Essential Functions:
Intake/Assessment
Conduct individual intake interviews of program candidates as assigned.Present on clients interviewed at intake disposition meetings; provide alternative referrals for persons unable to use the program’s services.Vocational/Employment Services
Support 8-10 cases providing the services below:Develop an Individual Work Plan (IWP) and a contract for services to be provided with each client served.Co-facilitate job search preparation workshops for clients seeking employmentWork individually with clients in helping them prepare for and in assisting them in implementing their job search strategy.Provide ongoing support and assistance to clients in the process of obtaining employment (including via outside referral, job development, entitlements counseling and advocacy).Post-Employment Services

Applicants for the position of Supported Education Counselor will perform a full range of services that support the successful pursuit of educational goals identified by service recipients.
Because the onset of serious mental illnesses typically occurs during late adolescence or young adulthood, people often have had their educations, careers, interests, and other life pursuits interrupted. The program seeks to help people to identify and define their educational needs based on their vocational goals. The program also seeks to identify and create access to appropriate resources to meet the needs and mitigate the unique challenges encountered by service recipients. Through a personalized peer support relationship, counselors are expected to work with individuals to identify assets, challenges, and appropriate resources. Services are delivered in a blended model of on-site, mobile community-based and remote telehealth services where most services will be delivered individually.
In-person and telehealth group services may be delivered to service recipients as a complement to one-to-one service.
Respondents should demonstrate that they have experience and interest in providing mental health services to young adults and older adults. Greater consideration will be given to respondents with two years or more of relevant experience working in mental health, advocacy social services or educational settings.
Respondents should also have some experience in providing the services outlined below.
They will have experience with and/or be trained in an array of practical tools and their relevance to providing Supported Education services that endorses empowerment of the individual; this includes basic principles of a self- help recovery model, the basics of psychiatric rehabilitation, time and money management, development of a personal Wellness Recovery Action Plan (WRAP), peer support and peer wellness coaching, and trauma-informed, trauma-sensitive, and trauma-responsive care.
Essential Functions:
Assist individuals by providing academic and vocational education preparedness planning services that develop & define steps towards education goals (in measurable outcomes) that enhance self-reliance and greater community interdependence. Baltic Street Wellness Solutions, in collaboration with Richmond University Medical Center (RUMC), is seeking Peer Specialists to provide recovery-focused support in emergency and community settings to individuals in acute behavioral crisis.
Working within RUMC’s Comprehensive Psychiatric Emergency Program (CPEP) and Mobile Crisis Program, Peer Specialists use their lived experience with mental health recovery to engage individuals in crisis, foster hope, and connect them to community resources.
This role requires strong communication skills, flexibility, and the ability to build trust in high-intensity environments. Peer Specialists will collaborate with clinical teams, support self-advocacy, and uphold the highest standards of confidentiality and person-centered care.
Key Responsibilities
Engage with individuals primarily in community settings (Mobile Crisis Program) and the CPEP with compassionate peer support, based on lived experience with mental health recovery.Participate in initial assessments and engage with patients and families to build trust and foster recovery-oriented relationships.Identify and facilitate access to resources including entitlements, housing, benefits, and Baltic Street programs.Provide individual peer support using a trauma-informed, person-first approach.Facilitate peer-led support groups, wellness activities, and psychoeducational workshops that promote recovery and self-advocacy.Collaborate with participants to develop personalized recovery plans, including Advance Directives and Wellness Recovery Action Plans (WRAP).Act as a liaison between participants and their treatment teams, ensuring care is coordinated, non-coercive, and culturally responsive.Develop community asset-based mapping to help participants identify and utilize local supports.Support participants with community integration and transitions from respite centers, emergency rooms, hospitals.Use harm-reduction strategies and crisis de-escalation techniques to support participants experiencing distress.
Documentation and Team Support Requirements
Maintain accurate and timely documentation of all participant interactions, recovery plans, and referrals.Conduct chart reviews and maintain participant records in compliance with program standards.Participate in staff meetings, case conferences, supervision sessions, ongoing peer support training to uphold professional standards and administrative meetings to share insights and improve care approaches.Obtain participant consent in accordance with existing consent policy at RUMC before initiating services, in accordance with privacy laws and internal policies.Coordinate with clinical teams and support integrated care planning.Compliance and Reporting

Minimum Qualifications:
High School Diploma or GED required.Certified with the Academy of Peer Services or completion of certification within the first two months of employment.Experience in housing assistance, case management or human services.Familiarity with affordable housing initiatives such as Section 8 and public housing.Familiarity with housing software and databases, such as AWARDS.Knowledge of local housing laws and policies, including the Fair Housing Act.Ability to travel by mass transit, bus and subway, up to 6 rides in a day. The Adult Homes Initiative is designed to provide peer support services to people transitioning from adult homes into the community through individual and group peer support meetings. Employees will use their lived experience to bridge individuals from Adult Homes into their desired communities in the five boroughs.

The Adult Homes Initiative is designed to provide peer support services to people transitioning from adult homes into the community through individual and group peer support meetings. Employees will use their lived experience to bridge individuals from Adult Homes into their desired communities in the five boroughs.
Essential Duties and Responsibilities
The essential functions include, but are not limited to, the following:
Offer positive, supportive peer relationships that foster trust and hope for positive change, choice, wellness, recovery, and personal empowerment Present a positive example of a person with lived experience who has been successful at managing their own recovery by actively investing in/maintaining their personal wellness Provide one-on-one and/or group support and continued short-term-support services following relocation Teach community adjustment and wellness self-management skills Assist with developing a Wellness Recovery Action Plan (WRAP) Help identify and/or re-engage with family, friends, and other natural supports Provide education and resources that support wellness self-management, self-determination, and self-advocacy Continue with individuals after transition to identify and engage with a range of community-based services and supports Complete detailed written record of activities in a timely and accurate manner Report to and meet with supervisor on a regularly scheduled basis Attend training and meeting as required and assigned by supervisor, funder and BSAEH Strong understanding and practice of peer support, recovery, wellness, and related servicesPossess strong engagement, and oral and written communication skillsAbility to travel and work afternoons, evenings and weekends Peers may be required to cover different homes in the designated borough as neededPerform other duties as required and assignedMinimum Qualifications (Knowledge, Skills, and Abilities)

Peer Bridger candidates are current or former recipients of mental health services. Must have good office skills. Experience or training in Peer Advocacy is also required as well as basic knowledge of Self-Help Techniques and the ability to be patient and supportive with consumers. A knowledge of Community Mental Health Systems is a plus. Basic phone and computer skills are needed.
