Rotating shifts Monday through Sunday (apply to holidays); shifts will be assigned monthly; shifts scheduled between 7:00 a.m. and 11:00 p.m. Based on clinic setting, daily shifts of 5, 8, or 16 hours may be arranged.
The proper and safe operation of an Emergency Room is an essential requirement for the licensing and maintenance of hospital facilities in Puerto Rico, as stipulated by the applicable regulations of the Department of Health. The Regulation to Govern the Nursing Profession in Puerto Rico (Regulation 9104) establishes the obligation for health institutions to maintain a sufficient and adequate staffing pattern aligned with the intensity of care and patient classification, in order to ensure safe, continuous, and high‐quality nursing services.
Likewise, the most recent public policies of the Commonwealth seek to establish minimum staffing levels per unit and reinforce the need for institutions to comply with adequate staffing in critical care areas such as emergency rooms. The proposed legislation regarding minimum staffing (P. del S. 1035) states that the government has the responsibility to ensure that hospitals have enough nursing personnel to safeguard the health and safety of patients, particularly in areas with high clinical demand.
Institutional Need:
The institution holds a Certificate of Need and Convenience (CNC) to operate within an established schedule that requires continuous coverage from Monday to Sunday. However, the nursing personnel currently available consists only of regular employees assigned to shifts from Monday to Friday, 7:00 a.m. to 11:00 p.m., which makes it impossible to meet the institutional obligation of operating during the authorized hours. Additionally, this limitation places the institution at operational risk due to:
Lack of available personnel for shift relief, Inability to cover scheduled or unexpected absences, Risk of operating below the minimum recommended level for a critical service.
Staffing Requirements for the Emergency Room:
To maintain an adequate level of care, the Emergency Room must operate with at least:
3 nurses in the 7:00 a.m. – 3:00 p.m. shift
1 nurse in the intermediate shift
3 nurses in the 3:00 p.m. – 11:00 p.m. shift
These needs correspond to patient flow, the complexity of the cases handled, and the safety and continuity-of-care standards required by the Department of Health and current public policy governing the nursing profession. The lack of sufficient personnel jeopardizes compliance with the principles of quality, access, and safety outlined in state regulations.
Responsibilities:
• Initial assessment and triage (prioritizing medical attention, measuring vital signs, among others).
• Administer intravenous, intramuscular, oral, and other medications according to medical orders.
• Monitor vital signs, neurological status, and response to treatment.
• Manage wounds, hemorrhages, fractures, burns, and other emergencies.
• Prepare procedures and assist the physician.
• Coordinate transfers when necessary.
• Accurately and promptly document all information related to evaluation, intervention, treatment, patient responses, and critical events.
• Comply with isolation protocols, infection control procedures, and safety measures.
• Verify the proper functioning of emergency medical equipment.
• Collaborate with supply inventory management.
• Provide emotional support to the patient and their family members.
• Perform shift handoff.
• Collaborate with specialist physicians.
• Manage adult and pediatric patients.

Requirements:
• Bachelor’s degree in nursing (BSN) and a minimum of 3 years of related experience is desirable.
• Experience in the Emergency Room.
• Active license as a Registered Nurse in Puerto Rico.
• Active professional nursing association membership.
• Valid Health Certificate.
• Law 300 Certificate.
• Advanced life support certifications (ACLS, PALS).
• Vaccination record.
• Criminal Background Check Certificate.
• Availability to work rotating schedules, weekends, holidays, and night shifts.
