    Qualifications

    Active Maryland license, permit, or internship status in good standing

    Reliable internet connection and private workspace required

    High School Diploma/GED + 2 years of experience OR Associates degree + 1 year in human services, behavioral health, healthcare support, or related field
    Experience working with at-risk youth, foster care, behavioral health, assisted living, residential care, or community-based populations preferred
    Strong communication, documentation, and interpersonal skills

    Job Type: Full-time / Part-time
    Salary Range: $20 - $25 per hour

    About the Role

    Wise Minds Therapeutic Services LLC is seeking compassionate, dependable, and motivated Community-Based One-to-One Support Staff to provide direct supervision, mentoring, behavioral support, and life skills coaching to youth receiving out-of-home and community-based services across Marylands Eastern Shore.

    This role focuses on ensuring the safety, stability, and emotional well-being of youth while supporting the development of social, coping, and independent living skills. Staff work closely with clinical leadership and care coordination teams to implement individualized service plans in home and community settings.

    Candidates with experience in behavioral health, assisted living, healthcare support, residential services, youth mentoring, or direct care are encouraged to apply.

    Key Duties and Responsibilities

    Direct Support & Supervision

    Provide one-to-one supervision and support to youth in home, school, and community settings
    Model appropriate social, coping, and communication skills
    Assist youth in developing independence, emotional regulation, and daily living skills
    Accompany youth to appointments, activities, and community-based programs as needed
    Maintain professional boundaries using a trauma-informed approach

    Behavioral & Clinical Support

    Implement individualized behavioral strategies based on service plans
    Participate in crisis prevention and de-escalation interventions
    Monitor and document behavioral progress and daily activities
    Communicate regularly with supervisors regarding youth progress and concerns
    Reinforce positive behaviors and support youth during emotional challenges

    Documentation & Compliance

    Complete service notes, progress reports, and timesheets accurately and timely
    Maintain confidentiality and comply with DHS, COMAR, and agency standards
    Participate in supervision, team meetings, and required trainings

    Requirements

    Must be 21 years or older with a valid drivers license and reliable transportation
    High School Diploma/GED + 2 years of experience OR Associates degree + 1 year in human services, behavioral health, healthcare support, or related field
    Experience working with at-risk youth, foster care, behavioral health, assisted living, residential care, or community-based populations preferred
    Strong communication, documentation, and interpersonal skills
    Must pass CJIS background check, CPS clearance, and sex offender registry screening

    Preferred Backgrounds & Experience

    Experience in the following roles is highly valued due to transferable skills in supervision, documentation, client support, crisis response, and community care:

    Certified Nursing Assistant (CNA)
    Geriatric Nursing Assistant (GNA)
    Medication Technician (Med Tech)
    Direct Support Professional (DSP)
    Residential Counselor
    Behavioral Health Technician
    Psychiatric Rehabilitation Staff
    Youth Mentor or Community Support Worker

    Training & Certification Requirements

    Staff are required to complete the following (preferred at hire, required upon onboarding):

    CPR and First Aid Certification
    Crisis Prevention & Intervention (CPI) or equivalent
    Trauma-Informed Care Training
    Mandated Reporter Training
    HIPAA / Confidentiality Compliance
    DHS-required onboarding and orientation

    Wise Minds provides support and reimbursement for required trainings.

    Ongoing Professional Development

    All staff must complete a minimum of two (2) in-service trainings annually, including topics such as:

    Child Safety
    De-escalation Techniques
    Youth Engagement Strategies
    Executive Skills Coaching

    Schedule & Compensation

    Flexible scheduling available, including day, evening, after-school, and weekend shifts based on youth needs

    $20 - $25 per hour based on experience and qualifications

    About Us

    Holistic Change, LLC is a licensed behavioral health organization in Maryland dedicated to supporting individuals in overcoming mental health and substance use challenges. We operate outpatient services and psychiatric rehabilitation programs (PRP) to empower adults and youth to achieve long-term stability and independence.

    Position Summary

    We are seeking a Rehabilitation Specialist to oversee our Psychiatric Rehabilitation Program (PRP). This is a key management position required by Maryland regulations. The Rehabilitation Specialist ensures program quality, supervises direct care staff, and supports clients in achieving their recovery goals.

    Key Responsibilities

    Provide leadership and oversight of PRP services in accordance with COMAR and BHA requirements.

    Supervise, train, and support PRP direct care staff.

    Review and approve individual rehabilitation plans and progress notes.

    Ensure compliance with state regulations, documentation standards, and quality assurance.

    Collaborate with clinical and community partners to enhance client outcomes.

    Minimum Requirements (per COMAR)

    Candidate must meet one of the following qualifications:

    Licensed Mental Health Professional (LCPC, LCSW-C, Psychologist, Psychiatrist, PMH-NP, etc.), OR

    CRC Certification from the Commission on Rehabilitation Counselor Certification, OR

    PRA Certification CPRP (adult) or CFRP (child/family).

    For PRP serving minors: At least 2 years of direct care experience with youth with a serious emotional disorder is also required.

    Schedule & Compensation

    Full-time or part-time (must meet COMAR staffing thresholds: 20 hrs/wk for <30 clients; 40 hrs/wk for 30 clients).

    Compensation:

    PRA/CRC certified candidates: $25 $35/hour (approx. $52,000 $72,000 annually)

    Licensed Mental Health Professionals (LCSW-C, LCPC, Psychologist, etc.): $55,000 $65,000 annually
