Job Summary Our organization is seeking a highly skilled and experienced Care Manager Supervisor to join our team. This role will be responsible for managing the initial review, triage, and assignment of patients to case managers, as well as coordinating all systems and services required for an organized, multidisciplinary, patient-centered care approach. The successful candidate will ensure quality, cost-effective care for the identified patient population, supervise staff in the department, and act as a resource and role model for staff members. The incumbent will also function as a contact person for patients, families, healthcare team members, community resources, and employees, ensuring adherence to hospitals and departmental policies and procedures.
