NURSE PRACTITIONER (ADVANCED)
The Nurse Practitioner functioning under the auspices of the Medical/Clinical Department, is a medical staff provider with a high degree of expertise in the nursing care and management of geriatric patients. The Nurse Practitioner, NP, assumes major responsibility for assuring continuity of patient care at the clinic; home care settings; ambulatory, assisted living, and skilled nursing facilities. Responsibilities include collaboration with physicians, comfort care staff, as well as other care providers to ensure well-coordinated, interdisciplinary continuity of care. The Nurse Practitioner will provide care within his/her scope of practice as outlined by applicable state law, licensing, regulations, and Bienvivir Policy.
RESPONSIBILITIES:
Clinical Knowledge
1. Demonstrate knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions.
2. Is knowledgeable of care of those with delirium, dementia, falls or other mobility impairments, questionable capacity to make medical decisions, complex social and caregiver situations. Select mood disorders, and polypharmacy.
3. Demonstrates a high degree of clinical expertise in working with patients with acute and chronic illness.
4. Develops history and physical assessments, of participants, that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical problem.
5. Orders appropriate diagnostic studies and correctly interprets the results.
6. Utilizes critical thinking to synthesize and analyze collected data.
7. Develops, implements, evaluates, and adjusts the plan of care as needed in coordination with the workings and practice of the Interdisciplinary Team.
8. Manages general medical and surgical conditions based on knowledge of the indications, contraindications, side effects, interactions and adverse reactions of pharmacologic agents and other relevant treatment modalities.
9. Demonstrates proficiency in procedural skills pertinent to practice area.
10. Other duties as assigned by the Medical Director.
Patient Care
1. Provides care that is patient and family centered, compassionate, and appropriate in the care of Bienvivir population, as well as Comfort Care participants.
2. Provides initial participant evaluation and six-month re-assessments, treatment, prevention, care coordination and management of acute and chronic health conditions as appropriate for Bienvivir participants care.
3. Assesses participants during routine office visits.
4. Counsels and educates patients and families.
5. Incorporates the patient and family in care planning and treatment across settings.
6. Prescribes routine drug therapy in accordance with established protocol. Thoroughly explains drug benefits, side effects, and potential reactions.
7. Refers more complex cases to supervising Physician for consultation. Directs patients to Specialists as necessary.
8. Completes all medical care functions in compliance with established protocol and clinic policies. Strictly follows infection control procedures and safety regulations.
9. Leads and coordinates family conferences.
Communication and Interdisciplinary Collaboration
1. Communicates and collaborates effectively with Physicians, Home Health Comfort Care Team, members of the Interdisciplinary Team, Pharmacists, Specialists and Hospitalists.
2. Works effectively with the Interdisciplinary Team members.
3. Promotes mutual respect and trust working within a team model.
4. Communicates effectively with participants and families, incorporating gender, age, culture, race, religion, disabilities, and sexual orientation.
5. Ensures all required charts, documents, and files ae accurate, timely, and in compliance with clinic policies and standards.
QUALIFICATIONS / REQUIREMENTS:
1. Masters of Science in Nursing (MSN) with certification as Nurse Practitioner in Family, Geriatrics, Adult, or Acute Care.
2. Current State License.
3. Annual CPR certification.
4. Other certifications as required.

Under the supervision of the Clinic Manager, the LVN Medication Nurse assists in the daily operation of the medical component of the Adult Day Care Center. Although the primary focus will be medication administration, the LVN Medication Nurse will also be responsible for education and monitoring of the participant's health status in accordance with the plan of care developed by the Interdisciplinary Team as related to medications.
RESPONSIBILITIES:
1. Administration of medication as per physician orders.
2. Management of participant's medications to include all charting/documentation in the monthly medication administration record (MAR) as prescribed by the Texas Department of Human Services Day Activity Health Care Services regulations.
3. Monthly updating of computerized MARs.
4. Direct responsibility for ordering of all medications from pharmacy and for verifying that all orders are correct.
5. Provide education and counseling to participants and/or families/caregivers for appropriate administration and monitoring of medications at home.
6. Provide patient care as appropriate under the supervision of the Clinic Manager or the Physician.
7. Assist or substitute for treatment nurse as necessary.
8. Motivate participants to maximize independence and maintain functional level and encourage full participation in day center program.
9. Participate in morning meetings as necessary.
10. Performs other duties as assigned by the Clinic Manager.
QUALIFICATIONS / REQUIREMENTS:
EDUCATION / EXPERIENCE
1. A graduate of an accredited nursing program with a license to practice in the state of Texas as a Vocational Nurse.
2. One year of related experience giving medications, preferably to a frail or elderly population.

HOME HEALTH SKILLED NURSING LVN PRN
The HH Skilled Nursing LVN is primarily responsible for providing services to participants in their homes who require skilled nursing care. Nursing care is provided under the PCP’s orders and under supervision of a Registered Nurse. The HH Skilled Nursing LVN, as per the HH department needs, will assist with the HH LVN Coordinator duties under the supervision of the HH Nursing Supervisor.
RESPONSIBILITIES:
Direct Patient Care
1. Provides direct nursing care to participants in accordance with the Provider’s Plan of Care and orders, under the supervision of a Registered Nurse, and within the scope of practice defined by Texas HHSC and state licensure regulations.
2. Performs routine and as-needed nursing tasks assigned and permitted under HHSC.
3. Observes participants for changes in condition, safety concerns and changes in participants needs and promptly reports findings to the Registered Nurse, Provider and/or designee.
Assessment & Care Coordinator
1. Participates in ongoing evaluation of participant status and progress toward established goals.
2. Provides input to the Registered Nurse and Provider regarding the need for revisions to the Plan of Care based on participant response and outcomes.
3. Identifies when nursing and/or personal care services may no longer be medically necessary and communicates recommendations to the supervising Registered Nurse.
4. Participates in discharge planning in accordance with HHSC requirements.
Documentation & Regulatory Compliance
1. Completes accurate, timely, and thorough documentation of assessments, care provided, and participant and/or family responses to interventions in the medical record, in compliance with HHSC documentation standards.
2. Ensures all documentation is completed within required timeframes, including completion within 24 hours of participant encounters.
3. Completes and submits all required paperwork and reports in accordance with Bienvivir Home Health policies and regulatory requirements.
4. Submits end-of-shift email report to Home Health and clinic staff to report continuity of care detailing all interventions and resolutions rendered.
Interdisciplinary Team (IDT) Collaboration
1. Communicate effectively with members of the Interdisciplinary Team (IDT), including nursing, providers, and other healthcare professionals, to ensure coordinated, participant-centered care.
2. Identifies and reports the need for involvement of additional IDT members in accordance with participant needs and HHSC standards.
Participant & Family Education
1. Provides education/teaching to participants and family/caregivers regarding aspects of care, treatments, and safety measures, utilizing written and verbal instructions as appropriate and consistent with the Plan of Care.
Professional Standards & Agency Compliance
2. Demonstrates knowledge of and compliance with Bienvivir Home Health policies and HHSC regulations.
3. Maintains professional conduct and participates as an active member of the healthcare team.
4. Participates in required in-service education, competency validation, and staff meetings to maintain compliance with HHSC training and competency requirements.
5. Performs other duties as assigned by the Home Health Nursing Supervisor or HH Administrator.
6. Protects participant confidentiality in accordance with HIPAA.
QUALIFICATIONS / REQUIREMENTS:
1. Graduation from a State approved school of Nursing.
2. Currently licensed (in good standing) through the State Board of Texas as a Licensed Vocational Nurse.
