    LPN-Nursing Unit Charge Nurse

    EDUCATION, QUALIFICATIONS AND CREDENTIALS

    Must possess a current Kentucky license as a Licensed Practical Nurse and have graduated from an accredited school of nursing.

    JOB ROLERecognize your role as part of the QAPI efforts of your organization.Attend trainings to build understanding and capacity to undertake QAPI work.Carry out QAPI roles and responsibilities as assigned.Follow established policies and procedures in support of QAPI efforts.Look for, and share with leadership, ideas for improvement in the organization.Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success.Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.ESSENTIAL JOB FUNCTIONSConsistently model and demonstrate excellence in customer service, with a constant focus on person-centered care/services delivery.Gives report to the oncoming shift and receives report from off-going shift nurse.Completes assignment sheet at beginning of each shift.Is responsible for administration of medications and treatments as ordered by the attending physician per facility policy.Administers feedings per facility policy.Maintains resident clinical records per facility policy and nursing standards of practice.Initiates, implements and revises the Resident comprehensive care plan and nursing assistant care plan as resident status requires.Monitors residents with unstable conditions, in accordance with physician orders.Monitors residents who have been involved in an incident, per facility policy.Notifies physician and resident/legal representative of resident condition per facility policy.Assists physicians in making rounds, as indicated.Reviews physician orders, medication and treatment administration records, as assigned per facility policy.Conducts head-to-toe skin assessments, per facility policy as assigned.Conducts resident admissions, transfers, and discharges, per facility policy.Assists with orientation of new personnel, as directed.Acts as a liaison between resident, resident's legal representative, staff attending physician and ancillary departments.Completes shift functions of the unit, as assigned and directed.Ensures that resident care assignments are completed in accordance with the plan of care.Supervises work performance of nurse aide floor staff including compliance with scheduled break and mealtimes. Immediately reports any and all allegations of abuse, neglect, exploitation or misappropriation of resident property as well as any injury of unknown etiology to the administrator or their designee, as appropriate.KNOWLEDGE, ABILITIES AND SKILLS

    Knowledge of current federal and state laws and regulations that apply to the practice of nursing in a long-term care setting. Has strong organizational and time management skills and the ability to prioritize responsibilities. Ability to make presentations before top management, public groups and staff. Ability to define problems, collect date, establish facts and draw valid conclusions. Ability to read, analyze and interpret journals, financial reports and legal documents. Ability to apply mathematical concepts such as fractions, percentages, ratios and proportions to practical situations.

    SPECIAL REQUIREMENTS

    Ability to work overtime as required. Ability to be available on an on-call basis. Ability to be summoned to the facility in an emergency (may be expected to report to the facility in an emergency).

    EDUCATION, QUALIFICATIONS AND CREDENTIALS

    High School diploma / GED required. Demonstrates knowledge of and ability to apply principles of customer service to a high degree. Demonstrates the ability to work effectively with all levels of personnel. Demonstrates the ability to utilize appropriate judgement in requesting assistance from licensed personnel. Ability to communicate well through speech, written word and body language to denote approval or displeasure. Ability to summarize and synthesize information and communicate the message to others. Ability to foster teamwork.


    JOB ROLE

    Learn and become proficient in the essential job functions of a PCA under the direction and supervision of the Assisted Living Nurse Manager.Attend trainings to build understanding and capacity to undertake QAPI work.Carry out QAPI roles and responsibilities as assigned.Follow established policies and procedures in support of QAPI efforts.Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.

    ESSENTIAL JOB FUNCTIONS

    Consistently model and demonstrate excellence in customer service, with a constant focus on person-centered care/services delivery.Delivers direct personal care tasks as described on each resident’s care plan and as directed per training standards.Maintains the residents’ rooms in a tidy, orderly condition with respect to residents’ individual rights and preferences.Serves residents’ meals and nourishments and collects meal trays when meal is completed.Transports residents to the dining rooms, activities, therapy department and/or beauty salon as needed per their request and/or schedule.Supply fresh ice, water and other refreshments as assigned.Completes all tasks as directed on the unit’s assignment sheet for the assigned shift.Reports any unusual occurrences (incidents), signs, symptoms, complaints or observations of the resident to the Nurse Manager. Reports any allegation of abuse, neglect, exploitation or misappropriation of resident property per facility policy.Follows the policies and procedures of the facility.Completes annual continuing education requirement on an annual basis.Attends staff meetings as directed.
