    Description

    We are looking for an RN or LPN for a part-time role for evening weekends. The RN or LPN will typically work as a charge nurse if not assuming a supervisory role. As a charge nurse they take on a key role in providing a productive work environment, quality resident care, and front-line leadership. Reporting to the nursing supervisor, the charge nurse maintains unit morale and ensures daily compliance of Federal, State, and local regulations.

    Unit Leadership

    1. Responsible for the unit morale by providing a team based work environment through effective team leadership and a demonstrated positive attitude.

    2. Assumes a lead role in employee related issues such as discipline, performance evaluations, retention, and development of qualified personnel.

    3.Works with the Nursing Supervisor to ensure compliance of Federal, State, and local regulations that govern the long-term care industry.

    4. Develops and maintains a good working rapport with inter-departmental and cross-functional personnel.

    5. Facilitates unit communications by conducting regular meetings with staff personnel designed to identify and correct problem areas and/or the improvement of services.

    6. Responsible for the training and skills assessment of assigned nursing personnel. Ensures all unit members are knowledgeable and compliant with resident's rights and responsibilities.

    Nursing Duties

    1.Assists with the admission, transfer, and discharge of residents.

    2. Participates in preliminary resident needs assessments and in the development of a written plan of care.

    3. Performs daily resident visits to assess and evaluate resident status changes and nursing needs.

    4. Completes nursing documentation, medical forms, evaluations, and accident/incident reports in a timely manner.

    5. Administers medications and performs treatments as directed by the attending physician or nursing supervisor.

    6. Accountable for the proper maintenance of written procedures outlining the administration, storage, and control of medications and medical supplies.

    7. Consults the nursing supervisor and/or attending physician in the planning, treatment, and rehabilitation of assigned residents. Ensures proper notification of appropriate personnel when a change in the resident's condition occurs.

    8. Remains current of applicable regulations and procedures by professional publications and attending applicable seminars.

    9. Administer I.V. fluids, medications or Total Parenteral Nutrition as ordered by M.D.

    We are looking for a PRN MDS RN. The MDS Nurse is responsible for maintaining a complete and accurate MDS program, by ensuring completion of all facility, state and federal resident assessment forms (MDS) and assisting in the development and implementation of each resident's plan of care. The MDS Coordinator audits, and submits all required resident assessment forms (MDS) and necessary corrections of the MDS to the State of Ohio. Coordinates the RAI process; reviews care plans prior to care conferences; completes CAA assessments in a timely manner; updates ODH forms for survey; and continuously reviews residents' conditions for need to update MDS's
    Accountable for team productivity and the timely completion of delegated work assignments in accordance with established policies and standards. Participates and/or approves the evaluation of personnel. Assist in initiation, completion of MDS and other facility, state and federal required assessment forms.Coordinate residents' level of care and assessment schedules with other departments within the facility, therapy providers and consultants.Assist with the initiation and completion of residents' plan of care.Assist with staff education and development of facility forms and tools to ensure accurate assessment of residents' needs.Obtain, distribute and review Q.M.'s monthly from the State.Update PCC as need and assign passwords for staff.Monitor Nursing documentation ensure completeness and accuracy.
    Requirements

    Educational Attainment • Graduate of accredited RN School of Nursing. • Bachelor of Science in Nursing.

    Licensure/Certification • Current Ohio Nursing License. • Certification in Gerontology.

    Specialized Knowledge/Skills • Working knowledge of CMS and ODH regulations governing care of nursing home residents. Training in the completion of the MDS, RAP instrument and the correction procedure. Knowledge of Medicaid, Medicare and HMOs

    Kind & Length of Experience • 3-years Geriatric experience.
    3-years experience with all components of the MDS process, including submission to the State. Read Less
