Responsibilities:Conduct comprehensive patient assessments and develop individualized care plans for hospice patientsAdminister medications, treatments, and pain management interventions in accordance with physician ordersProvide skilled nursing visits to patients in home, facility, or inpatient settingsEducate and support patients and families on disease progression, comfort care, and end-of-life planningCollaborate with interdisciplinary team including physicians, social workers, chaplains, and aidesDocument patient care accurately and timely using electronic medical records (EMR)Respond to after-hours on-call needs and urgent patient concerns as requiredRequirements:Active California Registered Nurse (RN) license in good standingMinimum 1 year of nursing experience; hospice, palliative care, or home health experience preferredStrong knowledge of pain management, symptom control, and comfort-focused careCurrent CPR/BLS certificationValid driver's license and reliable transportation for patient visitsProficiency with electronic medical records (EMR) systemsExceptional communication, empathy, and interpersonal skillsAbility to work independently and as part of a collaborative care teamAbout Us:Aura One Hospice is a trusted provider of compassionate end-of-life care serving the Palm Desert, CA community and surrounding areas.
