (OT) to provide rehabilitation services to adult and geriatric patients at Bethel Lutheran Home in Madison, SD.


    This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Occupational Therapist is responsible for assessments, care plan development, and implementation of treatment programs designed to improve activities of daily living (ADLs), cognitive function, upper extremity strength, range of motion, positioning, and overall functional independence.


    This position will serve multiple partner communities, including United Living Community and Peaceful Pines, with travel between locations as needed.

    This leadership role is responsible for the daily operations, clinical oversight, staffing strategy, and overall performance of the rehabilitation department within a skilled nursing facility (SNF) or post-acute setting. The Rehab Director ensures the delivery of high-quality therapy services while driving operational efficiency, regulatory compliance, team engagement, and measurable patient outcomes.


    This position provides direct patient care as needed and is accountable for achieving productivity and utilization standards.

    This role focuses on delivering skilled occupational therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Occupational Therapy Assistant works under the supervision of a licensed Occupational Therapist to implement treatment plans designed to improve activities of daily living (ADLs), functional mobility, strength, cognition, and overall independence.

    Provide skilled occupational therapy treatment in accordance with established plans of careImplement individualized treatment programs focused on functional and measurable outcomesTreat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settingsMonitor patient response to treatment and communicate progress with the supervising Occupational TherapistMaintain timely, accurate documentation to support quality care and reimbursement standardsCollaborate with interdisciplinary team members and facility staff to support positive patient outcomesRequirements:

    Graduate of an accredited Occupational Therapy Assistant programCurrent Occupational Therapy Assistant license (or eligibility for licensure) in applicable stateExperience in skilled nursing, long-term care, or post-acute rehabilitation preferredStrong communication, organization, and clinical reasoning skillsCommitment to patient-centered care and professional standards
