    Employee Role: Private Duty Nurse (PDN)-LPN

    The Private Duty Nurse (PDN)-LPN is a member of the client care team that provides services directly, or acts as a placement agency, to deliver skilled nursing services to clients in their personal residences. The PDN LPN works under the supervision of the Clinical Manager.

    Essential Job Responsibilities:

    Evaluates and treats clients with appropriate level of skill according to client’s needs under the supervision of an RN, Clinical Manager.Assist with providing continuity of care and emotional needs of client and family.Resourceful in meeting client’s psychosocial needs.Verbal demonstration of client rights and responsibilities.Make decisions based on sound nursing judgements.Recognizes and copes with emergencies appropriately.Ensures proper management of client services.Demonstrates competent use of equipment.Appropriately communicates information relative to client care in accordance with agency policies.Observes the client’s general physical, emotional, and mental status, and reports all changes in the client’s conditions, family situation, incident, and occurrences during shift immediately to agency RN/CM or Customer Care Coordinator.Ensure all forms of communication with clients, families, and health plan providers are documented timely to promote continuity of care across all interdepartmental functions.Build relationships with clients to encourage compliance with care plans and client safety. Maintains accurate clinical and shift work documentations to update system in accordance with guidelines set forth by the agency in a comprehensive and timely manner. Manage client/contract satisfaction with the highest level of customer service standards, communication, and documentation.Adheres to infection control policies and exposure control plan as mandated. Monitor and maintain clinical report, including Infection and Occurrence/Complaint Reports.Adheres to and promotes safety and injury prevention measures in client and workplace settings.Maintain and improve skills through attendance with staff development programs and in-service education requirements.Participate in community outreach activities that promote the goals and objectives of the agency.Maintain Confidentiality in all professional areas and consider the needs of clients and their families, other employees, physicians, visitors, and all members of the community in an informative, courteous, and compassionate manner.Ensure all HIPAA regulations are adhered to.Follow Agency Policies and ProceduresAll other duties as assigned.

These activities will be part of the client’s plan of care and must be provided under a physician’s order.

    Essential Job Responsibilities:

    Implements and provides day-to-day Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) support to clients within their home as ordered by the physician’s plan of treatment and client’s plan of care.Appropriately assist clients with activities of daily living and personal care in accordance with the care plan including:Assisting or performing personal care including: Baths (tub, shower, bed). Mouth care, denture care, skin care, foot care (no toenail cutting). Grooming (shampoo, comb hair, shave, file nails). Dressing and undressing activities.Elimination (toilet, commode, urinal, bedpan).Transfers.Assisting with ambulation with or without mechanical aids.Assisting the client with therapy exercises under the direction of a Registered TherapistAppropriately assist with household tasks directly related to the client’s health and in accordance with the care plan:Maintenance of a clean, safe, and healthy environment.Planning and preparing meals according to specific dietary requirements of the client, and if necessary, feeding the client.Light housekeeping of client’s immediate environment.Laundry of clients bedding and clothing.Assistance in completing activities such as shopping/grocery, errands, and appointments outside of the home.Providing companionship and stimulation for the client including reading, walks, etc.; and accompanying the client to doctors or other appointments.Utilize equipment in an appropriate and safe manner.Remind client for self-administration of medication.Do Not Administer Medications. Medications include eye drops, ear drops, ointments, pills, suppositories, injections, herbal remedies, and over-the-counter medications such as Tylenol, Aspirin, vitamins, and minerals.Accurately documents shift details and services accurately within established time parameters.Ensures the safety of clients and self.     Qualifications:

    Education: Graduate of an accredited school of nursing.Licensure / Certification: Licensed practical nurse (LPN) who is licensed by the North Carolina Board of Nursing (NCBON). Must have a current, unencumbered license as an LPN, or an Advanced Practice Nurse. Must be in good standing in the applicable state of practice. Must maintain all state and federal compliance requirements for licensure and employment.Travel: Travel is required for business purposes. Employees must have a valid driver’s license issued by the state in which they work and a satisfactory driving record.Work Experience: Minimum of one year of home care experience. One additional year of nursing experience, preferably in a hospital, is required for conducting applicable training classes.Other Hours of Work: Varies on a case-by-case basis. Full time is at least 30 hours a week from Saturday-Friday.Skills and Additional Requirements:A specific understanding of geographical/cultural requirements of branch office is preferred. Must be customer service oriented and prepared to liaise with patients, their families and other care providers.Ability to use computerized systems.Satisfactory verbal and written communication skills.Ability to work well under pressure.Bi-lingual preferred Read Less
