The Home Health Aide provides personal care services under the direction of the Registered Nurse or Therapist. The Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services for clients as necessary to maintain their personal comfort.
Essential Duties and Responsibilities
Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.Range of motion exercisesAssistance in ambulation or exercisesPerforms personal care activities, including but not limited to:BathingShampooingSkin care/nail careOral hygieneShavingDressingPerforms household services essential to health care at home, including but not limited to:Meal preparation/feedingLaundryLight housekeepingAssists in the administration of medications that are ordinarily self-administered under the direction and supervision of the Registered Nurse (per state nurse practice laws and Agency policy).Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse or Therapist. Follows reporting guidelines in the Care Plan.Documents care provided and completes the forms required for the client's records. Completes the appropriate records to document care given and pertinent observations. Submits documentation per Agency requirementsat least weekly.Promotes personal safety and a safe environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.Maintains confidentiality in all aspects of the job.Attends in-service programs to meet compliance requirements. Identifies learning needs to Agency supervisor.Performs other related duties and responsibilities as deemed necessary.Qualifications
Home Health Aide or Certified Nursing Assistant license in the state of Indiana required as obtained through successful completion of an approved program. (Required)Willingness to travel between Carmel, Indianapolis, and Zionsville. (Required)Licensed driver with automobile insured in accordance with state and/or Agency requirements. (Required)Preferred

Prepares residents for the day or night as necessary by assisting with bathing, dressing, and activities of daily living. a. Provides privacy during resident care. b. Applies supportive devices, braces, hearing aids, etc. c. Ensures that each resident receives oral care twice a day and hair grooming at least daily.Provides non-medicated skin care which requires reading an assignment sheet, obtaining skin care product(s), and application to the skin. Reports unusual signs or symptoms to the charge nurse, director of nursing, and/or the health center administrator.Assists residents with the following activities: a. Turning and positioning in bed at least every two (2) hours. b. Getting in and out of bed c. Walking d. Getting in and out of a chair or wheelchair e. Getting on and off a toilet or commode f. Getting in and out of a full tub or showerDistributes, removes, and cleans bedpans and urinals.Clean, sterilize, store, prepare, and issue dressing packs, treatment trays, and other suppliesServe and collect food trays and feed residents that require helpObserves and reports unusual symptoms, changes, accidents, and injuries to the charge nurse in a timely manner.Communicates verbally with residents while carrying out procedures. Assures anxious and disoriented residents to alleviate fears and to put them at ease.Responds to call lights by sight or sound in a timely manner and provides or obtains the necessary care.Other duties as assigned by the supervisor.Shift Details : Full Time, 7:00am - 7:00pm
Qualifications:
